1 September 27, 2017 Dear Prospective Student: City Colleges of Chicago s Dental Hygiene Program is now accepting applications for the class of 2020, which will enter the program in the 2018 summer session. Malcolm X College is a brand new School of Health Sciences that houses all City Colleges healthcare programs. The new $251 million campus offers students the opportunity to learn in a state-of-the-art healthcare training facility developed with input from industry leaders, complete with a virtual hospital, a dental hygiene patient care clinic, and easy access to employers in the Illinois Medical District for clinical rotations, internships and job opportunities. To be considered for the class of 2020, the Dental Hygiene Program must have your completed application and supporting documents on file by February 1, Neither applications received after February 1 st, nor incomplete applications will be considered. Please note that prerequisite courses must be taken in semester hours, not quarter hours. If you are attending a college or university that is on a quarter system, you will need to speak with an advisor. Please read the enclosed information carefully. All information needed to successfully apply to the dental hygiene program is included in this packet. We strongly recommend early submission of your complete application to ensure all necessary items are included. Other important information regarding applying to the dental hygiene program: 1. You will be required to take a drug test and complete a background screening. Information will be provided to you regarding these upon admission to the program. 2. In addition to the application, you will need to submit the following: Two official copies of high school transcripts or GED completion. Two official copies of transcripts from all college institutions attended. Please note that 1 official copy of all transcripts must be on file at Malcolm X College, Office of Registrar Services, Room (Electronic transcript submissions should be sent to mxregistrar@ccc.edu.) The second official copy should be sent with your admission packet materials to MXC Dental Hygiene Admissions Committee. If you have taken classes at any of the City Colleges of Chicago (CCC), you do not need to submit your CCC transcripts, simply include your name, student ID number, and dates of attendance in a letter. 3 CITY COLLEGES OF CHICAGO DENTAL HYGIENE PROGRAM 2017 FACT SHEET WHY CITY COLLEGES OF CHICAGO? The City Colleges of Chicago Dental Hygiene Program at Malcolm X College has the distinction of being the only dental hygiene program in the City of Chicago. It provides the dental hygiene student with excellent opportunities available when enrolled in a community college with access to the exceptional professional resources through community-based clinics and the new Malcolm X College of Health Sciences. The program has a successful track record of the students passing both clinical and national licensing examinations scoring well above the national average. The program in dental hygiene is accredited by the Commission on Dental Accreditation. The Commission is a specialized accrediting body recognized by the United States Department of Education. The Commission on Dental Accreditation can be contacted at WHY DENTAL HYGIENE? Dental Hygienists, as members of the oral health team, are licensed professionals who provide specialized educational, clinical and therapeutic services in preventive oral healthcare. Employment opportunities for dental hygienists are excellent in a variety of health care settings and are expected to grow through the 21 st century. The majority of dental hygienists are employed in clinical practice settings. Other career options include office management, extended care facilities, dental product sales, corporate educational consultant, insurance companies, public health clinics, educational institutions, and research. WHAT ARE THE ELIGIBILITY REQUIREMENTS? The program is very competitive and having the minimum requirements does not indicate that you will be accepted into the program. Students having less than the minimum requirements will not be considered. In order to enter the program students must: Be at least 18 years of age; Graduate from an accredited high school or earn a GED; Meet general admissions requirements for City Colleges of Chicago including Successful completion of all prerequisite courses for the Dental Hygiene Program; Have a minimum cumulative GPA of 2.5 in all college level work Proof of current certification in CPR Basic Life Support at Health Care Provider Level 2 letters of recommendation Autobiographical statement that addresses why the applicant chose Dental Hygiene as a career WHAT ARE THE PREREQUISITES? Prior to entering the dental hygiene program, prospective students must complete the following classes with a grade of C or higher: **General Biology with a Lab (At CCC Biology 121) 5 hours 4 semesters hours with a lab also is acceptable **Anatomy and Physiology (At CCC Biology 226 & 227) 8 hours Has to be 8 hours Can be anatomy & physiology I and II 4 hours each Can be 4 hours of anatomy and 4 hours of physiology **Microbiology (At CCC-Micro-233) 4 hours Speech (At CCC- Speech 101) 3 hours 20 hours 3
