SCOTTISH MEDICINE CONSORTIUM RECOMMENDATIONS: Recommended for use within NHS Scotland Embargoed Until 9 th July 2012 SMC Accepted Indication Medicine 793/12 In addition to standard optimal therapy, to reduce the risk of cardiovascular Epleroenone 25, 50mg mortality and morbidity in adult patients with NYHA class II (chronic) film-coated tablets heart failure and left ventricular systolic dysfunction (LVEF 30%). 794/12 In a combination regimen with ribavirin for the treatment of children Formulary Decision & Rationale for non-inclusion Date 2
3 Pegylated interferon alfa-2b 50, 80, 100, 120 or 150 micrograms powder for solution for injection in pre-filled pen years of age and older and adolescents, who have chronic hepatitis C, not previously treated, without liver decompensation, and who are positive for HCV-RNA. Recommended for use within NHS Scotland Embargoed Until 13 th August /12 Mercaptopurine 20mg/ml oral suspension For the treatment of acute lymphoblastic leukaemia (A) in adults, adolescents and children. Recommended for Restricted use within NHS Scotland Embargoed until 11 th June /10 Treatment of adult patients with macular oedema following either branch Dexamethasone 700 retinal vein occlusion or central retinal vein occlusion. microgram intravitreal implant Recommended for Restricted use within NHS Scotland Embargoed Until 9 th July /11 Alone or in combination with methotrexate, for the treatment of active and Golimumab, 50mg progressive psoriatic arthritis in adult patients when the response to solution for injection in previous disease-modifying anti-rheumatic drug (DMARD) therapy has prefilled pen been inadequate. 710/11 Tadalafil 20mg tablets 791/12 Fidaxomicin 200mg film-coated tablets 795/12 Rufinamide 40mg/ml oral suspension Treatment of adults with pulmonary arterial hypertension (PAH) classified as World Health Organisation functional class (WHO-FC) II and III, to improve exercise capacity. Treatment of adults with Clostridium difficile infections (CDI) also known as C. difficile-associated diarrhoea (CDAD). Adjunctive therapy in the treatment of seizures associated with Lennox- Gastaut syndrome (LGS) in patients 4 years of age or older. Recommended for Restricted use within NHS Scotland Embargoed Until 13 th August /12 abiraterone is accepted for use in patients who have received only one prior Abiraterone acetate chemotherapy regimen. 250mg tablets Is not Recommended for Use within NHS Scotland Embargoed Until 11 th June /12 Belatacept, in combination with corticosteroids and a mycophenolic acid, is 3
4 Belatacept powder for concentrate for solution for infusion 250mg vial and disposable syringe indicated for prophylaxis of graft rejection in adults receiving a renal transplant. It is recommended to add an interleukin-2 receptor antagonist for induction therapy to this belatacept-based regimen. Is not Recommended for Use within NHS Scotland Embargoed Until 9 th July /12 Thiotepa 15mg and 100mg powder for concentrate for solution for infusion 800/12 Adalimumab prefilled pen, prefilled syringe and vial 803/12 Azilsartan medoxomil 20mg. 40mg & 80mg tablets 804/12 Azithromycin dehydrate 15mg/g, eye drops, solution in single-dose container In combination with other chemotherapy medicinal products: 1) with or without total body irradiation (TBI), as conditioning treatment prior to allogeneic or autologous haematopoietic progenitor cell transplantation (HPCT) in haematological diseases in adult and paediatric patients; 2) when high dose chemotherapy with HPCT support is appropriate for the treatment of solid tumours in adult and paediatric patients. For the treatment of moderately to severely active ulcerative colitis in adult patients who have had an inadequate response to conventional therapy including corticosteroids and 6-mercaptopurine (6-MP) or azathioprine (AZA), or who are intolerant to or have medical contraindications for such therapies. For the treatment of essential hypertension in adults. Patients admitted to hospital on a zip feed. Patients admitted to hospital that require additional analgesics. Application from Consultant to use Ranolazine on a patient. Roll out for first presentation for DVT s. New guidance. Dietician needs to be involved and to be noted on kardex. Buprenorphine patients in hospital. Applications have previously come to the Individual Patient Treatment Request (IPTR) meetings. Going on GP forum some time soon. Noted by BFC group members. Joanna Cooper to inform nurses dietician available within the hospital. To make wards aware of where to get these drugs. to contact Joanna Cooper and Anne O Connor to put a process in place. To look again at the use of buprenorphine in primary care and contact patient team. Application to come to next ADTC. Meeting with Dr J Kerr and plan to make change beginning of October. 10.
