2 What are Novel Psychoactive Substances and Club Drugs and why does Cambridgeshire need a strategy? The Advisory Council for the Misuse of Drugs define Novel Psychoactive Substances as psychoactive drugs which are not prohibited by the United Nations Single Convention on Narcotic Drugs or by the Misuse of Drugs Act 1971, and which people in the UK are seeking for intoxicant use. These substances, which are legally available to purchase, align with the following four categories: 1) Products with names which give no indication of what they contain; 2) Named and specific substances which are designed to be similar chemically and/or pharmacologically to known specific, controlled drugs; 3) Substances related to medicines; and 4) Herbal or fungal materials or their extracts (ACMD 2011). As many of the Novel Psychoactive Substances which were formerly considered to be legal highs are now controlled under the Misuse of Drugs Act 1971 and do not align with the definition above, the term Club Drugs is also included within the strategy as there are considerable risks associated with the use of substances that are relatively new to the UK drugs market that will be incorporated within the scope of the partnership response. The term Club Drugs also enables consideration of the harms and risks associated with the new and more established psychoactive substances that are controlled under the Misuse of Drugs Act The National Treatment Agency for Substance Misuse defines Club Drugs as a collective term for a number of different substances typically used by young people in bars and nightclubs, at concerts and parties. These drugs can be harmful and heavy use can develop into a dependency. (NTA, 2012). The United Nations Office on Drugs and Crime (UNODC) World Drug Report 2013 identified the numbers of year olds in the UK who have taken a Novel Psychoactive Substance to be estimated at 670,000 which equates to 8.2% of the year population and represents the highest use in Europe. There has also been a significant increase in the availability of Novel Psychoactive Substances as evidenced by the European Monitoring Centre for Drugs and Drug Addictions European Drug Report 2013 which reported that 280 new psychoactive substances were identified between 2005 and Although many of the Novel Psychoactive Substances are legal in status, this does not render them safe to use with immediate, short term and longer term risks unknown due to their relatively short period of availability. Novel Psychoactive Substances and traditional Club Drugs which are controlled under the Misuse of Drugs Act 1971 are also causing national concern due to the consumption of substances which users believe to be established club drugs but which contain other substances resulting in an adverse effect on health and, in some cases, death. There are also significant health related issues associated with established club drugs which are only now being identified due to treatment presentations experiencing longer term issues which have not been observed before. The non-traditional supply routes for Novel Psychoactive Substances present significant challenges for partnerships in relation to establishing local patterns and trends as many purchases are made via the internet and bypass traditional routes of supply. There are concerns in relation to purchases of Novel Psychoactive Substances through websites, which have been found to contain substances that are controlled under the MODA 1971 when the purchaser believed that they were purchasing a non-controlled substance and, therefore, committing a criminal offence.
3 Prevalence of Novel Psychoactive Substances and Club Drugs use within Cambridgeshire The Novel Psychoactive Substances and Club Drugs strategy is a new concept for Cambridgeshire and has originated from a position whereby there are pockets of local activity across the UK, but limited strategic oversight at a local, regional or national level in relation to this area of substance use. There is limited local data available at present to provide a clear indication of need within Cambridgeshire s communities. Novel Psychoactive Substances have been in existence for a relatively short period of time when compared to other traditional drugs and, as such, data collation systems have not been set up to record information. This is inclusive of both local and national data systems. For example, the historical TOPS form used in treatment services records the use of Novel Psychoactive Substances under the category of other which does not allow for detailed information to be ascertained from this source. From the limited information available, it is evident that there is activity in relation to Novel Psychoactive Substances and Club Drugs within Cambridgeshire although a true reflection of prevalence is currently unavailable. It is clear from nationally reported data that the demographics of Novel Psychoactive Substances and Club Drugs users differ in multiple ways to the traditional drug treatment population and as such, a different approach to responding to this type of substance use will need to be adopted in order to achieve success with consideration given to novel ways of taking action in accordance with funding limitations. Due to the need for a proactive response to nationally observed patterns and trends in Novel Psychoactive Substances and Club Drugs use, this strategy has been produced as a flexible framework for ensuring a coordinated countywide response, inclusive of the need for enhanced data collection to inform needs assessment and delivery, and is based upon some of the key recommendations within the ACMD report: Consideration of the Novel Psychoactive Substances ( legal highs ) (2011).
