2 Foreword by Dr. Joshua Austin, DDS As a general practitioner, I am always looking for ways to increase my productivity, without sacrificing quality. Often times, that is not possible. Doing something faster can sometimes mean cutting corners and skipping steps. In fact, it is rare that we are able to find ways to do something faster AND better. 3M s newest bulk fill posterior composite is one of those rare unicorns. Fewer increments means fewer steps and less time per restoration. That helps the bottom line. It also means less chance of contamination and a lower risk of voids. That helps the quality of my restorations. The first iterations of bulk fill composites were not without their downsides, however. For what we gained in time savings, we lost in esthetics. Many bulk fill composite materials are too translucent and grey. That was somewhat by design. The more translucent, the more light could be penetrated deeper into the material. The result was restorations that, while fundamentally sound, left much to be desired esthetically. 3M s new Filtek One Bulk Fill has changed the way I look at bulk fill materials literally! When I finish and polish my 3M Filtek One Bulk Fill restorations, I see the most esthetic bulk fill material I have ever used. 3M has taken the best parts about bulk fill materials, the speed and ease of placement, and combined that with the esthetics and polishibility that has become synonymous with the name Filtek. As a clinical columnist and product reviewer for Dental Economics, many materials make their way through my office. I get to try out, demo and play with a lot of different stuff. I will use a material for 10 or 20 restorations and give my thoughts on it. After that, it is back to my regular armamentarium. I like the esthetic properties of it, as well as the handling properties and the ease of use. It s easy to handle and will be a great product to use on patients who want less chair time. Working efficiently, without compromising high-quality clinical results, is one of the main keys to achieving success, especially when performing time-consuming procedures, such as posterior restorations. I prefer bulk fill composites, which are designed to shorten procedure time and provide a strong restoration with high fracture resistance that is long-lasting and durable. Case Study A 42-year-old male patient presented with decay in the upper left first molar. I recommended 3M Filtek One Bulk Fill Restorative, which has an updated formula that turns more opaque as it cures, meaning that the final result is extremely esthetic. The Procedure 1. Cavity Preparation: The first step was to prep the cavity by isolating the tooth and placing a base material (Vitrebond Light Cure Glass Ionomer Liner/Base, 3M). 2. Etch: The enamel was etched for 15 seconds. 3. Apply adhesive: 3M Scotchbond Universal Adhesive was applied to the entire cavity, scrubbed 20 seconds, air-dried and light cured for 10 seconds (Elipar DeepCure-S LED Curing Light, 3M). 4. Placement of bulk fill composite: 3M Filtek One Bulk Fill Restorative was placed. The composite must also be light cured according to instructions for use. 5. Finishing & Polishing: Finally, a carbide bur was used to shape the restoration and the 3M Sof-Lex Spiral Finishing and Polishing Wheels were used for polishing. Conclusion The result of this case is a restoration that took minimal effort, while also satisfying the patient. The rise in the popularity of bulk fills has led to drastic improvements in the materials that are on the market, which saves crucial time while providing impressive esthetics, which means that both myself and my patients are pleased with the final result! 7 Performing Efficient Posterior Restorations How to Save Time Without Cutting Quality in a Bulk Fill Composite Read the full case here By Daniel Poticny, DDS* As a doctor, I always want to make sure that any product I m using for restorations is wear resistant, blends in with the surrounding tooth structure, and will be long-lasting. 3M Filtek One Bulk Fill Restorative has changed my perception of composites in general. I have concluded that composites can be routinely performed without compromising the outcome I desire for my patients in significantly less time by comparison to traditional composites. Case Study In the following case, a 34-year-old female needed to replace two restorations that were both fractured, chipped, leaking and had recurrent caries. The Procedure 1. Cavity preparation: I recommend using a rubber dam or isolation device to prep the area. 2. Apply adhesive and light cure: Selective etch is recommended when using 3M Scotchbond Universal Adhesive. Then, cure the adhesive interface. 3. Perform the restoration: Dispense the material in one increment, then sculpt and light cure using 3M Elipar DeepCure-S LED Curing Light. 4. Lastly, polish: I use the 3M Sof-Lex Diamond Polishing System, which I think is one of the best polishing products on the market. Conclusion After using 3M Filtek One Bulk Fill Restorative on my own patients, I am confident that it can be used in cases to provide esthetic results with significantly less time and effort than with traditional posterior composites. 8 Easy and Efficient See why high adaptability is crucial when it comes to successful molar and pre-molar caries restorations By Dr. Giuseppe Marchetti* High adaptability is key Another case that exemplifies the importance of excellent adaptation was the treatment of a carious lesion of second lower premolar by Dr. Filippo Dini*. Case Study This case involves a 35-year-old female patient who came into the dental clinic with primary and secondary caries. These caries were located in the upper premolar and molar teeth and were causing the patient a lot of pain. Challenge When performing direct restorations such as these, it is critical to have excellent adaptation to ensure good marginal contact. When shaping the material, it is also important for it to be easy to handle and provide excellent sculptability. Initial Situation: Second lower right premolar with a deep carious lesion restoration that required endodontic treatment. Image is showing the final preparation after removing the temporary filling. Final restoration is very natural-looking and esthetic. Initial Situation: Replacement of an insufficient amalgam restoration of first upper molar and treatment of carious lesion on premolar. Patient was experiencing pain. Amalgam and carious tissue removed. Placement and adaptation of the sectional matrix. Enamel is etched for 15 seconds using 3M Scotchbond Universal Etchant for a selective etch approach, followed by rinsing and lightly drying. 3M Filtek One Bulk Fill Restorative following contouring and finishing. 3M Filtek One Bulk Fill Restorative, in shade A2, is directly placed into the cavity in a single increment and then cured according to the Instructions for Use. 3M Scotchbond Universal Adhesive is applied and scrubbed into the surface for 20 seconds. After air drying for approximately 5 seconds, the adhesive is light cured for 10 seconds with 3M Elipar DeepCure-S LED Curing Light. After using the 3M Sof-Lex Pre-Polishing Spiral, a final high-gloss polish is created using the 3M Sof-Lex Diamond Polishing Spiral on a moist surface.
