Be sure to mention if you are insured by Minnesota Healthcare Programs and/or Medical Assistance. Also, do not hesitate to call the toll free number on the back of your insurance card and ask for assistance in locating a dentist for your pay source (see individual toll free numbers listed on the last page of this document.)
