1 JESSICA 16 year old, female patient. She is a high school student and goes to a nearby high school. She has a fever, diarrhea, and a rash on her belly. She has had heavy menstrual cycles recently. BOBBY 28 year old, male patient. He is a businessman who works in downtown Minneapolis. He has pain when he urinates, as well as an unusual discharge. He is sexually active and has recently has unprotected sex.

2 ELMER 40 year old, male patient. He is a cattle rancher. He has had severe headaches and nausea. He also has a couple of skin sores or ulcers. He is commonly around livestock and their wastes all day. ABBY 10 year old, female patient. She is in 4 th grade at a local elementary school. She came home from school with a sore throat and nausea. She has started to cough up yellowish fluid. There has recently been a lot of illness at the elementary school.

3 LOU 38 year old, male patient. He is a used car salesman at a local dealership. He once had a circular rash on has left leg. Now, he is often fatigued and has a fever and chills. He spends much of his free time outdoors, either hiking or camping for many days at a time. MABEL 68 year old, female patient. She is a retired school teacher who still does volunteer work at a local school. Initially, she developed a sore throat. Soon after, she had a stiff pain in the neck and started vomiting.

4 MARIE 22 year old, female patient. She is a graduate student studying bat populations in South America. She recently got back from a trip to South America complaining of a fever and sensitivity to light. She has felt more fatigued recently. JIM 14 month old infant, male patient. He seems to have problems with his breathing. X-rays show that there is some hardened tissue in the lungs.

5 MARCUS 35 year old, male patient. He is a stockbroker for a downtown firm. He has had sever fevers and chills. He has also developed some symptoms of nausea and vomiting. He recently came back from a trip to southern Mexico. While he was there, he had many insect bites. CONNIE 27 year old, female patient. She is a graduate student studying mathematics. She has severe diarrhea and vomiting. Tests indicate that she is dehydrated. She recently came back from Africa two days ago. When she was there, she was staying in villages that did not always have clean drinking water.

6 RYAN 60 year old, male patient. He is a postal worker for the Minneapolis area. He complains of headache and constipation. It seems that he has lost some quickness in nerve function. He thinks that he may have eaten something that made him feel this way. TOM 7 year old, male patient. He is currently in second grade at a local elementary school. He has a sore throat and a fever. He also seems to have enlarged portions of his neck. It seems that a small rash has just started to develop on his skin.

7 JOHN 43 year old, male patient. He is a business owner who travels to find items to sell in his stores. He complains of a sore throat, diarrhea, loss of appetite, and chills. He was traveling about 2 weeks ago. Many of his travels were to countries that did not have excellent sanitation. He often stayed in inns/hotels with potentially poor water supplies. JENNIFER 8 year old, female patient. She is a third grader in a local elementary school. She has been vomiting and has been coughing consistently. Skin color is pale and blue.

8 MORRIE 29 year old, male patient. He is a postal worker in the area. He complains of a stiff jaw, muscle shaking, and a headache. Recently, he was bitten in the leg by an unknown dog, leaving a large puncture wound in his leg.
