COPD Chronic Obstructive Pulmonary Disease is a serious lung disease that over time, makes it hard to breath. You may have also heard COPD called by other names such as emphysema or chronic bronchitis. While an estimated 24 million Americans suffer from COPD, as many as half remain undiagnosed. But, with early diagnosis and treatment, people with COPD can improve their quality of life and begin to breathe a little easier. As part of National COPD Awareness Month in November, if you or someone you care for may be affected by COPD, make an appointment with your family physician or a pulmonologist. Fortunately, pulmonary rehabilitation can help individuals with COPD manage their condition and improve their quality of life. One-on-one and group rehabilitation by a licensed respiratory therapist follows a plan of care tailored to each patient s particular needs and goals and includes self-care education, physical reconditioning and maintenance planning. While flu spreads every year, the timing, severity, and length of the season varies from one year to another. While it is not possible to predict what this year s flu season will be like, one fact that is certain is that the flu, or influenza, is contagious. Those who are at higher risk include children under 5 years of age, people over age 65, pregnant women, those with other chronic medical conditions, especially lung and heart disease, obesity, and the immunocompromised. The Centers for Disease Control and Prevention (CDC) recommends a yearly flu vaccine for everyone 6 months of age and older as the first and most important step in protecting against this serious disease. People should begin getting vaccinated soon after flu vaccine becomes available, if possible by October, to ensure that as many people as possible are protected before flu season begins. However, as long as flu viruses are circulating in the community, it s not too late to get vaccinated.

3 All New Jersey residents, especially those in at-risk groups, are urged to get an annual flu shot as influenza activity typically increases as flu season kicks in. Flu symptoms include fever, cough, sore throat, runny or stuffy nose, muscle or body aches, headaches, and tiredness. Some people may have vomiting and diarrhea although this is more common in children. It s important to note that not everyone with flu will have a fever. Most people who get the flu recover in a few days to less than two weeks but some can develop serious complications such as pneumonia, which can be life threatening. Pneumonia, bronchitis, sinus and ear infections are all examples of complications from flu. In addition to getting a seasonal flu vaccine if you have not already gotten vaccinated, take preventive actions like staying away from sick people and washing your hands to reduce the spread of germs. If you are sick with flu, stay home from work or school to prevent spreading flu to others. To schedule an appointment, call # # # Poor Quality Sleep Can Increase the Risk of Depression By: Dr. David Goldstein On Sunday, November 1, we ll all set our clocks back an hour to mark the end of Daylight Saving Time. Sunrise and sunset will be about an hour earlier than the day before, and it will be brighter in the morning for our daily commutes to work. How time changes actually affect you depends on your own personal health, sleep habits, and lifestyle. In general, "losing" an hour in the spring is more difficult to adjust to than "gaining" an hour in the fall. It is similar to airplane travel; traveling east we lose time. An "earlier" bedtime may cause difficulty falling asleep and increased wakefulness during the early part of the night. Going west, we fall asleep easily but may have a difficult time waking. There s no doubt that sleep difficulties affect our overall health and wellbeing. Obstructive sleep apnea, for example, is a leading cause of excessive daytime sleepiness. A chronic disorder which affects an estimated 25 million Americans, obstructive sleep apnea happens when you experience pauses in breathing or when

4 you develop shallow breaths while you are asleep. These pauses may occur many times during the night. When your breathing pauses or becomes shallow, you ll often move out of deep sleep and into light sleep. As a result, the quality of your sleep is poor, which makes you tired during the day. A recent study in the September issue of the Journal of Clinical Sleep Medicine reports that people with sleep apnea are at an increased risk for depression. Continuous positive airway pressure (CPAP) therapy, may ease their depression, the new study suggests. The study followed men and women who were newly diagnosed with obstructive sleep apnea. Nearly 73 percent had depression when the study began. The severity of the apnea influences the degree of the depression. However, after three months, only four percent of the 228 apnea patients who used CPAP for an average of at least five hours a night still had clinically significant symptoms of depression. People with symptoms of depression should be screened for obstructive sleep apnea by looking for complaints of snoring, breathing pauses while sleeping, or daytime sleepiness. Untreated sleep apnea also increases the risk of high blood pressure, heart disease, stroke, and type 2 diabetes, according to the American Academy of Sleep Medicine. If diagnosed, management of obstructive sleep apnea, including CPAP therapy, has been proven to successfully treat the disorder in many people. If you find yourself uttering these words, know this: many so-called miracle weight loss supplements and foods (including teas and coffees) don t live up to their claims. Worse, they can cause serious harm, say FDA regulators. The agency has found hundreds of products that are marketed as dietary supplements but actually contain hidden active ingredients

5 (components that make a medicine effective against a specific illness) contained in prescription drugs, unsafe ingredients that were in drugs that have been removed from the market, or compounds that have not been adequately studied in humans. For example, the FDA has found weight-loss products tainted with the prescription drug ingredient sibutramine. This ingredient was in an FDA-approved drug which was removed from the market because it caused heart problems and strokes. The FDA also found some weight-loss products, marketed as supplements, in fact contained dangerous concoctions of hidden ingredients used to treat high blood pressure and even depression. (Most recently, the FDA discovered products marketed as dietary supplements containing fluoxetine, the active ingredient found in Prozac.) Another product contained triamterene, a powerful diuretic (sometimes known as water pills ) that can have serious side-effects and should only be used under the supervision of a health care professional. Many of these tainted products are imported, sold online, and heavily promoted on social media sites. Some can also be found on store shelves. And if you re about to take what you think of as natural dietary supplements--such as bee pollen or garcinia cambogia - be aware the FDA has found some of these products also contain hidden active ingredients contained in prescription drugs. Under the Federal Food, Drug and Cosmetics Act, dietary supplement firms do not need FDA approval prior to marketing their products. It is each company s responsibility to make sure its products are safe and that any claims made are scientifically proven. The FDA has received reports of harm associated with the use of weight loss products, including increased blood pressure, heart palpitations, stroke, seizure and even death. It has issued over 30 public notifications and recalled seven tainted weight loss products in To help people with long-term weight management, the FDA has approved prescription drugs such as Belviq, Qysmia, and Contrave, but these products are intended for people at least 18 years of age who: Have a body mass index (BMI, a standard measure of body fat) of 30 or greater (considered obese) Have a BMI of 27 or greater (considered overweight) and have at least one other weight-related health condition Prescription drugs are not an answer for people who want to lose just a few pounds. Moreover, if you are going to embark on any type of weight control campaign, always talk to your healthcare professional about it first. Your doctor will also consider your health history, possible side effects and the potential interaction of weight-loss drugs with other medications you're taking.

6 When combined with a low-calorie diet and regular exercise, weight-loss drugs produce an average weight loss of 5 to 10 percent of total body weight within a year, which is a typical weight-loss goal. Diet and exercise are responsible for part of this weight loss, and medications are responsible for part as well. Even modest weight reduction can help to decrease blood pressure, lipid levels and blood glucose levels as well as increase insulin sensitivity. It's important to keep in mind that these medications may not work for everyone. And when you stop taking them, you could regain the weight you lost. If extra weight, obesity or morbid obesity is impacting your health and quality of life, consult your physician or schedule an appointment with a bariatric surgeon to discuss your options for more permanent weight loss solutions.
