1 Community Outreach Activity Summary July 2013 July 2014 Identified Prevention & Issues Education Lung Multi-disciplinary team of medical staff addresses lung CT scan survival research American Lung Association s Freedom From Smoking classes at Ingalls Wellness Center Smoking cessation info distributed via Aware screening programs, at community and Occupational Health health fairs/events Comments Meets monthly Offered 2-3 times per year Continuous Follow Up/Outcome Process developed to assure that patients found with lung lesions, even incidentally from CT scans, are flagged for physician review & follow-up One session completed in Six registered, 3 attended, 2 quit during the 8- week session. One session scheduled in October 2014 Any Smoking cessation callers to the Ingalls Wellness Center are offered enrollment in the Illinois Tobacco Quitline or the Lung USA toll free number. Prostate Referrals to and enrollment in the Illinois Tobacco Quitline Annual Family Health Fair Continuous pamphlets and anatomy models Active smokers who participate in screening telephone consultations are enrolled in or referred to call the Quitline. 25 referrals made since Oct 2013 Approx 700 community residents were exposed to education Breast US TOO speaker and support group meetings BreastAware held in cooperation with Support Center in Homewood Ongoing website risk assessment Six professional speakers, held every other month; about 15 in attendance each program Participants directed to schedule mammo African American Prominent More than 300 women

2 Breast Awareness Forum, Breast Self Exam Demonstrations Obesity KidFit camp, Sept. thru Nov Healthy Lifestyles through Ingalls Wellness Center Height, Weight, BMI, Waist Circumference, Nutrition services at health fairs WeightAware Healthy Habits through Ingalls Wellness Center Developed Community Garden on top of parking garage Survivorship US TOO prostate cancer; speakers and support Breast cancer support group Faye s Light cancer comfort services speakers on issues specific to their community Health Fairs throughout the year Nutrition and fitness program for children week program of nutrition and fitness education for adults 4-week program guides participants to achieve Healthy Goals, Eating, Moves & Partnerships Grantsubsidized classes on an overview of major health risks Plants grew in summer months. Groups held every two months in Homewood Held at Tinley Park FCC Ingalls sponsors a partnership to benefit our patients attend this annual event FIVE overweight children lost weight and learned to appreciate physical activity Bi-annual program for approx participants each time Dietitian follows up on high risk results Dietitian completes follow up on highest risk participants Wellness Nurse recommends programs & screenings as appropriate Fresh food provided to community pantries Attendance varies from Massage, reiki and other services provided to inpatients and outpatients free of charge

3 General Awareness Physical Activity Oncologist James Wallace spoke to Mallwalkers Oct Employee Wellness & Walking programs Gave a general overview on cancer risks and prevention IMH providing incentives. 75 seniors were in attendance 100 s of Employees are exercising.

Results are shared with the participant and their physician. More than 200 kits distributed; only a small number are returned Dispensed 100 kits Breast Silver Lining Foundation, and Ingalls partnerships to provide free mammograms and follow up services FY 13 Mammos and F/U services 203 Cornerstone Breast and cervical program (same as above) FY screening mammograms Rotary fair offering clinical breast exam Surgeons and NPs offer free exams 61 exams completed; Breast nurse navigator follows up Mammogram promo at Homewood Health Fair 30 women scheduled Received a free Bath & Body coupon
