Perform the 3 safety checks on the Simple Check List. 3. Explain to the client that you have a medication to be given. 4. Ensure privacy. 5. Place the meds in the client s hand w/o touching OR, with gloved hand, place one med at a time in the client s mouth, followed by a drink of water. 6. When finished, remove your gloves, and wash your hands. 7. Document administration. 8. Employee can state procedure if medication is refused. Sublingual (SL) Pass Fail 3. Explain to client what you are going to do. 4. Ensure privacy; client should sit upright. 5. Instruct client or place medication under the tongue. 6. Instruct to hold in place until completely dissolved. 7. Remind not to eat, drink, chew or swallow. 8. Remove gloves and wash hands. 9. Document administration. Buccal Pass Fail 3. Explain to the client what you are going to do. 4. Ensure privacy. 5. Instruct the client, or assist, to place the medication between the gum and wall of cheek. 6. Instruct to hold in place until completely dissolved, about 5-10 minutes. 7. Advise not to eat, drink, chew or swallow. 8. Remove gloves, wash hands. 9. Document administration. 2 Topical Medications (T) Pass Fail 1. Perform your 3 safety checks on the Simple Check List. 3. Explain to client what you are going to do. 4. Ensure privacy. 5. Observe any unusual conditions of the affected area. 6. Wash and dry the area PRN. Do not use alcohol. 7. Place cap upside down when removing cap. 8. Use clean tongue blade/cotton swab or gloved finger. 9. Apply thin layer; do not allow applicator tip to touch skin. 10. Cover with dressing, if ordered. 11. Recap/close container. 12. Dispose of gloves, wash hands. 13. Document administration. Eye Drops Pass Fail 3. Have client sit up. 4. Observe Red? Swollen? Drainage? Crustiness? Gently remove with clean, wet washcloth or moist cotton ball. 5. Position client with chin up, looking up. 6. With forefinger, gently pull down lower lid, creating a little pouch. 7. Approach eye from below, out of field of vision. 8. Avoid touching eye with applicator tip. Tip should be no more than one-inch away from pouch. 9. Place drop(s) in center of pouch. 10. Allow eye(s) to close gently. 11. Keep closed for about 1 minute. 12. If med is for glaucoma or inflammation, apply slight pressure to inside corner of eye and hold for 1 to 2 minutes. 13. Dab excess medication from eye with clean cotton ball or tissue. 14. Use separate tissue for each eye. 15. Wait one (1) minute before applying another drop of the same medication. 16. 3 Eye Ointment Pass Fail 1. Perform the 3 safety checks on your Simple Check List. 3. Position client with chin up, looking up. 4. Gently retract lower lid; approach from below. 5. Begin at inner aspect of eye, moving outward. 6. Place a thin ribbon of ointment along inside of lower lid; do not touch lid with applicator tip. 7. 1/3 inch of ointment is generally sufficient. 8. Break off ribbon of ointment from tube by releasing pressure on the tube; do not use fingers. 9. Replace the cap on tube. 10. Remove gloves, wash hands. 11. Document administration. Ear Drops Pass Fail 3. Gather a clean tissue or cotton ball. 4. Suggest/assist client to lie down, affected ear facing up. 5. Clean entry to ear canal with the clean cotton ball or tissue. 6. Gently pull outer ear (pinna) up, back, and out. 7. Slowly drop ear drops into ear; don t let dropper touch the ear. 8. Wipe away excess medication. 9. Encourage client to remain in this position 2-3 minutes. 10. If drops in both ears, wait at least 5 minutes before placing drops in second ear. 11. Assist to upright position. 12. Caution that ear disturbance may cause dizziness. 13. Remove gloves; wash hands. 14. Document administration. Transdermal Patch Removal Pass Fail 1. Wash your hands, apply gloves. 2. Remove the old patch, if present. 3. Fold old patch in half; cut into 2 or 3 pieces. 4. Note skin condition for any redness, rash, skin breakdown. Report this to nurse. 5. Clean area with mild soap and water. Rinse and dry well. Do not use alcohol. 6. Follow your company s policy and procedure for disposing of old patch. 