1 CRITICALLY APPRAISED PAPER (CAP) FOCUSED QUESTION Does occupational therapy with sensory integration (OT-SI) better improve attention, cognitive and social, sensory, or behavioral problems than an activity protocol (AP; active placebo) or no treatment (passive placebo) in children with sensory modulation disorder (SMD) Miller, L. J., Coll, J. R., & Schoen, S. A. (2007). A randomized controlled pilot study of the effectiveness of occupational therapy for children with sensory modulation disorder. American Journal of Occupational Therapy, 61(2), CLINICAL BOTTOM LINE: Researchers sought to measure improvements in attention, cognitive/social, sensory, and behavioral problems in children ( years of age) with sensory modulation disorder (SMD) among three different intervention s (OT-SI, AP, or No Treatment). Results showed significant improvements in the OT-SI on the goal attainment scaling (GAS) compared to AP and No Treatment, Leiter International Performance Scale-Revised (Leiter-R) Attention compared to No Treatment, and Leiter-R Cognitive/Social compared to AP. Nonsignificant results that seemed to indicate OT-SI being more effective were found in the Short Sensory Profile (SSP) Total Score and Child Behavior Checklist (CBCL) Internalizing Composite score. Children in the No Treatment exhibited greater, but nonsignificant results of the CBCL Externalizing Composite score, suggesting that changes in internalizing and externalizing factors may be related to exposure to intervention. Limitations of the study included small sample size, a heterogenous sample rather than homogenous sample, various unusable data from electrodermal reactivity (EDR) measurements, the issue of comorbidities in disorders, the blinding and possibility of inadvertent unblinding of parents and experimenters, and the majority of measures being parent report tools that may lead to recall or memory bias. Deficits in attention, cognitive/social, sensory, and behavior have the potential to negatively affect children s participation in their daily activities of school and play. Based on this study, OT-SI may be a feasible intervention for children with SMD. However, further research, ideally with a homogenous and larger sample, must be completed to determine if OT-SI is truly an effective intervention, for whom it is an effective intervention, and under what specific conditions it should be utilized. 2 The study evaluated the effectiveness of three treatment s for improvements in attention, cognitive/social, sensory, and behavioral problems in children ( years of age) with SMD. The three treatment s were OT-SI, AP, or No Treatment. DESIGN TYPE AND LEVEL OF EVIDENCE: Level I: Pretest posttest Limitations (appropriateness of study design): Was the study design type appropriate for the knowledge level about this topic? Participants were recruited and referred through convenience sampling from The Children s Hospital of Denver from April 1999 to December Occupational therapists recruited families, and the evaluating occupational therapists referred those children who met the authors inclusion criteria to the research staff. Informed consent was obtained from all families who agreed to participate in the study. Upon completion of the Short Sensory Profile (SSP); the Swanson, Nolan, and Pelham (SNAP-IV) questionnaire; an intake interview (detailed history, Vineland Adaptive Behavior Scale [Socialization], review of difficulties for the child, and parent goals); physiologic testing; the Wechsler Intelligence Scale for Children (WISC-III short form); the Leiter International Performance Scale-Revised: Parent Rating Scale (Leiter-R; Attention and Cognitive/Social); and Child Behavior Checklist (CBCL), children were randomly assigned into one of three intervention s (OT-SI, AP, or No Intervention). Inclusion Criteria Children between the ages of 3 and 11.6 years of age with a clinical diagnosis of SMD made by the referring occupational therapist after a comprehensive evaluation; hyperreactive electrodermal activity (EDR) to stimuli in 2 sensory domains on the Sensory Challenge Protocol; a total z score of -3 standard deviations (SD) below the mean, >-2.5 SD on 2 or more subsets, or >-4 SD on one subset of the Short Sensory Profile (SSP); confirmation of clinical SMD by the first author after the parent interview. Exclusion Criteria Other DSM-IV/ICD-9 diagnoses except attention deficit hyperactivity disorder (ADHD), learning disabilities, or anxiety symptoms; children younger than 3 or older than 11.6 years of age; an IQ <85, as scored on the short form of the Wechsler Intelligence Scale for Children (3 rd ed.; WISC-III); previous individual occupational therapy treatment, excluding occupational therapy at school; serious confounding life events; special education, specifically enrollment with an individualized education program (IEP) resulting in pull-out services. 2

These goals are guided by predetermined priorities for the children by the parent(s). Questions were asked on a moment-bymoment basis, rather than utilizing prescribed activities. The child interacted with the sensory materials provided in a meaningful and fun method. Activities were graded so that the children are challenged, but always successful. Setting Who delivered? Group 2 A large occupational therapy room with different sensory activities and toys Occupational therapist, with parents actively participating in the sessions 2 times per week 10 weeks total Brief Description 10 children (3 female, 7 males) received the Activity Protocol (AP) active placebo intervention, which included an assortment of tabletop activities such as blocks, puzzles, arts and crafts, reading stories, and other interactive games. Adults present during intervention participated only as much as the 3

