2 Your Support Team Hillary Miller, MD Medical Director Dr. Miller oversees the weight management program which she helped start in She is board certified in Family Medicine and graduated from Harvard Medical School. She joined ARC in Lindsey Jennings Program Director Lindsey leads the support team that helps you create long- term lifestyle changes to manage your weight. She graduated with a Bachelor s Degree in Kinesiology from the University of North Texas. Deanna Engbar, M. Div.,LPC, OTR Deanna is a licensed counselor whose role is to lead the groups and help patients identify the reasons behind food usage. She has been with the Weight Management program for over 12 years. Nancee Lottmann, LPC Nancee also leads groups in gaining problem solving skills surrounding long term weight management.
3 Program Options OPTIFAST Full liquid program Optihealth Partial liquid program
4 OPTIFAST Full Liquid Program Approximate weight loss: lbs* in 6 months Medically-supervised program Weekly weigh-ins, vitals check, visit with behaviorist Monthly doctor visit and lab draw (Tues. and Wed.) Thoroughly-studied, medicallycreated liquid formula plan Full liquid program: 800 calories per day = 5 OPTIFAST shakes (available in powder and ready to drink form; chocolate, strawberry, and vanilla). OPTIFAST soups and bars also available. Attend weekly group sessions Facilitated by a licensed counselor Explore the psychological aspects of relationships with food Discover ways to create a healthier lifestyle Monthly maintenance and exercise information *ARC Weight Management OPTIFAST patients who actively participate in a 26-week program typically lose approximately 50 pounds with weight-loss-related improvements in blood glucose, blood pressure, and cholesterol levels.
5 Optihealth Parital Liquid Program Approximate weight loss: lbs in 6 months Not medically supervised Weekly weigh-ins and one-on-one coaching with behaviorist Partial liquid program: calories per day (depending on plan). Program consisting of OPTIFAST shakes(powder and ready to drink), bars, and one balanced portion controlled meal. 9 Insurance Insurance Coverage Insurance coverage varies depending on your policy. Some plans may cover the cost of the doctor visits and lab work, minus your co-pay. However, ARC cannot bill insurance for OPTIFAST products or group sessions. Prior Authorization You must have prior authorization before we can submit the charges to your insurance company. If we bill insurance and they do not cover your visit, you will be responsible for the charges. Check Your Coverage To check your coverage, call the customer service number on the back of your card. Ask if you are covered under the diagnosis of Obesity, code E66.9.
10 New Patient Packet $ * This package for new program participants includes an orientation by the program director, an initial assessment and evaluation by the physician, and all of your nurse visits and group sessions for your first calendar month in the program. Also Required Monthly lab fee: $47.00 (CMP) $75.00 (TSH) 1 st month only Initial EKG fee: $ nd EKG after 50 lb. loss: $33.00 * There may be additional fees for high risk patients that may require a weekly office visit and/or additional lab the first month in the program.
11 Monthly Package $ * This package for established program participants includes a monthly follow-up visit with Dr. Miller, all your nurse visits, and all of your group sessions for the month. This package is to be paid at the beginning of each calendar month. Also Required Monthly lab fee: $47.00 (CMP) 2nd EKG after 50 lb. loss: $33.00 * There may be additional fees for high risk patients that may require a weekly office visit and/or additional lab the first month in the program.
12 OPTIFAST Formula Prices OPTIFAST Powder OPTIFAST Ready To Drink (RTD) OPTIFAST Soup OPTIFAST Nutritional Bars $19.00/box (7 packets) $3.50/single RTD $3.50/soup $3.50/bar There are no refunds or exchanges for product purchases. Shake Subsitutions One shake = one soup packet, limit 2 soups/day. One shake = two bars, limit 2 bars/day. 35 Units of Product Required Each Week It is recommended that you purchase more than 35 units each week to ensure you have what you need for a week and extra formula if you need it. 18 Start Today Check Your Insurance Coverage Call your insurance company to see if your plan covers the diagnosis Obesity (Diagnosis code: E66.9) and that Dr. Hillary Miller is in your network of providers. Register as an ARC Patient If you have never seen an ARC doctor, please call the registration department at before your appointment date. You must register in our system prior to your appointment before being seen by the doctor. Your appointment may be rescheduled if you have not registered. Submit Your Information the Program Director at Include in your Completed patient registration form The program you want to do (OPTIFAST or Optihealth) Payment method: covered by insurance or self-pay If you have had a lab or EKG within the last six months (only necessary for OPTIFAST patients).
19 Starting the Program Complete all paperwork mailed to you before your appointment and bring it with you to your first appointment. Have any previous lab and/or EKG results faxed to , Attention: ARC Weight Management Program Director. Please do this before your first appointment or bring copies with you. If labs or EKG are not received, we will have to do them at the time of your visit. If you have had lab work done at ARC, we can access the results through our system. However, you will need to request a copy of your EKG and bring that with you or have it faxed.
