1 The Resolution of the 2 nd International Conference "Risk Factors for Human Health: Current Challenges and Innovative Solutions" - "Risk Factors 2017" Conference participants leading international (Russia, Kyrgyzstan, UK, Austria, Israel) and Kazakhstan scientists and experts on risk factors management, delegates of executive and representative state bodies of RoK, international organizations, businesses, NGOs and mass media having heard and discussed reports, note that significant demographic changes and epidemiological shifts as well as increasing scarcity of resources, rapid technological development and rapidly growing demands of people create new challenges for the health system of most countries of the world. To that end, health protection, investments in health gain particular significance and become an important tool for promoting country s economic development and achieving social well-being. Today, chronic non-communicable diseases pose a serious threat to the health of the world's population. They are an important cause of premature mortality and, since they also lead to disability, they significantly affect the duration of a full-fledged life. According to the WHO, 38 million people die from non-communicable diseases every year, including cardiovascular diseases lead to the majority of deaths million people. These are followed by oncological diseases million, respiratory diseases - 4 million and diabetes million. 1 During the years of independence, Kazakhstan managed to achieve a 6% reduction in overall mortality. 2 However, in the overall mortality structure in the Republic of Kazakhstan the leading cause is non-infectious diseases 3 : circulatory system diseases , carcinology 12.1% (of which about 17 thousand people die each year from this, of which 16.9% is lung cancer 4 ). Among the causes of the burden of chronic diseases in the Republic of Kazakhstan, 4 risk factors were defined in 87.5% of cases: unhealthy feeding, insufficient physical activity, tobacco and excessive alcohol consumption. 5 According to national studies of the prevalence of these risk factors among the population of the country conducted by the National Center for Healthy Lifestyle Development, in Kazakhstan there is a tendency to decrease the most significant behavioral risk factors, including smoking and alcohol consumption, and an increase in the proportion of physically active persons. Nevertheless,

2 the relevance of the issues related to chronic non-communicable diseases does not decrease due to the priority in the main indicators of lost years of healthy life. Being a member of the world community, Kazakhstan, as a UN member state, adopted the Agenda of Sustainable Development until It also contains a number of goals aimed at eradicating poverty, conserving the planet's resources and ensuring well-being for all. Each of the 17 Goals contains a number of indicators that must be achieved within 15 years, in particular, one of the most important goals is to ensure a healthy lifestyle and promote well-being for all at any age. One of the main directions of the State Health Development Program - "Densaulyk" for is the formation and development of the public health system as the basis of public health protection, aimed at achieving 3 main goals: - policy development through the improvement of the legislative framework to support public health activities; - implementation of measures aimed at the prevention and monitoring of infectious and noninfectious diseases; - mobilization and coordination of all resources of society to ensure public health. The main functions of the Public Health Service are: - raising public awareness and involvement in prevention activities (healthy eating, smoking, alcohol consumption, physical activity) - coordination and expansion of intersectional interaction with state bodies, ngos; - conducting epidemiological monitoring of communicable and non-infectious diseases - introduction of international systems for long-term modeling and disease propagation at the regional and national levels; - control over the observance of the rule of law According to WHO international experience demonstrates sound economic arguments for taking urgent public health measures, especially on the prevention of non-communicable diseases. Effective reduction in the number of non-communicable diseases is possible through prevention and a systematic approach to the management of risk factors, their causes by adopting proven measures available in value in all countries of the world. In Kazakhstan, for example, the concept of healthy school meals is being developed, the promotion of consumption of fresh and environmentally friendly foods, low-fat foods, salt and sugar has been strengthened, and there has been continuous monitoring and supervision of risk factors for reducing professional, environmental and social risks Page 2 of 6

3 Steps to reduce the level of alcohol and tobacco consumption, as well as the correction of eating behavior, should include instruments of tax and regulatory impact on the producers of the products concerned: the introduction and increase of excise taxes on certain food groups that could potentially harm the health of the population, restrict advertising, requirements for their packaging and labeling. An important positive role can also be played by measures aimed at promoting products that have reduced harm or the ability to compensate for harmful effects. For example, reducing the risk of tobacco smoking can be achieved by developing products that on the one hand, do not contain at all or contain a minimum of dangerous and potentially dangerous components, and on the other hand, it meets the needs of adult smokers. Thus, tobacco products, for which reduction in adverse effects that have been shown, can help reduce the overall mortality and morbidity statistics from its use. If we talk about reducing the risk of diabetes and various degrees of obesity, then an important direction can be used to stimulate the consumption of foods with low sugar content, adherence to the principles of proper nutrition and the promotion of active lifestyles. It should be noted that the introduction of such measures should be carried out after careful analysis and a comprehensive assessment of the possible consequences of both economic and social nature and in terms of impact on public health. In particular, the products of reduced risk produced by the tobacco industry have a positive impact on the health of the population, these products should be sufficiently acceptable for active adult smokers, so that they can switch to its use, but should not attract new consumers. It should be noted that legislative and tax measures are one of the most effective tools that both state and regional authorities can apply to improve the health of citizens. Therefore, it is extremely important to establish a mechanism for preliminary assessment of proposed measures in the field of taxation and regulation of the promotion of products related to risk factors. To this work, it is necessary to involve experts in the field of health and economy, take into account international experience, and then disseminate the developed recommendations at the regional, national and international levels. To increase the effectiveness of the non-communicable disease prevention system, it is rational to consider the prospects of reducing the level of risk factors not only by medical means, but also by introducing innovative non-medical approaches that can minimize the harm of behavioral risk factors for public health in general. Health promotion is "not only a task for the health sector, but also an effort to create a healthy lifestyle and more broadly, an all-round improvement in the overall well-being of people," the Ottawa Charter for Health Promotion is emphasized. For this reason, it is necessary to develop and implement economic and social incentives for the transfer of production to produce products, focused primarily on human health. It is advisable to involve business associations and specific manufacturers in solving this problem. Page 3 of 6

4 Based on the above, the Conference participants consider it necessary to: - focus attention of all structures of the government, scientific and public organizations on strengthening of a preventive orientation of struggle against non-infectious diseases with orientation to preservation of health of the person; - recognize the need and importance of developing a systematic approach to managing behavioral risk factors for non-communicable diseases; - suggest the Government of the Republic of Kazakhstan to consider the possibility of developing and implementing regulatory instruments that could counteract the consumption of potentially "harmful" products, and reorient producers and consumers products with reduced harmful effects on human health, if they are recognized as less harmful on the basis of relevant scientific evidence; - suggest the Ministry of Health of the RK to create a working group under the Ministry with the participation of state bodies and other parties to discuss and develop administrative, economic, educational measures of influence, both state and society, on risk factors of non-communicable diseases and offer the Aman-Saulyk Public Foundation to lead this group as the national coordinator; - strengthen the promotion of healthy lifestyles and management of risk factors, improve its quality and develop methodological guidelines on the fundamental principles of the national policy on managing risk factors; - continue the exchange of experience and the development of international cooperation in the field of managing non-infectious disease risk factors.
