1 MM01-Facebow And Bite Registration Procedure Checklist Bite fork Registration OK Redo 1. Estimate the shape of the patient s arch 2. Place the bite tabs on the bite fork 3. Warm the compound in hot water 4. Check the softness of the compound 5. Check how hot the bite fork is 6. Imprint the maxillary teeth on the bite fork compound 7. Place the bite fork in ice water for a few seconds Bite Registration OK Redo 1. Place the wax wafer in warm water 2. Cut the bite wafer 3. Check your patient s overjet 4. Fold the anterior bite wafer to fit the overjet 5. Place the folded wax wafer back in the warm water 6. Complete a one-step bite registration 7. Label the top side of each bite registration 8. Cool both bite registrations in ice water for a few seconds Facebow Registration OK Redo 1. Instruct the patient how to hold the facebow 2. Insert the bite fork into the bite fork apparatus and line up the blocks 3. Get the A lined up on the bite fork apparatus 4. Insert the bite fork into the patient s mouth 5. Insert the A of the bite fork into the A of the facebow 6. Check that the A is inserted all of the way 7. Tighten the A screw 8. Instruct the patient to slowly lift up the facebow 9. Have the patient hold the facebow in his ears 10. Position the facebow parallel to Frankfort Horizontal 11. Tighten the front screw of the bite fork apparatus 12. Position the crossbar rod of the bite fork 90 degrees to the vertical bite fork rod and parallel to the crossbar of the facebow 13. 2 14. Firmly hold the bite fork apparatus and tighten the screws one last time 15. Grip the facebow, ask the patient to open wide and remove the ear pieces from his ears 16. Remove the facebow and bite fork from the patient 17. Loosen the A screw and remove the bite fork apparatus from the facebow 18. The posterior bite registration captures the patient s true bite position between the molars 3. The bite registrations are between 1 to 3 mm thick at the thinnest part 4. The bite imprints are distinct and sharply delineated with no smears 5. The bite registration is accurate without distortion 6. The bite registrations have been cooled but are not brittle or cracked 7. The top side of each bite registration is labeled with Sharpie marker pen with the patient s initials 8. The bite registrations are stored safely with the patient s impressions and later with the patient s mounted models Page 3 of 6

4 MM03-Facebow Registration Product Checklist Facebow Registration OK Redo 1. The facebow was held in the patient s ears in the correct position 2. The patient was biting into the distinct tooth imprints in the bite fork correctly holding it in position against the maxillary teeth 3. The A of the bite fork was inserted correctly into the A of the facebow and the A screw was tightened correctly 4. The facebow arms were aligned parallel with the patient s Frankfort horizontal plane 5. The front screw of the bite fork apparatus was tightened completely 6. The crossbar rod of the bite fork apparatus was positioned parallel with the crossbar of the facebow and 90 degrees to the vertical bite fork rod 7. The underneath screw was tightened correctly holding the crossbar rod in place 8. The facebow and the bite fork were removed from the patient without disturbing the bite fork registration 9. The bite fork apparatus was removed from the facebow correctly without disturbing the relationships of the bite fork 10. The bite fork apparatus was stored safely with the patient s bite registration until the models were mounted on an articulator Page 4 of 6

5 MM04 Mounting Models Procedure Checklist Preparing The Models OK Redo 1. First debubbilize the mandibular impression 2. Mix and vibrate hard stone and pour the mandibular model 3. Shape the base of the mandibular model and the tongue area 4. Debubbilize the maxillary impression 5. Mix hard stone and pour the maxillary impression 6. Separate the models from the impressions 7. Remove bubbles from the models 8. Wet the models before trimming 9. Trim the back of the maxillary model 10. Mark a trim line around the maxillary model 11. Trim the maxillary model along the trim line 12. Wet and trim the mandibular model especially along the back Mounting The Models On The Articulator OK Redo 1. Insert the bite fork rod into the slot on the articulator 2. Screw on a mounting plate 3. Place some support under the bite fork 4. Mix a small amount of plaster and mount the maxillary model 5. Remove support under bite fork and attach the articulator to the base plate 6. Open the articulator and remove the bite fork assembly 7. Place bite registration on the maxillary model, attach the mandibular mounting plate, check the clearance 8. Secure the models together with a rubber band 9. Mix some plaster and mount the mandibular model 10. Check that there is space between the two models 11. Remove both models from the articulator 12. Fill in and finish the base on the mandibular model 13. Fill in and finish the base of the maxillary model Page 5 of 6

6 MM05 Mounted Models Product Checklist Mounted Models Criteria OK Redo 1. The models are accurate without distortion, bubbles, broken teeth, and were poured in hard stone 2. There is a distinct straight line of demarcation at the depth of the vestibule between the stone model base and the plaster base 3. The mandibular model tongue area is flat 4. The models were trimmed correctly with proper shape, the back perpendicular to the maxillary median palatal raphae or the mandibular dental midline 5. The bases of the models are filled with plaster and smooth with a distinct line of demarcation from the stone model 6. The maxillary model has been mounted to the top arm of the articulator in a position similar to the patient s Frankfort Horizontal Plane 7. The mandibular model articulates in the correct bite position with the maxillary model when the models are brought together and the pin is set at zero 8. There is no interference preventing the models from articulating together properly 9. The models are labeled properly with the patient s name, date of impressions and facebow transfer, and the patient s age Page 6 of 6
