Frank Office: CHS E Office Hurs: 11:00 t 11:50 n Thursdays befre class and by appintment Cntact Inf: jcfrank@ucla.edu Curse Descriptin This curse fcuses n evidence-based prgrams t prmte healthy aging: preventin, health prmtin, disease self-management, and self-care prgram mdels. T understand the public health cntext f healthy aging, we will review: the scial determinants f lder adult health; risk factrs fr late life health prblems and threats t independence; health behavir change and health educatin mdels; health status and health behavirs f lder adults; behaviral science theries relevant t prmting health fr lder adults; an expanded view f preventin as applied t lder adult health pprtunities t prmte healthy aging thrugh evidence-based self-management and preventin prgrams. Self-management tls, such as gal setting and actin plan mnitring, will be used by students t develp and track their wn healthy behavir imprvement prgram. Health educatin change prcesses, such as mtivatinal interviewing, will be demnstrated and practiced thrugh in-class exercises. Students will select an evidence-based health prgram area fr lder adults t review in depth. Based n class size, prjects may be cmpleted using a grup frmat. This class is designed fr graduate students; undergraduate gerntlgy minr students are als welcme t enrll. Health fcused self-imprvement Plan 10 pint maximum (10% f curse grade): Since we will be studying healthy aging behavirs and explring health behavir change prgrams, each student will identify a persnal health imprvement gal and a set f activities t reach the gal. Cmplete a health self-assessment: G t Cmplete the nline Hw Healthy are Yu self-assessment. Click n Chapter 1; then Self-Assessment in the side bar. Cmplete fr yurself yu d nt need t have yur assessment ed t me. the assessment t yurself r write dwn yur scres. The self-assessment may assist yu in identifying a behavir change gal t wrk n during the class. Based n yur health self-assessment and persnal gals, yu will develp yur health imprvement plan using SMART bjectives during class n January 15. We will utilize tls frm health behavir change prgrams and track ur persnal prgress during the quarter. Participatin in this activity by applying what we are learning t this persnal gal will cunt fr 10% f the curse grade. Please keep yur weekly tracking reprt up t date and be ready t discuss prgress, barriers and/r insights during class as indicated. D nt select a self-imprvement plan that yu are nt cmfrtable discussing with thers in class. Due dates fr the six tracking reprts are indicated n the weekly class schedule belw. 2. Interview assignment 15 pints maximum (15% f curse grade) due January 22, 2015: Each student is expected t spend at least 30 minutes interviewing an lder persn (can be smene yu knw r a stranger). An lder persn is smene wh is 65 years f age r lder. Interview cntent shuld include: What des being healthy, mean [t them]? What activities d they d t keep themselves healthy? Have they dne these activities all their lives r are these mre recent additins?

If they have ne r mre chrnic (nging) cnditin(s), what d they d t manage it/them? What advice wuld they give a yung persn t stay healthy? Please write up a shrt (maximum tw pages, duble-spaced, 12 pint fnt) summary f yur interview using the utlined questins abve; als include the date f the interview and length f time f the interview the relatinship (if any) with the persn interviewed, Please adhere t length f assignment infrmatin prvided in additin t the 2-page limit will nt be cnsidered in grading. Fifteen percent f the curse grade is assigned fr the interview and write up. Mid-term Exam 20 pints maximum (20% curse grade): Questins will address majr key pints based n reading, presentatins and lecture materials. The exam will include shrt essay and multiple chice questins and will be given in class n February 5, Review f evidence-based health prmtin and disease management prgrams fr lder adults 25 pints maximum (25% f curse grade). Students will select a prgram area frm the list prvided in class and write a prgram area review paper and make a presentatin in class. Students may be asked t wrk tgether in a prgram area grup depending n class size. Outline fr Written Review: What is the prblem area the prgram(s) are addressing? Hw imprtant is this prblem t lder adults? (NOTE: please d nt spend mre than ne-half page n this bullet cntent) Please list the key AA/ACL listed evidence-based prgrams that address the prblem (see

Prvide verview f each prgram s features (frmat [grup, individual] rganizatin [# sessins, length f prgram], characteristics f grup leaders [as applicable], characteristics f lder peple prgram is apprpriate fr [pririty audience], ther features yu believe are imprtant t share What are the prgram s utcmes (benefits t participants)? Describe the reach f the prgram as translated t cmmunity settings (hw widespread is its availability, what pririty audiences des it reach, what types f agencies are ffering it)? If yu had funding t bring ne f these prgrams t a new (nt yet reached) audience, which prgram wuld yu select, what new audience wuld yu identify and why? What are the prgram s limitatins d yu have any cncerns abut the prgram(s)? Prvide references in the paper and a reference list that yu used fr this review. Use APA reference style. The reference list can be n an additinal separate page (page 5) f the review. Written prgram review: Maximum f 4 pages duble spaced and 12 pint fnt. Please limit the prblem area cntent (bullet ne abve) t ne-half page r less. The majrity f the paper shuld be abut the prgram(s) and fcus n the remaining bullets abve. In class presentatin: Please review the abve in a 15 t 20 minute presentatin. Class members shuld cme prepared t ask questins and engage in a discussin f the prgram area. 5. Minute Papers: T encurage cmpletin f the assigned readings, there will be tw unannunced Minute Paper assignments during the quarter. During class, each student will be given up t 5 minutes t write a summary f the selected reading and its relevance t the curse subject. Each Minute Paper will be 5 pints, r 5% f the grade. The tw papers taken tgether will be a maximum f 10 pints, r 10% f the curse grade. If a student is absent fr a class that includes a Minute Paper assignment, a makeup time will be arranged. 6. Reflectin Paper: Please write a maximum f 2 pages, duble spaced, 12 pint fnt, n the key learnings frm the class and hw what yu learned in the subject area may influence yur future wrk in public health r yur selected field.
