2 Congratulations! You ve taken a positive first step by talking with your doctor about your symptoms of depression. No doubt you re committed to getting the most out of depression treatment by communicating with your doctor and taking your medicine as prescribed. But sometimes, despite your best intentions, you may experience certain setbacks during your treatment, such as side effects. Side effects are one of the most common reasons people stop taking their medicine. If you develop side effects that might cause you to stop taking your medicine, contact your doctor immediately. Your doctor will decide the best way to handle any side effects you might experience. Your doctor may consider changing your medication, the amount of medicine you are taking, how many times you take the medicine each day, or even the time of day you take your medicine. 1 During the course of your treatment, your doctor will follow your progress. At times during treatment, you may find that your prescription may be changed. If your prescription is changed for any reason or your treatment program is complete, be sure to discuss with your doctor how to dispose of any unused medicine. You may find that the form on the back of this sheet may help you keep track of your medicine and your progress. Always keep an open line of communication between you and your doctor, and take your medication exactly as prescribed. Remember, at any time during your treatment, if your thoughts should turn to suicide, call your doctor immediately. The contents of this brochure are for informational purposes only. 3 My medicine isn t working. Everyone responds differently to medicine. Sometimes even after a number of weeks, you may still have symptoms like irritability, low energy, or a lack of motivation. This doesn t necessarily mean your medicine isn t working. Speak with your doctor about your concerns. You and your doctor should decide together whether to change your medicine, change your dosage, or discontinue treatment. My family and friends don t know what I m going through. Ask your doctor about adding talk therapy to your treatment plan. The additional guidance of support groups, for example, can help you learn from the positive experiences of people who have been where you are now. I don t enjoy doing all the things I used to. Getting back to the activities you enjoyed before you became depressed may help measure your progress. You may start noticing that you are doing at least one of your favorite things and may be thinking about doing more. If not, you should talk with your doctor. 3 of 8

Fill out a new sheet for each visit, as your feelings may change from week to week and from appointment to appointment. Together, you and your doctor can use your Progress Report to build understanding and begin a discussion about your treatment plan. 1. Who can I turn to when I need to talk? 5. What is my current situation at work? How have my feelings toward work changed now that I m taking my medicine? 6. 6 For each of the following questions, draw a circle around the number that best describes your situation. Changes should be recorded on the scales from 1 (no change) to 10 (significant change) and should be discussed with your doctor. 7. How do I rate my energy level?
