1 Influenza Situation Update 14 October Influenza surveillance summary This surveillance summary includes countries where routine surveillance is conducted and information is available. Countries in the temperate zone of the northern hemisphere In the countries within the temperate zone of the Northern Hemisphere, Influenza-Like Illness (ILI) activity is declining and seasonal patterns are being observed. Japan In Japan the ILI activity continued to be low, following seasonal pattern. 2 The proportion of patients with pneumonia among hospitalized patients has increased over the past month and is over the tolerance limit. 3 Republic of Korea The proportion of patient visits to sentinel physicians due to ILI continues to remain below the National baseline level (12.2) and below activity at this time in 2010 to 2013 year. Hong Kong (China) The overall influenza activity remained at the baseline level. The ILI consultation rates at sentinel general outpatient clinics showed a decrease compared to the previous week (Figure 6). 4 The ILI consultation rates observed at sentinel private doctor sites showed a slight increase in the past week however was lower than 2012 and The average consultation rate for ILI among sentinel private doctors was 29.4 ILI per 1,000 consultations. (Figure7) Figure 7: ILI consultation rates at sentinel private doctors (Source: Hong Kong Centre for Health Protection) In week 40, hospital admission rates with principal diagnosis of influenza for persons aged 0-4 years, 5-64 years and 65 years or above were 0.12, 0.01, and 0.01 cases per 10,000 people in the age group, respectively (Figure 8). Singapore The average daily number of patients seeking treatment in the polyclinics for acute upper respiratory infection (ARI*) decreased in week 40 (1839) compared to week 39(2099).

(Figure 9) Figure 9: Singapore average daily attendances for ARI (Source: Singapore Ministry of Health) Southern China (No new update) During week 38, 2014, the influenza activity slightly decreased but followed the seasonal trend of previous years in south China (Figure 10). 6 Countries in the temperate zone of the southern hemisphere Overall ILI activity has continued to decrease, following seasonal patterns. Australia Throughout the country, seasonal influenza activity has continued to decline following recent peaks in activity. As of 28 September 2014, the sentinel general practitioner ILI consultation rate decreased markedly. Currently, the rate of ILI consultations is within the range observed at a similar period in recent years (Figure 11). There have been 5,900 notifications occurring in the two weeks ending 26 Sept Figure 12 shows the decline in notifications, showing seasonal patterns. 7 New Zealand The current rate of influenza-like illness is below the seasonal threshold (Figure 13). The number of ILI cases in Federated States of Micronesia, Guam, New Caledonia, Northern Mariana Islands, Solomon Islands and Tokelau were above the threshold of 90% of historical values in week 40. 9 Northern China (No new update) During week 38, the percentage of specimens that were tested positive for influenza in north China was 3.2%, this was lower than the previous week (4.3%). 10 Countries in the tropical zone Hong Kong (China) Among the respiratory specimens received in week 40, 38 (2.0%) were tested positive for seasonal influenza viruses, including 8 influenza A(H1N1)pdm09, 16 influenza A(H3), 12 influenza B and 2 influenza C. (Figure 19) Figure 19: Data from Hong Kong Centre for Health Protection Singapore The overall prevalence of influenza among ILI samples (n=92) in the community was 28.3% in the past 4 weeks. Of all the influenza virus isolates in September 2014, influenza A(H3N2), influenza B and influenza A(H1N1-2009) comprised 63.2%, 31.6% and 5.3%, respectively. Southern China (No new update) During week 38, the percentage of specimens that were tested positive for influenza in south China was 8.9%, this was lower than the previous week (11.7 %).

Influenza A (H3N2) viruses were predominantly reported (Figure 21). Figure 21: Influenza Positive Test Reported by Southern China Laboratories (Data from China National Influenza Center) Countries in the temperate zone of the southern hemisphere Australia Influenza A (H1N1) pdm09 is the predominant subtype in Australia (Figure22). Specimens were collected from 45% of Australian sentinel practices research network (ASPREN) general practitioners ILI patients. Of these patients, 23% were positive for influenza, compared with 30% in the previous fortnight.