7. 4 Transdermal Patch Application Pass Fail 1. Perform your 3 safety checks on the Simple Check List. 2. Wash your hands, apply gloves. 3. Apply patch to clean, dry skin. Firmly press patch in place for seconds. 4. Date and initial new patch. 5. Remove gloves; wash hands. 6. Document administration. Inhaler/Metered Dose Inhaler (MDI) Pass Fail 2. Wash your hands. 3. Shake the MDI. 4. Instruct client to exhale completely. 5. Instruct client to inhale deeply at same time as depressing activator for puff. 6. Instruct client to hold breath for at least 10 seconds or as long as possible. 7. Wait 1 minute between puffs of same inhaler. 8. If you have two inhalers: First: bronchodilators Second: steroids 9. Rinse mouth after steroid. 10. Rinse equipment with warm water; air dry. 11. Return MDI to storage location. 12. Wash hands. 13. Document administration. Mirabelle Management, LLC 2007 Page 4 of 6

5 Dry Powder Inhaler (MDI) Pass Fail 3. Explain what you are going to do. 4. Provide privacy. 5. Assist client to a sitting position. 6. Remove capsule from package, peeling back foil cover to expose only one capsule. 7. Open the outer cap of inhaler device (pull cap upward). 8. Place capsule/tablet into center of chamber of the inhaler. 9. Hold device upright, leaving outer cap open. 10. Close mouthpiece/lid firmly until you hear a click. 11. Press the side-mounted piercing button in complete and then release. 12. Have client breathe out completely. 13. Instruct client to put mouthpiece of inhaler into mouth and take a deep breath, holding as long as possible. 14. Repeat steps if necessary. 15. When finished, open mouthpiece and discard remaining capsule. 16. Close mouthpiece and outer cap and store where instructed in client s room. 17. Clean unit only as necessary. 18. Use warm water and allow to air dry thoroughly. 19. Remove gloves; wash hands. 20. Document administration. Nebulizer (Nebs) Pass Fail Information for Nebulizer treatments is given in EduCare Medication Administration II Mini Modules. Nasal Spray Pass Fail 3. Explain what you re going to do. 4. Shake the bottle. 5. Request client blow nose. 6. Have client tilt head slightly forward. 7. Have client pinch closed one nostril with finger. 8. While keeping the bottle upright, carefully insert the nasal applicator into the other nostril. 9. Advise client to start to breathe in through the nose. 10. WHILE BREATHING IN, press down, firmly and quickly, once on the applicator to release the spray. 11. Have client breathe in through the nostril and breathe out through pursed lips or through the nose. 12. Repeat in other nostril or per instructions. 13. Wipe the nasal applicator with a clean tissue and replace the dust cover. 14. Most nasal sprays should be stored upright. 15. Discard gloves; wash hands. 16. Document administration. 6 Vaginal Creams Pass Fail 1. Perform the 3 safety checks on the Check List. 3. Explain what you are going to do. 4. Provide privacy; assist to comfortable position. 5. Encourage deep breathing. 6. Gently insert applicator into vagina as far as it will comfortably go. 7. Push plunger to release medication. 8. Remove applicator from vagina. 9. Offer tissue/towel to wipe excess. 10. Assist to position of comfort. 11. Wash applicator with soap and hot water; rinse thoroughly and dry on paper towel. 12. Discard gloves; wash hands. 13. Document administration. Rectal (R) Suppository Pass Fail 2. Wash hands, apply gloves. 3. Explain what you are going to do. 4. Provide privacy. 5. Assist to comfortable side-lying position, top leg drawn up, with the client s back to you. 6. Undergarments should be down or removed; drape. 7. Remove suppository from foil wrap. 8. Extra lubrication may be provided. 9. Encourage deep breathing. 10. Insert suppository into anus, along rectal wall, angling toward the belly button. 11. Gently wipe excess lubricant from rectal area. 12. Encourage client to squeeze buttocks together in an effort to retain suppository as long as possible or directed. 13. Remove gloves, wash hands. 14. Document administration and any results, if appropriate.
