3 Withdrawal Management Centre (WMC) 135 McLaughlin Road South, Brampton 20 Bed Co-Ed Facility, Open 24/7 Safe, non-medical medical, brief residential services (average stay 3-5 days) for individuals 16 years of age and older who are intoxicated, in withdrawal, in a crisis related to alcohol, drugs, and/or gambling, and/or who are waiting for addiction treatment. Support and monitoring through withdrawal Only Residential Withdrawal Management Centre within a large area Accepts clients from any region Smoking Policy in effect

4 Withdrawal Management Services All staff are Addiction Crisis Workers Staff will assist clients with referrals (counselling, treatment, housing, etc.) when needed Assessments can be completed by staff to help with the referral process While at the WMC, clients will be participating in 12 step meetings, daily psycho-education groups, Auricular acupuncture, information sessions on community agencies and supports. For example: Harm Reduction, Hep C/AIDS info sessions, etc. Telephone support available 24 hours a day, 7 days a week

5 WMC Admission Criteria 16 years of age or older Intoxicated (stable on foot) In Withdrawal (used within 72 hours) In Crisis (Risk of Using) Pre-Treatment (Confirmation required, either by referring counsellor or Treatment Centre)

6 WMC Referral Process Self Referral. Client must call the Centre directly Counsellors/Case Workers/Physicians/Probation Officers etc. can call the Centre and check availability at any time Client must call the day they are ready to come in If a bed is available then a Telephone Screening would be completed with the client Telephone screening typically takes minutes to complete. A number of questions are asked to determine if the client meets the admission criteria

7 WMC Referral Process (cont d) Medical clearance may be required if there are some medical/withdrawal concerns noted by the client or the Addiction Crisis Worker. In the event that a medical clearance is required the clients bed will be placed on hold pending the clearance. Medical clearance can be done with their family doctor or at the hospital. If a bed is not available at the time, the client can be referred elsewhere, or they can be placed on the wait list. If placed on the wait list the staff will call them back as soon as a bed is available (typically the client will receive a phone call within 24 hours)

8 What We Do at WMC... Support and monitor clients through their withdrawal Once admitted clients are placed in our Observation Area Typically, clients stay in the observation area for a minimum of 4 hours to ensure stabilization Once stable, clients are moved to the residential side Counselling (1 on 1 and in group) Treatment / Discharge Planning Average length of stay is 3-5 days, however it is assessed on an individual basis

9 Bridging Addiction Counselling (BAC) Coordinated services offered by William Osler Health System, CMHA Peel, Punjabi Community Health Services, and Peel Addiction Assessment and Referral Centre Support clients during the transition from Withdrawal Management Centre to residential treatment or long-term counselling Referrals accepted through Central West LHIN Health Links Short-term addiction assessment and counselling Assists the individual to engage with community resources that will meet their longer term needs

10 Bridging Addiction Counselling (cont d cont d) Assists individuals with the application process for these programs Families or concerned persons of the individual will be referred to appropriate resources where applicable Mobile services to meet clients in a safe place within Central West LHIN

11 Addiction Counselling Services (ACS) Primary location at Peel Memorial Centre for Integrated Health and Wellness. Additional Locations: - Etobicoke General Hospital - Homewood Community Addiction Services (CADS) Orangeville One on one counselling for alcohol, drugs, and problem gambling Open during the day, Monday through Friday some evening appointments available Comprehensive Assessment/ Treatment Planning / Referrals

13 Central West LHIN Narcotics Strategy Context: $12 Million annual funding from Province since In response to the increase of opioid addiction and associated mortality Strengthen existing services through collaborative partnerships integrated across sectors (interdisciplinary, outreach-based services) Increase access to care Target populations: Poly-substance users, concurrent disorders, youth, homeless persons, people living in rural or remote areas, First Nations Services based on the local needs and context

15 Resources Shared amongst Strategy Partners 1 Narcotics Strategy Coordinator 2 Psychiatrists specializing in Addiction Medicine 1 Nurse Practitioner Telemedicine Facilitator 11 Ontario Telemedicine Network (OTN) sites. 7 clinicians (Counsellors and Case Managers) work as part of the Narcotics Strategy team and are supported by the shared resources.

17 South Asian Opioids Addiction Program Supports individuals 16 years of age and older Current or previous opioid use Methadone or buprenorphine (Suboxone) treatment referrals Individual and family counselling & case management Group therapy Assessments, Referrals and home visits Culturally and linguistically appropriate services

18 Peel Addiction Assessment and Referral Centre (PAARC) Choices Program Supports individuals 16 years of age and older Current or previous opioid use Methadone or buprenorphine (Suboxone) Individual counselling/ Groups/ Case Management Services at the office or in the community May include family members and concerned persons

19 Family Transition Place Mothers Changing Path Program Supports pregnant and parenting females of any age Opioids and /or any substance use Individual counselling and case management Trauma-informed practices Parenting support Child welfare issues Childcare available onsite Serving Caledon and Dufferin regions