A large occupational therapy room Activity Partners (those with education or psychology degrees and experience working with children), non occupational therapy staff members, or graduate students 2 times per week 10 weeks total Group 3 Brief Description 7 children (2 female, 5 males) were placed on a 10-week waitlist for the OT-SI and received no intervention (passive placebo, no treatment). Co-intervention ADHD or anxiety medications taken by any of the children could skew the results of the intervention on improvements in attention, cognitive/social, sensory, and behavioral problems. 5 Use of different therapists to provide intervention Six occupational therapists provided the intervention for the OT-SI, yet no occupational therapists were present during the sessions for the AP. Fidelity within each treatment was maintained by reviewing videotaped intervention sessions at weeks 1, 7, 14, and 20. MEASURES AND OUTCOMES Complete for each relevant measure when answering the evidence-based question: Leiter International Performance Scale-Revised: Parent Rating Scale (Leiter-R) The Leiter-R is a parent rating scale of Attention and combination Cognitive/Social subtests (other subtests were not utilized for this study). Authors report this measure as being both reliable and valid. The Leiter-R was administered once pre-intervention and once post-intervention with all three intervention s. Short Sensory Profile (SSP) The SSP is a parent report used to screen an individual s functional behaviors, specifically related to sensory responsivity. Authors report this measure as being both reliable and valid. The SSP was administered once pre-intervention and once postintervention with all three intervention s. Vineland Adaptive Behavior Scales The Vineland scale is a parent interview that differentiates atypical performance on daily living skills. For this study, only the socialization subtest was utilized. Authors report high validity, but did not report information about reliability. The Vineland scale was administered once pre-intervention and once postintervention with all three intervention s. Child Behavior Checklist (CBCL) The CBCL is a parent report tool measuring social and emotional behaviors, comprising eight syndrome scales (anxious/depressed, depressed, somatic complaints, social problems, thought problems, attention problems, rule-breaking behavior, aggressive behavior), which into internalizing and externalizing higher-order scores. Authors report this measure as substantiated for wide use in research. Authors did not report reliability or validity. The CBCL was administered once pre-intervention and once postintervention with all three intervention s. 5

6 Goal Attainment Scaling (GAS) The GAS is used for the parent and interviewer to establish and rank-order five goals of changes that are feasible for the child to achieve within the frequency and duration of the study. The rank weights the goal, and all scores are standardized based on equidistance between response options, allowing for mathematical calculations to determine the extent to which goals were met. Authors did not report the reliability or validity of this measure. The GAS was administered once pre-intervention and once post-intervention with all three intervention s. Electrodermal reactivity (EDR) EDR measures changes in electrical conductivity of the skin (specifically associated with eccrine sweat gland activation), representing sympathetic nervous system activity. The Sensory Challenge Protocol was used to collect EDR data. Authors did not report the reliability or validity of this measure. EDR was administered once pre-intervention and once post-intervention with all three intervention s. Measurement Biases Were the evaluators blind to treatment status? Circle yes or no, and if no, explain. YES There was intention to blind both parents and physiological experimenters to the children s assignments. However, with such a small sample and small team, some information regarding assignment may have been disclosed, essentially unblinding those involved. Recall or memory bias. Circle yes or no, and if yes, explain. With the exception of the EDR, all other measures are parent-report. This increases the chance of recall or memory bias in the study. RESULTS List results of outcomes relevant to answering the focused question Include statistical significance where appropriate (p<0.05) Include effect size if reported Significant improvements were seen in the OT-SI over the other two s on the GAS (p <.001) and Leiter-R Attention (p =.03 compared to the No Treatment ) and Leiter-R Cognitive/Social (p =.02) from pre-intervention to postintervention. Changes in the hypothesized direction, trending toward significant, for the OT-SI were seen for Leiter-R Attention (p =.07 compared to the AP ), SSP Total Score, and CBCL Internalizing Composite from pre-intervention to postintervention. Children in the AP exhibited greater, but nonsignificant, results on Vineland Socialization from pre-intervention to post-intervention than the other two s. Children in the no-treatment exhibited greater, but nonsignificant, results on the CBCL Externalizing from pre-intervention to post-intervention than the other two 6

7 s. The OT-SI showed greater reductions in EDR data than other s, but results were not significant from pre-intervention to post-intervention. Effect sizes were reported for Leiter-R (Attention and Cognitive/Social;.29), SSP Total (.08), Vineland Socialization (.14), CBCL Externalizing (.10), CBCL Internalizing (.07), and GAS (1.62). CONCLUSIONS State the authors conclusions that are applicable to answering the evidence-based question. Findings suggest that OT-SI intervention may show significantly greater effectiveness in improving goal attainment, attention, and cognitive/social difficulties in comparison to the active (AP) or passive (No Treatment) placebos in children ages 3 to 11.6 with SMD in this small-sized study. It also is suggested that this study is the beginning of investigation to prove the effectiveness of OT-SI intervention.
