4 4 Foreword It is my privilege to introduce the Mapping of AIN Members Contributing to Health Sector in Nepal 2016 prepared by the Health Working Group of the Association of International NGOs in Nepal (AIN). The intent of this mapping is to play a more strategic role by analytically summarizing the information of the organizations working to strengthen the outcome of the health sector. The prioritization of the health sector, as an important development agenda emanates from the fact that it cuts across the notion of personal welfare and impinges the growth of the country. The programs and interventions, guided by the broader policy environment of the country, helps to attain the outcome of the sector. The expanse of the health sector requires meaningful support and the INGOs act as a catalyst to strengthen the mechanism at the implementation level. But, the presence of many organizations, contesting for a common goal, in itself doesn t indicate effective deliverance. The effectiveness of the program implementations can be accessed when there is a proper documentation of the various efforts. Facilitating this process, this mapping presents a detailed analysis of the program components, geographical coverage, and financial contribution of the forty INGOs working to achieve the targets set forth by the Nepal Health Sector Strategy III and the SDGs The program components of the organization is rightly orchestrated following the twin approach of the demand and supply side strengthening of the health system. The geographical coverage of the INGOs creates a semblance across all the 75 districts of Nepal imbibing the GESI perspective. This document also informs about the financial contribution through various health program in the districts. The mapping will be imperative for efficient planning as it reflects the discrete set of projects/programs/ interventions and also helps to make the needed improvements to the operations by circumventing the duplication in geographical coverage and interventions for impactful results. It will also support constructive learning of member organizations to strengthen the collaboration continuum. The strategic planning is useful only if it becomes the basis for management action and the mapping will be useful in this aspect as well. I believe, this initiative of Health working Group, will help bring visibility, transparency, and promote collaboration to avoid duplication of the resources. My sincere gratitude to all the members of the Health Working Group for their active support by providing the information and suggestion to organize the information. 5 5 Message from AIN Chairperson On behalf of AIN, it gives me an immense pleasure to present the Mapping of AIN Members contributing to the health sector in Nepal in This is the continued effort of AIN Health working group (HWG) to showcase and document technical and financial contribution by AIN members involved in health sector through mapping of INGOs contributing to health sector in Nepal. The Government of Nepal has impressively recognized the role of INGOs and civil society organization to accomplish results and outputs set by the Nepal Health Sector Program III ( ).The AIN HWG has been supporting government of Nepal and working in collaboration to achieve the targets set by Nepal Health Sector Strategy, MDGs, and SDGs. In the absence of proper documentation, it is challenging to analyze the actual representation of activities supported by the INGOs. In this regard, the mapping initiative of the HWG has been important. We are challenged by the ever evolving health care landscape which requires time, vigilance, dedication, and constant flow of new ideas to shape the pathway towards attaining global goals with local focus. The extensive outreach of information will be particularly important for the government and member organizations to constructively leverage learning experiences, strengthen the collaboration continuum, and plan for future activities ultimately devising a strategic plan that can be part of the management aspect. I am confident that this mapping will be able to serve the purpose of information generation and usage to optimize the efforts in the health sector. 8 8 I. Overview of Health Sector Mapping Health as development agenda When we refer to health from the individual lens it enshrines only the personal well-being. But it amasses deeper aspect from the standpoint of the social structure in developing economies which is intricately woven with different indices. The growth of a society is measurable by its total capital which consists of physical capital, social capital, human capital and natural resources. In this, the physical capital only represents 15-36% of the society s total capital (Hjerppe, 2000). Therefore, a much emphasis is laid on the human and social capital where the former comprises indices of health and education and later the wider social structure. Healthy population can optimize the utilization of the resources of the country in the right ways thereby acting as a catalyst in the higher gross domestic product (GDP) per capita as well as gross national product in the long run. Being a major part of the development agenda, the approaches around health are changing and contextualized. The MDGs were guiding the health sectors globally until the 2015 as today it is the Sustainable Development Goals (SDGs). Health sectors is now aligning with the SDG 3 - Ensure healthy lives and promote well-being for all at all ages. Even the LDC graduation has health, nutrition and education as the three major indices reflecting the economy of the country associated with the health outcomes. Global Health Goal and Country Specific Policy The global goal steers the momentum of the country towards the burning issues. These issues are structured and supported by the country specific policies and strategies which spells out the vision of the sectors bonded by the guiding principle. The state actors and supporting agencies work venerating the guided framework. The periodic plan presents the guiding tool for the Government of Nepal (GoN) to design developments policies and programmes. Health has been prioritized as a development agenda and the government has earmarked NPR billion that is 3.8% of the total budget. The constitution has embodied health as a fundamental rights of the citizen. The National Health Policy 1991 was updated to address the major changes in the country from federal structure to the global goals. The updated policy (2071/2014) gives momentum to the achievements made in the health sector and to tarnish the loopholes to meet the global goal by optimizing the resources and available means to provide promotive, curative, preventive and rehabilitative services. The health sector plan embodies a visual graph of the sector policy. The second long-term health plan envisions a healthcare system with equitable access and quality services in both rural and urban areas by encompassing the concepts of sustainability, full community participation, decentralization, gender sensitivity, effective and efficient management, and private and NGO participation. The Nepal Health Sector Strategy-NHSS is recognized as the operational aspect of the policy and plan that will guide the sector, taking into account multi-sector collaboration to address social determinants of health, over the next five years. The goal of NHSS III is on improved health status of all people through

9 9 accountable and equitable health service delivery system. The focus has been more on the quality of the health services as the lacuna founded by the midterm review of NHSPII. Maintaining the equilibrium the role of AIN The health sector has been prioritized by the government which requires collaboration across multiple agencies to drive results. The INGOs play a crucial role to strengthen the government system by working on the unmet gaps and resources following the organizational strategic directives. But there are some shortcomings, when many parties work for a common (national) goal following their respective organizational pathway. A common platform is crucial to streamline the various efforts of these organizations. The association of International NGOs helps to cement this gap and maintain equilibrium by providing a platform for effective coordination and communication. The origination of this association dates back to the conflict time, as the guardian of the INGOs. Today, the boundaries of functionality has transcended, as it is a bystander for the government as well as the INGOs. The Health working group under AIN provides a platform to its member organizations for constructive learning, show casing good practices, creating solidarity on any policy related difficulties or challenges, and representing the INGOs (health sector) in various forums organized by GON and other stakeholders in health related matters. It also helps avoiding duplication of programs, and maximizing utilization of the resources and to foster partnership based on the shared vision of the organizations. The association is equally useful for the government for monitoring the efficacy and efficiency of the project activities/ interventions to the overall outcome of the health sector. Succinctly, AIN provides the connectivity between the government and member organizations for advocacy, policy dialogues and monitoring. Health Sector Interventions of the members of AIN There are total of forty one INGOs supporting through various programs as per the priority of the government for the health sector. The geographical coverage of the interventions at the implementation level is a crucial marker of the rightly structured approach/programs. To minimize the impact of coverage gap, the INGOs work with the existing structure of the government and also build partnership with the locally active NGOs. As per this mapping, the programs of the INGOs have covered all the seventy five districts of the country. The districts of the central development region have the maximum number of INGOs. This indicates, the active relief and recovery response of the organizations, to address the impact of the massive earthquake. Dhading, one of the earthquake affected district, has the highest concentration of INGOs (19 in total).in the eastern development region, Sunsari with 16 organizations, represents the highest coverage in that region. The districts of the western development region, like Manang and Mustang, have only three organizations supporting the health sector outcomes, which represents the least coverage in the entire mapping. The districts of the Karnali zone (Humla, Jumla, Dolpa, Mugu and Kalikot) with the Human Development Index much lower than the national average, also has a low coverage. The Kailali district, in the far- western development region, has the highest number of organizations (16 in total).

