8 Full Medical Report Post Examination Review by Doctor **Flu Vaccination only available during January - August Medical Reports Medical reports will be collected by examinee during the review of medical findings. Request for group reports with consolidate findings must be specified in writing. Note: Hitch partners is advised to contact our Health Screening and Wellness Centre at least 3-5 working days prior to book an appointment. Reminder s and calls will be made to Hitch partners, if required. At the health screening facility, Hitch partners would be required to show Grab Hitch ID at a mode of identification for the health screening service 8

9 ANNEX A INSTRUCTIONS FOR HEALTH SCREENING TO PATIENT 1. Please DO NOT EAT or DRINK after pm the night before the appointment date. Fasting is required for accurate assessment of your glucose and cholesterol levels. A glass of plain water may be consumed, if so desired, on the morning of the appointment date. Breakfast will be served after the blood sample is drawn. 2. Postpone taking any morning medication, unless otherwise advised by your physician, until after your screening. 3. Your stool and urine specimen will be collected on the morning of your appointment. 4. Containers / stool cards will be provided. 5. Please bring along your previous medical records, if any. 6. As we will be testing your vision, please remember to bring along your glasses for distant and near vision. 7. The entire screening will be completed in one morning. 8.