4 Priorities and Aims The partnership has agreed four strategic aims for Cambridgeshire underpinned by one overarching priority. These are aligned to the Cambridgeshire DAAT Strategic Priorities for which are: Delivering new approaches to drug and alcohol treatment and social reintegration. Preventing harm to children, young people and families affected by drug and alcohol misuse. Protecting communities through robust enforcement to tackle drug supply, drug and alcohol related crime and anti social behaviour. The strategic aims are also aligned with the Cambridgeshire Constabulary Substance Misuse Strategy (Drugs, Alcohol and Prescription medication) which states: Cambridgeshire Constabulary play a lead role in enforcement activity; significant and essential roles in preventing harm and communication; and can assist in appropriate diversions into treatment and social re-integration. The Constabulary will continue to do all it can to reduce the harm caused by drugs. The force will take account of new issues and tackle them in a way that is proportionate and consistent. Part of that approach is to ensure that our policies and tactics are adapted to respond to new and emerging threats and the offending behaviour. Drug Experts will continue to work closely with Trading Standards to support enforcement work and share intelligence with regard to new psychoactive substances. Overarching Priority: To work in partnership with stakeholders to plan, implement and deliver an appropriate and effective response to Novel Psychoactive Substance and Club Drugs within Cambridgeshire, incorporating demand and supply reduction initiatives, in order to raise awareness and reduce harm to Cambridgeshire s communities with a particular emphasis on young people and those in vulnerable groups. Aim 1) To increase the DAAT partnership understanding of Novel Psychoactive Substances and Club Drugs use within Cambridgeshire through the delivery of countywide training programmes and the development of robust multi-agency strategic and operational communication pathways to ensure the effective collation, recording and sharing of information in relation to Novel Psychoactive Substances and Club Drugs. Aim 2) To increase awareness and understanding of the risks associated with the use of Novel Psychoactive Substances and Club Drugs across Cambridgeshire s communities through the use of appropriate public health awareness campaigns and initiatives. Aim 3) To plan, implement and deliver appropriate and effective demand reduction responses to the use of Novel Psychoactive Substances and Club Drugs across community and custodial adult and young people s treatment services in accordance with need, which emphasise the risks of use and associated health issues when used alone or as part of a poly-substance using repertoire, evidence positive outcomes for individuals and demonstrate good value for money.
5 Aim 4) To utilise existing countywide supply reduction measures to increase knowledge and understanding of Novel Psychoactive Substances and Club Drugs patterns and trends across Cambridgeshire and Peterborough including anti-social behaviour as a consequence of use and official responses to suppliers of substances not controlled under the Misuse of Drugs Act 1971 located within the county using the full force of the law to tackle the Novel Psychoactive Substances trade. Expected Outcomes 2014/15 The focus of 2014/15 will be to establish a baseline in relation to local need which will be utilised to inform the operational and strategic direction of activity in relation to Novel Psychoactive Substances and Club Drugs in subsequent years. 1) Baseline established in relation to local prevalence of Novel Psychoactive Substances and Club Drugs within Cambridgeshire. 2) Increased awareness and understanding of Novel Psychoactive Substances and Club Drugs use and associated harms across the DAAT partnership. 3) Increased public awareness and understanding across Cambridgeshire s communities of Novel Psychoactive Substances and Club Drugs use, associated harms and where to access advice, information and support. 4) Increased staff confidence across all services to identify and respond to Novel Psychoactive Substances and Club Drugs use on presentation. 5) Accessibility and availability of effective, evidence-based Novel Psychoactive Substances and Club Drugs interventions within community and custodial adult and young people s treatment services ensuring effective and efficient use of partnerships and resources.
6 Ownership and delivery The strategy falls under the remittance of Cambridgeshire Drug and Alcohol Commissioning Group (DACG) who hold strategic responsibility for commissioning, monitoring and service governance; this group reports into the DAAT Executive Group. The successful delivery of this strategy requires a partnership approach. We expect that a wide range of partners will proactively engage in this work. The strategy is supported by an action plan that sets out objectives, accountability and milestones for planning, implementation and delivery. These objectives will be led by specific agencies and regularly monitored by the DACG. There will be an established Steering Group which will progress the objectives within the strategy. This data will be used to inform the direction and focus of the strategy over time. As such, the initial strategic document is intended as a framework to underpin a consistent and coordinated response across the partnership to Novel Psychoactive Substances and Club Drugs use and will be adjusted in accordance with local data once this is available.
