1 International SOS China Before You Go Print See your doctor and dentist and ensure you are in the best health before you leave. Other preparations: Check your routine vaccinations are up to date (polio; varicella; measles, mumps and rubella; tetanus, diphtheria and pertussis). See a travel health practitioner 6 to 8 weeks before departure for destination-specific health preparations. You may need additional vaccinations, some of which require several doses, or be recommended malaria medication which may need to be started a week or more before arriving in the malarial country. Documentation: Arrange a copy of your personal health record to carry with you when you travel. Include a letter from your doctor explaining your need for all medications you are carrying, including any over-the-counter medications, in English and the language of your destination(s). Make sure you have copies of your prescriptions. Medication: Check the regulations of your destination country regarding importation of your medication, as some drugs may be strictly prohibited (especially narcotics and psychotropics) and may result in severe penalties. Take any medicines you require in their original packaging, including any information leaflets, with them clearly labelled with your name (matching your passport name), and your doctor s name. Have enough to cover the trip, and extra in case of delays, however note that many destinations limit quantities of certain drugs to a 30-day supply. Carry medication in your hand luggage, with copies of your prescriptions. Vaccinations for China Recommendations may vary for short-term visitors. Always consult your travel health advisor or contact International SOS to discuss your specific needs. Hepatitis A Hepatitis B Japanese encephalitis Recommended for all travellers and expatriates. Recommended for all travellers and expatriates. Vaccine is recommended for people who will participate in higher risk' activities while in an area where Japanese encephalitis risk exists.
2 You are engaging in a higher risk' activity if you: Travel during the peak Japanese encephalitis season (consult the Diseases section of the International SOS country guides to see specific season details for this country). Spend a significant amount of time outdoors, particularly in the evening and night-time, in areas outside of cities. (ex. camping, trekking, biking, fishing, hunting, farming). Stay in accommodation that will likely have mosquitoes indoors, ex. lacking air conditioning, window screens, and bed nets. Spend a month or more in a risk area during transmission season. Polio Japanese encephalitis vaccine is not available in many of the risk countries. Have the complete vaccine series before departure. Polio occurs. A booster is recommended. Consider for certain travellers, especially: Rabies For expatriates and long-term visitors. For children who tend to play with animals and may not admit to being bitten or scratched. For travel to areas where quality medical care (including international standard rabies immuneglobulin) may not be available within 24 hours of being bitten or scratched by an animal. If contact with dogs, monkeys or other potentially rabies-carrying animals is likely. Jogging increases your risk of dog bite. Tickborne encephalitis May be recommended for those visiting forests, fields or pastures during the summer tick season or who drink unpasteurized milk. Recommended for adventurous and long-term travellers, especially those who will: Typhoid fever Visit friends and relatives. Eat from local vendors or restaurants. Be exposed to conditions of poor sanitation. Visit smaller cities or rural areas. Yellow fever Required for anyone > 9 months old arriving from a country with a risk of yellow fever transmission, including people
3 whose flight has transited through those countries. Malaria (Deciding whether to have a yellow fever vaccination is complex. Always discuss with your travel health professional well in advance of your trip. If vaccination is required by a country on your itinerary, you may need proof that you were given the vaccination at least 10 days before entering that country. The nation's official vaccination requirements for entering travellers are detailed above, though these may be implemented unevenly by immigration personnel on the ground. Your health practitioner may recommend vaccination for entry purposes beyond the 'officially' stated policy - particularly if you've been in a country where yellow fever circulates within the past 6 months.) More on diseases in China Spread By: Insect Bites Malaria is present in areas of China. Chloroquine-resistant and mefloquine-resistant P. falciparum malaria are present. There is no malaria in: Urban areas, or Areas above 1500 m (4,921 ft) elevation. Malarial areas: Most malaria is seen in rural areas of Yunnan province, followed by Hainan. Lesser risk occurs in rural areas of the provinces of Anhui, Guizhou, Henan, Hubei, and Jiangsu provinces. Drug resistance varies by region: "Multi drug-resistant" (chloroquine-resistant and mefloquine-resistant) malaria occurs in southern China, along the China-Myanmar border in western Yunnan province. Doxycycline or atovaquone/proguanil (Malarone) are recommended for prophylaxis in these areas. See the CDC map. Chloroquine resistant malaria occurs in Hainan and all other parts of Yunnan. Chloroquine sensitive malaria occurs in Anhui, Guizhou, Henan and Hubei provinces. Travellers to other areas where malaria may occur, and those on river cruises that enter areas with malaria, are generally not recommended to take chemoprophylaxis. However they should focus on mosquito bite avoidance.
