2 FOLLICULAR LYMPHOMA: Definition Lymphoma arising from B cells located within follicular centers, which consist of centrocytes and centroblast (cleaved & non-cleaved cells). This lymphoma has a follicular pattern (not follicular colonization). 3 SIGNIFICANCE OF BACK-TO-BACK PATTERN OF FOLLICLES 1. In 98% of the cases, such a pattern indicates that the follicles are malignant. 2. In 2% of the cases, this pattern is associated with benign follicles, especially in early HIV. 10 CYTOLOGIC FEATURES Cytologic criteria FCC: Unusual cytologic features Grading of Follicular Lymphoma Differences between grade 3A and grade 3B MEASURING CELL SIZE 1. By comparing size with: - red blood cells - small mature lymphocytes - nucleus of a benign histiocyte 2. By using a built-in in micrometer in the eye piece lens. CELL SIZE DEFINITIONS Small cells (up to 2 times the surface area of RBC or small lymphocyte) Medium size cells (2 to 3 times the surface area of RBC or small lymphocyte) Large size cells (>3 times the surface area of RBC or small lymphocyte) MORPHOLOGY OF FOLLICULAR CENTER CELLS (FCC) FCCs are divided into two groups based on the presence or absence of distinct nucleoli 1. Non-nucleolated nucleolated cells are called centrocytes ( cleaved cells), and on the basis of size, are subdivided into two types: A. Small centrocytes B. FOLLICULAR LYMPHOMA (cont.) 4. The small cells ( cleaved ) show pronounced variation in nuclear shape. 5. Numerous spindle shaped cells in non sclerotic areas. 6. The small and medium sized nuclei show nuclear convolutions. 7. Cytology of cells within and outside follicles is identical. The data presented here show that extranodal,, nodal, & splenic marginal zone B-cell lymphomas are - morphologically similar, but different clinically & genetically 3. In addition, primary nodal marginal zone B-cell lymphoma in children & young adults is probably a distinct entity 4.
