2 ASTHMA EDUCATOR BOOKLET INSTRUCTIONS 1. This is an educator companion to the patient Asthma Booklet with special instructions to aid in effective and consistent delivery of asthma self-management lessons. 2. Important Educator Tips will be highlighted on pages 3. Content or wording is numbered (large ORANGE numbers) to indicate the order for content delivery to patient/family. 4. The booklet is divided into four sections for easy access to find important topics during education. TABLE OF CONTENTS: 1. 3 WHAT IS ASTHMA?
4 For first time asthma diagnosis teach the whole section. If not first time diagnosis, choose the parts most relevant to fit your patient/family needs. 5 Before you begin, make sure the patient/family has a color copy of the asthma action plan and an Asthma Booklet. State content in #1 first and then move to #2. State content in numerical order (large ORANGE numbers) throughout this Educator Booklet 1 2 3
6 Name the symptoms and have the patient/family check off their own symptoms in the table on page 4 of the Asthma Booklet
7 Make sure the family notices that the number 2 is common in the table 2 times a week 2 times a month 2 times a year Asthma Control can be checked on the table on page 31 of the Asthma Booklet 1 The Goal is to have well-controlled asthma 2 3 5
8 As you say the words for how the air moves through the lungs (1), point out the locations on the lung picture on page 6 in the patient Asthma Booklet
9 As you say the words for each number in table (2) point out the locations on picture of the airways on page 7 in the patient Asthma Booklet Say all of the words in each table (2 and 4) 1 Asthma airways are sensitive or easily bothered by things we breathe into the lungs
10 As you state what each medicine does in the airway (2) point it out on picture of the airways located on page 7 in the patient Asthma Booklet Asthma medicines work to either STOP or PREVENT symptoms 8
11 Ask the patient/family if they use any of the pictured medicines. Point out the most common names and colors (ProAir, Ventolin & Proventil ) and make sure the family understands that they all work the same. Place FAST sticker on the quick reliever inhaler or give to parents to place on inhaler at home
12 Ask the patient/family if they use any of the pictured medicines. Look at the patient s AAP. If a controller is listed in Green Zone, reinforce everyday use. The benefit of a daily controller is: As long as the medicine is going into the lungs regularly it will keep the airways open Open airways lead to well-controlled asthma Place SLOW sticker on the controller or give to parents to place on inhaler at home. 1 2 Circle the controller you use at home on pages 10 and 11 in the Asthma Booklet
13 Typically one dose of oral steroids is prescribed for stand-by at home in case of an emergency. If medicine is listed in Red Zone on asthma action plan the patient should receive a prescription for the stand-by dose of oral steroids. A typical Red Zone burst is for 3 5 days. Liquid oral steroids do not have to be refrigerated
14 As patient identifies triggers, state how to reduce exposure (page 13 and 14 in the Asthma Booklet). If patient/family states there are many triggers in the home, go to page 34 in the Asthma Educator Handbook and follow instructions in the Educator Tips Triggers can start asthma symptoms by bothering sensitive airways. The most common trigger is a cold or virus Treat asthma symptoms during a cold with asthma inhaled medicines Check your asthma triggers on pages 13 & 14 in the Asthma Booklet 13-14

17 Make sure the patient/family has a color copy of the asthma action plan before you begin. Say all of the words in 1. If a controller is listed on the asthma action plan, place the respective controller sticker in the appropriate zone (see example below). The Goal is to have well-controlled asthma 1 An Asthma Action Plan will help you manage symptoms at home. It will also help you to know: How to keep your child feeling good What to do when symptoms start or are about to start What to do in an emergency 16
18 Compare zones to stoplights
19 If controller listed in Green Zone, review importance of daily use. Make sure the patient/family know the name and color of the controller. 1 2 The Goal is to have well-controlled asthma 3 18
20 Simple explanation: Anytime you use the quick reliever for symptoms, get in the habit of looking at your child/yourself after about 20 minutes. If better, things are going ok. If not better, give quick reliever again following the steps below. 19

22 Some families understand when to go to Red Zone by comparing it to baseball. 3 strikes (3 treatments every 20 minutes) and you are out in Red Zone It may help to circle the actual patient home dose of liquid oral steroids to prevent confusion:
23 Read the questions below to the patient/family and have them circle the answers on page 22 in the Asthma Booklet. Review answers and re-educate on incorrect responses. Ask other scenario questions as needed to ensure understanding
24 Page 23 in the Asthma Booklet is a blank asthma action plan. Some people learn better when they write. Have the patient/family write their medicine in the zones and then step you through the asthma action plan zone process. If unable to complete during educational session have the family complete it at home. 23

34 Patient/family can use this table to check their asthma control at home on a regular basis. Table is on page 31 in the Asthma Booklet. Instruct patient to call their asthma health provider if they do not check all of the boxes. 31
35 The Asthma Diary and Asthma Control Test can be printed from the Asthma Website, which is listed on the back cover of the Asthma Booklet. 1 2 You can track the asthma control at home with an Asthma Diary Make a note of symptoms, medication use and triggers on the diary You can also use an Asthma Control test This is a simple 5 question test that you complete once a month 32
36 Have the patient/family fill out their provider information on page 33 of the Asthma Booklet. Have the patient/family add follow-up appointment information if it is available. 1 2 Bring your completed Asthma Control Table/Asthma Diary/Asthma Control Test to your visit with your asthma provider. Together you can assess asthma control to en-sure the right treatment to have well controlled asthma. 33