4 Malaria is transmitted by mosquitoes that usually bite from dusk to dawn. Symptoms can develop as early as six days or as late as several months after exposure. Early malaria symptoms are flu like, such as head/body aches and generally feeling tired and unwell. Untreated, it can cause complications including anaemia, seizures, mental confusion, kidney failure and coma. It can be fatal. Malaria in this country is resistant to chloroquine, and in some areas is resistant to mefloquine. PREVENTING MALARIA Prevent mosquito bites. Chemoprophylaxis. If visiting malarial areas, use a medication to prevent malaria. These include: o Atovaquone plus proguanil (Malarone and generics) o Doxycycline (many brands and generics) o Mefloquine: do not use if travel includes resistant areas (see CDC map) Continue malarial prophylaxis while exposed to malaria. Stopping the medication while still exposed leaves you susceptible again to the dangerous consequences of malaria. Standard of Care Print Emergency Response Always try to call International SOS whenever medical care or advice is required, especially in emergencies. Overall, the public ambulance service is poor in mainland China. However, the availability, quality and efficiency of ambulance service have been significantly improved in Beijing and Shanghai after the country hosted the Beijing Olympic Games in 2008 and Shanghai EXPO in All cities have a government emergency service accessible by dialing 120. Response times can be very slow due to traffic congestion, poor telecommunications and understaffing. When you call an ambulance, the operator will require a brief description of the medical problem. This can be difficult, because most operators do not speak English. Ambulances that respond to 120 calls may not have well trained medical staff or appropriate medical equipment on board, and so they must take patients to the nearest medical facility. Beijing has a private ambulance service provided by the Red Cross. Dial 999 to contact them. This is a more flexible service than most 120 ambulances, and the staff may agree to bring a patient to the hospital of their choice. Emergency Numbers Fire 119 Police 110 Standard of Health Care
5 Although the standard of health care in mainland China has improved in recent years, it remains well below internationally accepted standards. Many hospitals have the same highquality medical equipment found in Western hospitals, but diagnostic abilities and treatment procedures can vary greatly. Serious medical conditions should only be managed in wellknown hospitals and may require medical evacuation, often to Hong Kong. Most large cities have a number of well-trained doctors. However, their effectiveness is limited by poor nursing standards, lack of hygiene and sporadic quality control in most Chinese hospitals. Large government hospitals in Shanghai, Beijing and Shenzhen have opened VIP wards that deliver a higher level of care, hygiene and comfort. These hospitals also have more English speaking staff. OutPatient Care International clinics in Beijing offer in-house expatriate doctors, ambulance, pharmacy and dental services, laboratory, X-ray, ultrasound and telemedicine. House calls can also be arranged. Primary and specialist care can be arranged in other major cities. In smaller cities and rural areas, referral to the most suitable local clinic is available. If possible, take a translator with you. Also, keep in mind that these facilities may appear "run-down", the doctor may not speak English, and there may be little privacy. Paying for Health Care Doctors and hospitals will usually expect immediate cash payment. Some of the large hospitals are beginning to accept major international credit cards, though only during regular business hours. Unfortunately, foreign health insurance plans are not accepted directly by the hospital. Patients may be expected to pay a deposit at the time of admission. Do not defer medical treatment because of financial concerns. Contact International SOS, and if our terms allow, we will make financial arrangements on your behalf. Dental Care In the large urban regions of mainland China, such as Beijing and Shanghai, high quality dental services are available. The dentists may be trained locally or abroad and offer a general range of dental services, including dental hygiene, extraction, dental fillings and orthodontic work. These dental clinics are usually clean, modern and equipped to an international standard. Avoid dental care in less developed regions. Blood Supplies In China, blood may not be screened to international standards, so the blood supply is considered UNSAFE. Also, there is a shortage of blood and blood products in most Chinese hospitals, especially Rh negative blood. If a blood transfusion is required, evacuation to Hong Kong is recommended. Even in areas where the blood supply is considered safe, it's best to avoid blood transfusions
6 if possible. Screening cannot detect every blood-borne disease, and immune reactions can vary from minor to life-threatening. If a blood transfusion is recommended and circumstances permit, seek a second opinion from International SOS or your health advisor. Medication Availability Foreign manufactured medications and vaccinations are not widely available in China, even in the major cities. Regulations prevent the commercial importation of many brand name medications and vaccinations, including the anti-malarial brand Malarone. High quality "joint venture products" - medications produced in developed countries and packaged in China - may be available. Locally manufactured medications are often considered to be of a lower standard and should be avoided. Avoid small local pharmacies, because they may sell locally produced goods. Use only large government pharmacies and pharmacies in clinics designed for expatriates and visitors. The foreign clinics in Beijing and Shanghai do require a doctor's prescription. People who will stay in China for a long time are permitted to hand-carry a one year supply of medication with them through customs if they have a prescription. Since brand names vary, know the generic (chemical) names of your medications. It is always advisable to bring an adequate supply of prescription and other medications from your home country. It may be advisable to drink bottled water only, especially on short trips. Always wash your hands with soap before eating, or use an alcohol-based hand sanitizer. When purchasing bottled water, ensure that the lid is securely fastened. Avoid ice, as it may have been made from unsafe water. Food Risk Food served in large hotels and well-known restaurants should be safe, but always choose
10 food that has been cooked while fresh and is served hot. Ask local friends to advise you of the "safe" restaurants. Avoid street vendors and market food, because the standard of hygiene may be low and food may not be fresh. Do not buy pre-peeled fruit or salad. Fruit that you wash and peel yourself should be safe. Avoid shellfish. Ensure that milk and other dairy products have been pasteurized.
