3 5CLEANING CORONAL 6OBTURATION 7 RESTORATION Obturation / Root canal filling The function of root canal filling is to obturate the canal and eliminate all portals of entry between periodontium and the root canal. It should provide an hermetic seal in order to: Prevent new infections Wall in potential remaining bacteria after shaping and irrigating Fill the remaining spaces to create a favourable biological environment for healing Root canal filling cannot compensate for poor shaping and / or incomplete disinfection.

4 Techniques: summary and protocol THE TECHNIQUE MOSTLY LEARNT AT SCHOOL Lateral compaction (cold) 1. Select the master cone of gutta-percha based on the largest file used, and the corresponding sized finger spreader 2. Coat canal walls and the master cone with sealer before sliding the master cone to working length (drawing 1) 3. Place the spreader into the canal alongside the master cone, then compact laterally under vertical pressure (not by pushing sideways) (drawing 2) 4. Remove the spreader 5. Place an auxiliary cone in the canal, compact with the finger spreader (drawing 3) 6. Repeat the previous step as long as you can fit an auxiliary cone 7. Cut the cones at the coronal part using a heated carrier (drawing 4) THE FASTEST, THE CHEAPEST, YET THE MOST CHALLENGED QUALITY Single cone (cold) 1. Select the gutta-percha master cone corresponding to the size of the last file used 2. Coat the root canal and the gutta-percha cone with sealer 3. Insert the gutta-percha cone into the apex of root canal 4. Cut the cone at the coronal part using a heated carrier 4

5 CONTROLLABLE AND TRIDIMENSIONAL OBTURATION Warm vertical compaction (manual / warm) 1. Select a gutta-percha cone corresponding to the last file used, and adjust it 1-2mm back from the apex 2. Prepare three sizes of manual pluggers and heat carriers (small, medium, large) 3. Irrigate and dry the canal. Coat the gutta-percha cone with a small amount of sealer 4. Insert the master cone into the canal and cut the coronal end with the large heat carrier 5. Heat the gutta-percha cone in the canal with the large heat carrier Take away the upper part of gutta-percha by pulling out the carrier Compact the gutta-percha cone with the large plugger Repeat the same steps with the medium and the small heat carriers and pluggers, ensuring the apical 3-4 mm segment of the canal is filled This step is called the down-pack (drawings 1 and 2) 6. Coat the coronal part of the canal with sealer and backfill, heating small segments of a gutta-percha cone and condensing them with the large plugger (by waves) This step is called the backfill (drawings 3 and 4) flows in to secondary canal down-pack 4 backfill Warm vertical compaction with the CALAMUS (mechanised / warm) 1. The downpack part of this obturation procedure is similar to the manual vertical compaction but executed with the CALAMUS electronical heat carriers and pluggers 2. The backfill: The backfill hand piece contains a cartridge of gutta-percha which, when heated, is injected into the canal using the easy-touch activation Use it to fill the middle third in a continuous wave (drawing 1 & 2) Compact the gutta-percha with a manual plugger (drawing 3) If needed, repeat to fill the coronal third in a continuous wave backfill Continuous Wave: The remaining portion of the root canal (middle and coronal thirds) is filled with heated gutta-percha in one single step. 5

6 Techniques: summary and protocol 1 2 QUICK AND EASY TO LEARN CONVENIENT FOR ALL CANALS, ESPECIALLY LONG AND CURVED ONES Carrier-based technique (warm) 1. Verify the apical diameter with a size verifier. If you feel tug-back, it s the correct size. Insertion occurs in one action with very little opportunity to revise the result if the carrier does not go to length, therefore this is an important step (drawing 1) 2. Select the carrier-based obturator, corresponding to the size of the last shaping file used (confirmed by the size verifier) 3. Coat the canal with a thin layer of sealer. Remove excess sealer with a paper point 4. Heat the obturator as per DFU s instructions 5. Insert the obturator in one single smooth motion into the root canal until it is snug (drawing 2) 6. Cut the handle with a bur FOR THE INITIATED DENTIST Thermoplastified condensation (warm) 1. A gutta-percha cone coated with sealer is placed into the root canal 2. Select the appropriate gutta-condensor (with the needed length and diameter); place it on your motor, and into the canal (drawing 1) 3. The gutta-condensor is engaged, at a speed of 8000 t/m in the canal (drawing 2) 4. It will frictionally warm, plasticise and compact the gutta-percha in the root canal (drawing 3) 6
