Skills and Knowledge on Overdose Prevention Bill Matthews, RPA-C Bethany Medley, MSW Bruce Trigg, MD

Harm Reduction Coalition POLICY & ADVOCACY TRAINING & CAPACITY BUILDING OVERDOSE PREVENTION & ADVOCACY NATIONAL & REGIONAL CONFERENCES RESOURCES & PUBLICATIONS Harm Reduction Coalition creates spaces for dialogue and action that help heal the harms caused by racialized drug policies.

What is naloxone (Narcan)? A medicine that stops the effects of opioids causing an overdose o Restoring breathing and consciousness in the person up who is overdosing in minutes Pushes opioids off the receptors and blocks them for minutes o Can cause sudden withdrawal (unpleasant feeling) in people who are opioid dependent Has no effects on alcohol or other drugs (non-opioids) but does no harm if an opioid is not present

Formulations

How to Respond to an Overdose 1. Recognize signs of an overdose - Check for a response 2. Respond: Call 911/administer naloxone - Recovery position 3. Provide resuscitation - Rescue breathing 4. Follow-up 5. Report and get a refill

Recognize the signs of an overdose Unresponsive Shallow or no breathing Snoring or gurgling Turning blue around nail beds or lips

7 Try shake and shout to wake them up Check for a Response If no response, grind knuckles into their chest bone (sternal rub) for 5-10 seconds If the person still does not respond, they could be experiencing an OD

8 Respond: Call 911/administer naloxone Call 911 My friend is unresponsive and not breathing. And Administer naloxone Do first whichever is closer at the time, then do the other one!

Give Naloxone (Narcan) 1. Peel 2. Place 3. Press Peel back the package to remove the device. Hold the device with your thumb on the bottom of the plunger and 2 fingers on the nozzle. Do NOT press the plunger. 4. Repeat After 2-3 minutes if there is no or minimal response, repeat with second device into other nostril. Place and hold the tip of the nozzle in either nostril until your fingers touch the bottom of the person s nose. Once the tip is in the nostril, press the plunger firmly to release the dose into the person s nose.

Put person in a supported position on their side to prevent choking on vomit: o Any time you have to leave the person alone (even after administering naloxone) Recovery Position o Person responds to naloxone and is not okay

How to give rescue breathing 11 Tilt the head back, pinch nose Start with 2 quick breaths oif the chest doesn t rise, reposition head, check mouth for food, gum, etc. and try again Then give one breath every 5 seconds ogive normal sized breaths

12 Follow-up: After naloxone is used Once the person wakes up: o Explain that you used naloxone o Encourage them to seek medical attention o The person may be in withdrawal, explain this is only temporary o Tell them not to take any more drugs because that could cause another overdose

EMS not contacted? If the person awakens but is not fully alert (walking and talking), call 911 or take them to ER yourself If person feels okay or is refusing medical care, still stay with them for 3 hours; longer if a long acting opioid was taken (methadone) 13

14 Report and Get Refill Report use of naloxone to dispensing agency o Anonymous report of date, place, drugs used and outcome Get a refill of naloxone (even if you use one dose) o If overdose rescue kit is lost or confiscated o If naloxone is nearing expiration date

15 Overdose Rescue Kit

Naloxone Access in NYS Pharmacy o With or without an individual prescription o Naloxone Copayment Assistance Program covers the copayment cost up to $40 o Must have health insurance with prescription coverage Opioid Overdose Prevention Programs o Registered programs with NYS DOH o Train and distribute free naloxone rescue kits o No individual health insurance needed

Key Resources NYS DOH: Harm Reduction Coalition: Bethany Medley, MSW:
