Key of Endodontics The three most important points for the success of Endodontics are as follows: 1. Access opening and canal preparation. 2. Sterilization of the root canal. 3. Obturation of the root canal Complete filling is necessary. Above all, item 1 is the most important point for the removal of an inflammation factor. FIG.1 FIG.2. Feature of the Obturation System Obturation System which we present today is one of the vertical condensation system and the syringe technique using Obturation Gutta Toyo chem.labo.inc.japan. The formar lateral condensation method is adaptable for a relatively straight or narrow root canal but it seems not for a somewhat curved or wide canal. The present method can operate easily in the latter case and moreover in a short time with a small quantity of the Obturation Gutta, so we think that the present method is ideal for the root canal filling. 4 FIG.14 FIG.15 In other words, A dental cusp is removed with a dental cusp removing bur. rest, a wide external form is obtained and an exact canal length can be measured. Trepanation with a round bur. A pulp chamber is opened with a butt cone bur. it is accomplished in a short time A dentinal bulge is removed with a butt cone bur to make easy to see through the inner part of a root canal and to insert an instrument. The root canal is made straight to the middle part of it by the method of the former item 4 when the root canal is curved strongly Convinsience form, Otani's Method for instance, 67 medio buccal canal and 67 medio canal. The canal preparation is made by a Kerr file, Triple file, Erweiterer reamer, Endo-dia bur or an apical reamer. The completion of the canal preparation is checked by a sound checker, the above-mentioned items are Otani's canal preparation method.. Composition of the Obturation Gutta The Obturation Gutta contains more amount of gutta percha than that of the former gutta percha point and the injurious ingredient to a tissue is excluded as shown by the slide. FIG.16 FIG.17 Experiments on living animals show a capsuling after 2 weeks without a cellular infiltration on a paradentium as indicated by the slide and it proves that the Gutta is harmless. There is a problem that the Gutta shrinkes, but it passed the experiments of premeation of coloring matter for 72 hours as shown by the slide and we obtained Japanese patent and U.S. patent for the Gutta.. Outline of the Obturation Gutta The syringe is filled with the Obturation Gutta and warmed at about 47. Then, the warmed Gutta of a semi-fluid state is injected into the root canal. The Gutta is pressed in just before it stiffen with a plugger so as to fill a lateral canal and a ramification. Even somewhat wide canal can be filled by one operation and the Gutta can be pressed into the curved canal 4mm ahead of the plugger, so it is not necessary that the syringe reaches to the apex of a curved canal. -3-

Method of the Obturation System A plugger is selected which reaches 3 4mm before the apex of root among Obturation System Kits or Schilder's plugger and a rubber stopper is adjusted to the working length. The root canal must be made completely dry with a suction paper point and a heat carrier in this method a rubber dam must be used for the prevention of various troubles and moisture by breathing. ThetipofasyringewhichisstuffedwiththeObturationGuttaiswarmedbyanalcohollamp. The Gutta extruded first is excluded and the syringe is warmed slightly again and carried in the root canal. The filling of the Gutta Percha may be done one operatoin or more. Then, the Gutta Percha is pressed in the canal by the selected thin plugger first and thicker ones in turn, and the root canal is filled tightly. But, the Gutta Percha softened must be pressed in while it is gradually harden, so the principle of paradental and four-handed Endodontics must be held fast to accomplish the purpose with an assistant, not a doctor only. FIG.18 FIG.19. Apical cone technique Otani-1979 It is apt to become over filling when the last file for the most narrow part of the apex of root is above #35. In this case, the Gutta percha cone which is 2 marks thicker than the final file is selected and cut in 3 4mm length and inserted in the root canal without a sealer. The over filling is avoided by carrying the apical cone technique and making an apical plug on the apex of root. The residual space is filled by the Obturation System and the operation is completed.
