6 Dementia diagnosis Benefits of early dementia diagnosis. Dementia is one of the health conditions that people are most frightened of Planning for the future. Early diagnosis increases opportunity for patients to discuss the future with healthcare professionals and consider how to stay independent Getting treatments for dementia - Dementia is not a single condition Secondary prevention- Treating high blood pressure, high cholesterol and poorly controlled diabetes is important, as is smoking cessation and keeping to a healthy weight Accessing Post diagnostic support New integrated post diagnostic support service provided by the Alzheimer s Society next month.

7 Dementia in Numbers Diagnosis of Dementia from 60.5% in April 2016 to 64.1% at the end of March 2017 we are now at 66.9% which equates to 3,020 patients diagnosed on our practices registers with dementia. This meant more of our patients benefitted from receiving personalised care planning tailored to their needs 850,000 people in the UK have dementia Multiply this by 2, 3 or 4 for numbers of people affected Costs an estimated cost 26 billion per annum 25% of people in hospital have dementia People with dementia stay 7 days longer in hospital 70% of care home residents have dementia 33% of people who die, die with dementia Dementia is the most feared illness in people over 50.

9 Our population 22.6% of the population are over 65 higher than the England average In the next ten years, 19,000 more people over 65 and 3,715 of these will be over 85

10 In ELR we have better than average life expectancy Health is generally better than overall population of England A significant number of people affected by ill health, including GP-diagnosed coronary heart disease (10,564 people), hypertension (49,493 people), and diabetes (17,494 people) There were 2,967 deaths per year, on average, of patients registered with one of our member practices; cancer, CVD and respiratory disease accounted for 69% of these deaths.

18 2016/2017 Accounts Our accounts were scrutinised and our auditors opinion said that our financial statements give a true and fair view that, in all material respects, the expenditure and income have been applied to the purposes intended by Parliament our accounts been properly prepared in accordance with the accounting policies directed by the Secretary of State we have proper arrangements for securing economy, efficiency and effectiveness The full set of our annual accounts is available within the annual report on the CCG website.

25 Urgent Care We have a complex service The CCG have been reviewing Acute Access coverage across ELR and LLR Acute access services are currently provided at by GP practices and Urgent Care centres There is clearly a need for on the day health care access and new models of delivering this Single / Joint practice delivery New model of care focussed on simplicity and improving access Integration with all parts of the Urgent Care System Builds on what we do well and have now that works.

26 Cancer One in two people will develop cancer at some point in their lives, according to the most accurate forecast to date from Cancer Research UK, and published in the British Journal of Cancer. Thanks to research, the UK s cancer survival has doubled over the last 40 years and around half of patients now survive the disease for more than 10 years.

27 1 year survival higher than national average (ELR CCG received an award from The All Party Parliamentary Group on Cancer for one of the most improved cancer survival rates ). Screening rates are higher than national average in breast / cervical / bowel. Cancer services are a truly multi-disciplinary service. Working with the 2 neighbouring CCGs Public health England, UHL, Macmillan Cancer charity and Patient Groups.

28 NICE Cancer referral guidance Primary care is gearing up - PRISM pathways, education event Practice Learning Time. New decision making tools - Cancer Maps Secondary Care now seeing per week cancer referrals. Improved cancer performance New ways of working straight to test CT Colon/ Thyroid nodules ultrasound NHS England 2020 A World Class Service challenge will be quicker /more local / more effective to match and then better the worlds best cancer services.

29 Mental health and learning disabilities We have continued to maintain waiting time targets in Improving Access to Psychological Therapies (IAPT) Recovery rates for IAPT has been very positive during 2016/17. They are 6% higher than the national average Significant partnership work with Leicestershire Partnership Trust (LPT) helped to reduce our out of area mental health placements during the year Transforming Care Partnership Female Psychiatric Intensive Care Unit (PICU).

30 Looking ahead These are just some of the highlights of last year We have made some good progress during 2016/17 and need to continue to do so, against what will be another financially challenging year We ll be heavily involved in the 5 year Forward View and STP as we work towards delivering a simplified and integrated care system We recognise that we will also have to make some difficult commissioning decisions, but patient safety and quality will remain at the centre of everything we do.
