5 Background: Why Address Tobacco? Remains number one cause of preventable disease and death Causes 5,100 deaths per year in Minnesota Impacts the whole person Secondhand smoke causes disease and death for non-smokers
6 Background: Why Low Socioeconomic Status? SES is one of the greatest predictors of smoking Tobacco has disproportionate impact People of low-ses smoke at higher rates Greater financial and health burden
7 Funding Initiative Goals Primary Goal: create pathways to connect low-ses smokers to existing cessation services Secondary Goal: to build capacity within grantee organizations to continue linking clients to existing cessation services after grant funding has ended
8 Funding Goals: Important Details The intention is to connect smokers to existing services and not to create new programs Examples of existing services: QUITPLAN Services Health plan quitlines Local programs (e.g., offered through a clinic) Talking to one s doctor
9 Size and Duration of Grants Funding range is $10,000 to $50,000 One year grants Grants will begin June 15, 2015 We expect to award four to six grants
10 Applicants must: Eligibility Requirements Be an incorporated nonprofit organization, a tribal group recognized by the U.S. federal government, a safety net clinic, or a local government entity Be located in the state of Minnesota and all work must be conducted in Minnesota Work directly with and have a primary focus of serving low-ses Minnesotans Request an amount that does not exceed 48 percent of the organization s total yearly budget Comply with all relevant ClearWay Minnesota policies, including those outlined in Form 2 in Appendix 3
11 Eligibility Requirements (cont.) Organizations that receive funding shall: Comply with ClearWay Minnesota reporting requirements (e.g., written and financial reports); Participate in QUITPLAN Services training; and Collaborate with ClearWay Minnesota s external evaluator as part of the evaluator s work to identify key learnings.
12 Project Types Grantees will have the flexibility to design proposals and project activities based on expertise, organizational programs and population(s) served Sample project activities are included in RFP Appendix 1 (page 12) Organizations may propose one or more of the sample activities provided, or use the samples as a guide Including sample activities provided in the RFP does not guarantee funding
13 Activities that are Not Covered ClearWay Minnesota will not fund: Creation of cessation programs and/or delivery of direct cessation counseling Connections to tobacco cessation services that are not considered to be evidence-based, evidence-based services are: Counseling: group, individual or telephone-based and/or Medications approved by the Food and Drug Administration for cessation
14 Activities that are Not Covered (cont.) ClearWay Minnesota will not fund: Work with a primary focus on youth (those under age 18) Tobacco cessation medications Research projects
15 Timeline RFP is released January 6, 2015 Applicant Informational Webinar Offered both January 12 and 13, 2015 Letter of Intent due January 27, 2015 ClearWay Minnesota invites approved applicants to submit full applications February 12, 2015 Full application due March 17, 2015 Award notification May 20, 2015 Grants begin June 15, 2015
16 Application Process
17 Letter of Intent (LOI) LOIs should provide an overview of how your organization will use the funds to connect low-ses smokers to existing cessation services LOIs should include the following information: A brief organizational overview A description of how your organization serves low-ses Minnesotans A description of any work currently being done to address tobacco, if any An overview of the proposed project and how the work will link low-ses smokers to existing cessation services See page 6 of the RFP for more details
18 Submitting the LOI Follow the formatting requirements (page 6 of the RFP) letters and forms to cspecktor@clearwaymn.org Required forms to be completed and submitted with the LOI Letter of Intent Cover Sheet (Form 1) Acknowledgment of Application Terms and Conditions (Form 2) Letters of Intent are due no later than 4:30 p.m. Central Time on January 27, 2015 Letters must be sent via and by the deadline in order to be reviewed
19 LOI Review Process ClearWay Minnesota staff will conduct a detailed review of LOIs Invitations to submit a full proposal will be based on letters with the greatest promise to meet the project goals Letters of Intent will be evaluated on: Eligibility of the applicant organization Potential reach to low-ses smokers Potential of the proposed approach to create a pathway to connect low-ses smokers to cessation services Applicants will be notified if they are eligible to submit a full proposal by February 12, 2015
20 Full Application Full proposals will be by invitation based on the letter of intent review process All application materials must be received by 4:30 p.m. Central Time on March 17, 2015 All instructions for formatting and online submission of the full application will be provided with your LOI notification letter
21 Full Application Review Process All proposals will be evaluated by a review panel of members with expertise in tobacco control, community-based programs and working with low- SES populations The review panel will include both ClearWay Minnesota staff members and external experts Review Criteria (see page 9 of the RFP) ClearWay Minnesota Board of Directors makes all final funding decisions
