Hello, This membership packet will help you understand your Smilebuilderz Dental Plan (SDP) as well as includes important information about Smilebuilderz and the services we offer in all locations. SDP is not dental insurance. It is a discount dental program. If you have signed up for a family membership, all immediate family members are included in the dental plan. (Children under 18 do not require cards). Your SDP card is immediately active. Remember, with SDP your exams and x-rays are always free. If you need a new patient exam or want a second opinion, the service is free. Individuals who are not currently patients with us can expect the following when they schedule their first appointment and free comprehensive exam. We will spend about 80 minutes with you. A thorough review of your dental history will be completed at that time and we will take a complete set of x-rays. You will then receive a comprehensive oral exam from the dentist. Your dentist will review your x-rays and high resolution color images of your teeth with you so you are completely educated about the status of your oral health. Your dentist will also show you how to properly care for your teeth and gums. Again, there is no charge for this visit. Urgent Dental Care is open 7 days a week, no appointment is necessary. Are there waiting periods for enrollment or procedures? A. No, your SDP card is immediately active. Q. Can I use my Smilebuilderz Dental Plan card just for my children s braces or my spouse s root canal? A. Yes, you can use any of the specialists at Smilebuilderz including Orthodontists (braces), Endodontists (root canal), Periodontists (gum and implant), Pediatric Dentists (children s care), and Oral Surgeons (wisdom teeth extraction).