4 **Basic Sciences must be completed within 5 years of entering the program. WHAT ARE SUPPORT COURSES? Other general education courses are required prior to graduation from the program. It is highly recommended that these courses be taken prior to entering the dental hygiene program. However, they may be taken during the program. Support courses that have been completed prior to entering the program will provide extra evaluation points during the admissions evaluation process. Those support courses are: General Education Mathematics (At CCC Math 118 or Higher) Chemistry 121- Basic Chemistry English Composition I (At CCC English 101) General Psychology (At CCC Psychology 201) *CCC indicates City Colleges of Chicago 4 hours 4 hours 3 hours 3 hours 14 hours CRITERIA FOR SUCCESSFUL PROGRAM COMPLETION 1. In order to advance to the next semester, the students must complete all dental hygiene courses in sequence with a final grade of C or higher. 2. In order to receive an Associate of Applied Science in Dental Hygiene, students must complete all program and degree requirements established by the Dental Hygiene Program and Malcolm X College. 94 credit hours 3. Upon successful completion of the Program, students must take and successfully complete the national and regional board exams prior to obtaining professional licensure. All students must pass appropriate didactic and clinical program requirements and be evaluated as progressing satisfactorily in the last semester of the dental hygiene program prior to being deemed eligible for application to take the written Dental Hygiene National Board Examination and State or Regional Clinical Examinations. 5. In order to become licensed as a dental hygienist in the State of Illinois, the student must complete the Dental Hygiene Associate in Applied Science Degree, meet eligibility requirements for the Illinois Department of Professional Regulation s Acceptance of Examination and pass appropriate written and clinical boards. 5
6 PROGRAM GOALS AND STUDENT OUTCOMES GOAL A: Assist students in developing critical thinking skills, basic research skills and a respect for individual differences so that they can provide quality comprehensive patient-centered care to all patients. Objectives 1. Provide students with a framework for the ethical care of patients; 2. Provide instruction in current clinical practice with emphasis on patient-centered care; 3. Integrate basic science, behavioral science, and clinical practice; 4. Provide the opportunity for critical thinking and the use of evidence-based research in provision of oral health care to patients; GOAL B: Accept qualified students and provide them with an education and support they need to graduate and meet their professional/employment/goals. Objectives 1. Provide instruction in a body of knowledge that is well grounded in the basic sciences; 2. Recruit and educate qualified students; 3. Provide retention programs to enable students to complete their education. GOAL C: Provide students with opportunities for community service and professional participation so that they develop skills to become socially responsible. Objectives 1. Provide broad-based and varied clinical experiences; 2. Provide clinical experiences in a variety of external settings; GOAL D: Provide students with the knowledge and skills necessary to pursue alternative roles related to dental hygiene, dentistry, and public health. Objective Provide a basis for effective interaction with other health professionals Student Outcomes: Upon Completion of the Dental Hygiene Program, the student will: Interpret and apply aspects of dental sciences including tooth morphology, head, neck and oral anatomy, histology, oral pathology, radiography, pain management and dental materials. Perform the following skills: oral health education, preventive counseling, pain management, clinical dental hygiene, radiology competency, provision of services and management of patients with special needs, community dental/oral health, medical and dental emergencies, infection and hazard control and management for patient and operator safety. Express a personal philosophy of social responsibility through various clinical and outreach experiences. Explain a personal philosophy of professional responsibility that is based on principles of ethical decision making and adhering to legal and regulatory concepts. 6
7 Assess, plan, implement, and evaluate an oral health program. Demonstrate effective communication and interpersonal skills to provide care in a multicultural setting. Apply principles of evidence-based research in order to make decisions regarding delivery of care Critically self-evaluate and implement corrective action to prepare for life-long learning; MALCOLM X COLLEGE DENTAL HYGIENE PROGRAM BLOOD-BORNE PATHOGEN POLICY Blood-Borne Pathogens Students and faculty may be providing care to patients with conditions caused by blood-borne pathogens as well as other infectious diseases. For this reason, students are required to have the Hepatitis Vaccine and provide evidence of TB test, 1. Care is provided to all patients unless their medical condition should be under further control prior to treatment for the benefit of the patient. 2. Faculty & students may not refuse to treat a patient 3. Dental hygiene faculty, staff and students are obligated to respect the privacy and confidentiality of all patients. 4. All health care providers and faculty will use universal precautions when treating each patient regardless of the health status of the patient. 5. Students and faculty are obligated to follow all patient care policies and procedures of the Malcolm X College Dental Hygiene Program and is affiliated clinics. The Malcolm X Dental Hygiene Program has an infection control and clinic manual. In the Clinic Manual it is stated that the goal of the College s Infection Control Manual is to protect College personnel and patients from cross infections related to the dental clinic environment. Dental health care providers are responsible for monitoring their own health status. When a dental health care provider is known to have an infectious disease it is their professional responsibility to take steps to prevent its spread. The management of the College reserves the right to restrict individuals from providing patient care or impose restrictions on dental health care workers in the event that an infectious disease is deemed to be at a level that presents unsatisfactory risk of transmission. The current best research will be used in making such a decision. Infection Control Policies The Dental Hygiene Profession willingly complies with the guidelines set forth by OSHA and the CDC. Any student, faculty, or staff not complying with the Malcolm X College Dental Hygiene Program and its affiliate clinic s Infection Control Protocol will be dismissed from the clinic, lab or classroom. 7