10 10 The constellation of many organizations in one region, also subtly indicates the need of effective collaboration, to minimize the duplication of efforts and resources. The VDC /municipality coverage within each districts represents a good proportion while simultaneously also signifying the need for scaling up of targeted interventions. The program selection has not followed a blanket approach but based on the need of the specific region. Principally the two pronged approach of the demand and supply side strengthening, which covers the full spectrum of health-curative, preventive, promotive and rehabilitative as righty enshrined by the National Health Policy 2071 (2014) has guided the INGOs in their program selection.it also reflects the wider social determinants of health such as empowering of women, improving of rural and urban infrastructure, nutrition and food security, expanding access to clean water and sanitation, better waste management, and improving access to quality education which ensure stronger public health actions at national and community levels and a more integrated approach on putting health as a central component of development. The component of the supply side strengthening has been prioritized by all the forty one INGOs working in the health sector. The capacity development of the health system and the human resource, for the deliverance of the quality of the services, has been the focus of all the organizations. In addition, the logistic support for the smooth functioning of the health care system has also been prioritized. The role of effective planning and coordination from central to the district level is another crucial area that has been highlighted by the program components. The ultimate goal of primary health care is better health for all and WHO has identified five key elements to achieving that goal-universal coverage reforms; service delivery reforms; public policy reforms; leadership reforms and increasing stakeholder participation. These dimensions are embedded into the program component of the organizations. The demand side strengthening, which is essential to bring responsiveness of the supply side, through the proper adoption of methods of advocacy and awareness, has been rightly ensured. Within this overall approach, the program components have been structured, where the demand and supply side strengthening has been highlighted as a sub-component as well. The program constituents of the INGOs can be broadly categorized as Sexual and Reproductive Health comprising of the complete spectrum of the services including the Adolescent sexual and reproductive health (43.9%); Maternal, New born and Child Health (MNCH-34%); Nutrition specific interventions(imam, growth monitoring, IYCF, counselling, community nutrition camps, diet management training,miycn-31.7%); Water Sanitation and hygiene as a health sensitive intervention (31.7%); Mental Health and psychosocial support(19.5%); prevention and control of Diseases(Diarrhoea,leprosy,malaria,polio,TB) (14.6%); HIV and AIDS prevention, care and support(12.19%); Disability services (4.8%).The percentage signifies the category being addressed by the organizations(approximation). The Sexual and reproductive health component, which has the highest number of organizations, is also well represented in terms of geographical spread, as it covers all the seventy five districts. The Maternal New born and child health component is prevalent in 65 districts. The remarkable aspect of the INGOs working in the health sector was seen during the humanitarian crisis of the mega-earthquake. In the earthquake, 8702 people lost their lives (45% male, 55% female). A total of 446 public health facilities and 16 private facilities were completely destroyed while a total of 765 health facilities were partially damaged. This calculates to 43% of country s health institution directly affected by the earthquake. These required additional capacities and resources which have been managed in a coordinated efforts of the members of AIN. Presence of the INGOs in the earthquake affected districts is a good indication of adaptation of the organizations to the emergency situation. The program component under this covers the following-

11 11 Emergency Medical Services(EMS) System & Infrastructure Development Restoring FP counseling and Service Delivery in the health care facility (Obstetrician, Pediatrics, Clinical and Pubic health nurse). Re-establishing Quality Obstetric and Newborn Care Services in Earthquake-Affected District Reconstruction of damaged health posts with complete set of equipment, supplies including birthing center equipment as per the GoN standards Emergency Sexual and Reproductive Health Establishment of Injury rehabilitation unit(iru) Health Sector's disaster preparedness Health and Nutrition Earthquake Response program The allocation of funds is crucial to achieve the activities/programs as planned for an organization. The AIN members have allocated significant funds for the health sectors service delivery and system strengthening which is approximately NPR 6 billion(usd 58 million). Nutrition specific budget allocation supported through health sector is also included in this figure. In conclusion, the mapping reflects on the crucial aspect of functional domain of an organization- Viz program components, geographical coverage and budget allocation which will be important for effective planning and strategizing future roadmap encompassing the government priorities.
