English, DDS Little Rock, Arkansas When performing implant placement in the interforaminal symphysis area of the mandible, control of the environment is very important. This minimizes or eliminates salivary contamination of the implant which enhances the probability of successful integration. The author has been employing the NOLA Dry Field System with placement of IMTEC MDI implants for nearly years with great success. The NOLA system was invented by New Orleans orthodontist Dr. Jack Hickham in the mid-80 s. It was developed for the placement and bonding of orthodontic brackets as it controls tongue restraint, cheek retraction, and salivary evacuation. These are the same requirements needed for MDI placement. NOLA Dry Field System Some patients have very little control of their tongue placement and try to help during the procedure, but end up creating more interference. The tongue holster allows the patient to be more relaxed during the procedure as their tongue is secure. In addition to the evacuation system, the author places a rolled 2 X 2 in the floor of the mouth in front of the tongue guard for additional salivary control. Cheek retraction is very important as it allows more access to the premolar to premolar implant sites. Gagging has not been seen as a problem with this device in my practice. In fact, I think it helps rather than hinders. Patients with large tongues or with a tongue thrust tendency can be slightly problematic when using the winged wrench tool for screwing the MDI into place. In that case, I merely use an instrument or an assistant s finger to push the tongue back slightly for ample clearance. All things considered I find this NOLA Dry Field System to be unique and very applicable to techniques requiring isolation in the anterior portion of the mouth. CASE 1: NOLA manages excess salivation and aggressive tongue. CASE 2: NOLA controls saliva, tongue, and lips for flap procedure.