8 ADMISSIONS INSTRUCTIONS FOR THE DENTAL HYGIENE PROGRAM The absolute deadline for everything to be received is February 1, The first step is to apply for admissions to City Colleges of Chicago (if you are not already a student). Click on link to apply: Apply to City Colleges of Chicago 2. The second step is to visit the department webpage and complete the Health Science Program Selective Admissions Application and transcript submission process to Malcolm X Registrar. High School Transcripts: If you have not already submitted high school transcripts to Malcolm X College, it is imperative that you submit 1 copy of your official high school transcript to MXC Registrar Services. College/University Transcripts: If you have attended any of the City Colleges of Chicago (CCC), you do not need transcripts from CCC institutions. It is imperative that you 1 submit copy of each official grade transcript from all colleges and universities you have attended outside of City Colleges of Chicago to MXC Registrar Services. (Each hard copy transcript has to be in a sealed envelope from your college. For hard copy submission, please include this entire address: 1900 West Jackson Blvd, Attn: Office of Registrar Services, Room 1404, Chicago, IL For electronic submission for official transcripts, have the institution send them directly to mxregistrar@ccc.edu). If you are taking courses during the Fall 2017 semester, please make sure the transcripts are current and include your final grades for Fall If you are enrolled in a pre-requisite course during the Spring 2018 semester, please make sure there is a copy of your enrollment status included in your packet. Once this second step is submitted, the Admissions office will review the student s academic history and official college transcripts to determine qualifications. If the applicant meets all qualifications, the applicant will receive the Dental Hygiene Program Applicant Information for Admission form. 3. The third step is to complete the Dental Hygiene Program Admissions Packet and submit the following information in one large envelope. Write on the front of the envelope your name; student ID number; and Dental Hygiene. The completed packet is submitted to the Malcolm X College Student Services Office [The Heart] room The packet must include: 1. Dental Hygiene Program Applicant Information for Admission Form 2. One official copy of your high school transcript or GED (in addition to the one sent to MXC Registrar) 3. One official copy of transcripts from every college you have attended (in addition to the one sent to MXC Registrar) 4. A 2x2 inch sized photo printed on matte or glossy photo quality paper 5. Two letters of recommendation from professor, employer, or community leader 6. Proof of Current Certification in CPR Basic Life Support at the Health Care Providers Level 7. Personal Statement- A one-page statement addressing why you chose dental hygiene as a career 8. (**optional, yet strongly encouraged Verification of Dental Hygienist Observation Hours) In addition, the student must: 8
9 1. Complete a Background check and drug screening. Information will be given to you upon admission into the program. 2. Provide documentation of Hepatitis B, pertussis, tetanus, and diphtheria vaccinations and Annual TB (Tuberculosis) test. More information concerning this documentation will be provided upon admission to the program. HOW WILL MY ADMISSIONS PACKET BE EVALUATED? The dental hygiene program admissions committee evaluates all completed applications. Incomplete application submissions will not be considered. Thirty students will be selected to enter the program in the summer session of 2018 at Malcolm X College. Admission to the Dental Hygiene Program is competitive and completion of all requirements does not guarantee admission. Candidates are evaluated based upon the following criteria: 1. Math and Science GPA from all colleges and all courses 2. Cumulative GPA from all colleges and all courses 3. Autobiographical Statement 4. Two (2) Letters of Recommendation 5. Dental or health care experience involving direct patient contact 6. If accepted into the program, you will be required to have a background check. Additional Information will be provided when you turn in your acceptance form. 7. If accepted into the program, you will be required to have a drug screening Additional information will be provided when you turn in your acceptance form. Preference is given to City Colleges of Chicago students, followed by residents of the City of Chicago. WHEN WILL I KNOW IF I AM ACCEPTED? If you apply to City Colleges of Chicago/Malcolm X College, and receive notification from the College that you have been admitted, that only means you have been admitted to the College. You will be contacted by from the Dental Hygiene Program Director if you are accepted into the Dental Hygiene program. Notification of acceptance or non-acceptance will first be by with a follow-up letter during the first week of April Please do not call the Dental Hygiene Program to inquire about the status of your application. Applicants who are not accepted may reapply for the following year. *Acceptance in the Dental Hygiene Program will come from the Dental Hygiene Program Director. 9
