Subject Number POST Did you get a hepatitis C test? Did you get your test results back? No (skip second questions) No Please identify the following items as true, false or don t know. There is a vaccine (shot) that can prevent people from getting hepatitis C. Hepatitis C is a sexually transmitted infection Hepatitis C is the most common blood borne infection in the U.S. People can get hepatitis C from kissing on the lips. People can get hepatitis C from sharing needles. People with hepatitis C can look and feel fine. True It is OK for people with hepatitis C to drink alcohol. Hepatitis C can cause liver cancer. False Don t Know For each statement that I will read to you, pick strongly disagree, disagree, neither agree or disagree, agree or strongly agree. In terms of your choice for hepatitis C testing.. It was the right decision. or 1
I regret the choice that was made. I would go for the same choice if I had to do it over again. The choice did me a lot of harm. The decision was a wise one. Since you have had your health education session I would like to ask you a few questions about hepatitis C. or I am likely to get hepatitis C. I am more likely to get hepatitis C compared to other people at my age. Finding time to get a hepatitis C test would be difficult. Getting the hepatitis C test would be easy for me Hepatitis C is a serious disease. or It is important to get tested for hepatitis C if you are at high risk. C with my doctor or health care professional. C with my friends and family. C with my partner/sex partner. I can explain to my doctor or health care professional why I did or did not choose to get the hepatitis C test. 2
I did what my family thought I should do about hepatitis C testing. I did what my doctor or health professional thought I should do about hepatitis C testing. Preventive Health Model Prompt to respond strongly disagree, disagree, neither agree or disagree, agree or strongly agree. or Hepatitis C testing makes sense. Hepatitis C testing can help protect my health. Hepatitis C testing is important. I will be just as healthy if I don t get a hepatitis C test. Hepatitis C testing is an effective way to detect hepatitis C. There is a treatment for hepatitis C. The earlier treatment is started for hepatitis C the more effective it is. The next 5 questions are for those who went through decision counseling ONLY. Thinking back to when you made your decision about hepatitis C testing or I was scared of being told I have hepatitis C. I didn t want to know if I have hepatitis C. I thought that testing for hepatitis C may lead to more tests. 3
I thought that if I am meant to get hepatitis C I will get it. I thought blood tests are uncomfortable. Now I would like to ask you a few questions about the health education session you attended. or I learned a lot from the health education session. The information that I learned about hepatitis C surprised me. The health education session was worth my time. I understood what was talked about (the information) at the health education session. The health educator was respectful of my thoughts. The health educator knew what she was talking about. I could ask questions during the health education session and feel comfortable. Overall I really liked the health education session. The health education session would be helpful for other health decisions. I learned enough about hepatitis C testing that I feel comfortable about my decision to test or not test. The educational book was helpful. It was helpful to have information to take home. 4
It was convenient to have the health education session at my doctor s appointment. The location of the health education session was convenient. I would make time to have health education sessions before my doctor appointments. Was the health education session. Did you share the book with other people? Rank the following four ways of getting health information with 1 being the best way to get health information, and 4 being the least favored. Is there another method that you prefer to use to get health information? Too short The right amount of time Too long No For those who did not test for hepatitis C (for any reason) Why did you not get a hepatitis C test? Online Health brochure or booklet Talking with a health educator Talking with your doctor or health care professional For those who DID test for hepatitis C (for any reason) Why DID you get a hepatitis C test? Thank you for your participation in this study. 5
