1 Diabetes Newsletter I N S I D E T H I S I S S U E : I S S U E S I X M O N D A Y 1 6 J U L Y Round-up 1 Congratulations HCP training Bexley diabetes team updates 2 3 & 4 Congratulations to all the surgeries regarding 2011/2012 QOF targets for HbA1c. Since 2009 Bexley has achieved a 4.9% improvement in patient s glycaemic control. Percentage of patients with an HbA1c <7.5% (58mmol/mol) has improved from 70.5% to 75.4% and the variation between practices has reduced by 21%. X-PERT and patient survey Diabetes website and patient support group news 5 6 Diabetes ball a great success On a lovely, rain free evening at Danson Boat House, good fun was had by all. The dance display by Carla Rana and Dan Hudson encouraged everyone to dance till midnight. We look forward to witnessing Ethan Harris Faulkner s dancing after the lessons he bid for in the auction. 2 P A G E 2 HCP training update 2012 The training programme so far this year has been a big success. We have run our second diabetes injectable training course and are looking into gaining credits from a university. We developed and ran a successful training programme for local pharmacists and our first foundation course for healthcare assistants. The 3-day diabetes foundation course was well evaluated and will run again next spring. Healthcare professional training opportunities Course Date Venue CPD Injectable Update for Tier 2 HCPs An update session for all tier two HCP s that have already completed the Warwick Insulin for Life course or Bexley Injectable modules. 20th Sept 9.00a.m - 5p.m Holiday Inn, Bexley Year of Care: Care Planning The national 1 ½ day training for any practices who have not yet embedded the care planning process into their diabetes annual reviews. It is advisable for two HCP s to attend per practice, including those actively involved in completing annual reviews. 26th Sept 9am-5pm & 24th Oct 9am TBC Consultation skills A half day refresher for staff involved in the care planning annual reviews that are keen to practice their consultation skills related to care planning. 24th Oct 1.30pm- 5pm TBC 2012 Evening Diabetes Meeting A summary of the 2012 audit and findings from the Diabetes Practice Development team, collecting your diabetes reports and any updates we have. 28th Nov 7pm (time TBC) TBC To register for any of the above courses please contact Natasha Collett on natasha.collett@bexley.nhs.uk or or speak to your named Diabetes Specialist Nurse 2

3 I S S U E S I X P A G E 3 Book your in-practice Virtual Consultant Diabetologist visit (KPI 8 of the LES) Both Dr Ibrahim and Dr Mehmet will visit your practice and sit with members of the practice team to discuss individual patients you have identified your diabetes audit presenting a teaching session of your choice during their visit. They will require an outline of what you would like covered when you book Updates from the team Practice Development Team Audits. The 2012 Audits have now started. John Davis will be contacting your practices to arrange a date to complete your audit between July and September. Karen Ellard will then be in contact to arrange the Practice Development team follow-up visit. Please try and arrange for as many members of your diabetes team to be present. Local Enhanced Services (LES) for 2012/13 You will have received the Local Enhanced Service for Diabetes practice teams which was sent out in June. We have received 15 signed contracts back from Practices. We would appreciate prompt return of contract sheets stating your interest in taking part in this year s LES. Also 14 Practices have not sent in their invoice for 2011/12 LES payment. DART- Diabetes Accurate Referral and Triage form The DART referral form is in the process of being reviewed and edited to make the referral options more specific and reflect the price changes for the low risk podiatry service. Nail Cutting Service price increases There has been an increase in the price for the nail cutting service: from 1 st July Clinic appointment from the 1 st September 2012 Domiciliary Visits will be: initial visit per visit thereafter. 3

4 P A G E 4 Updates from the team continued... Year of Care template for vision The new YOC template for Vision was remotely installed at the end of June. The recording of depression screening onto the journal has been improved and all other issues reported have been dealt with. Please report any future problems with the template to Carla. carla.rana@nhs.net Metformin titration card Available to distribute to patients when they are first prescribed Metformin. It is available to download on the Bexley Diabetes website and can be ordered from Natasha. natasha.collett@bexley.nhs.uk Carbohydrate awareness handout This has been developed as a teaching aid to use with patients when discussing the amount of carbohydrate in their diet. The general message is in line with X-PERT all carbohydrate foods turn to glucose and it is the total quantity of carbohydrate consumed that impact on the blood glucose level. The handout gives examples of 15g portions of carbohydrate and also references the book Carbs and Cals by Chris Cheyette and Yello Balolia. It is available to download on the Bexley Diabetes website. Pre-diabetes handout We have written a handout for people diagnosed with Impaired Glucose Tolerance / Impaired Fasting Glucose. It now has to be set out in booklet form and will be available for distribution and to download from our website from September

5 I S S U E S I X P A G E 5 X-PERT diabetes patient education programme Update Bexley records the baseline, six months, annual and two yearly outcome data of patients who have attended the programme anonymously onto the X-PERT national database and this is benchmarked with all the other centres who offer X-PERT. Bexley won the National X-PERT Health Award for the greatest improvement in glycated haemoglobin of those who had attended the programme, at the first national awards ceremony. Since January 2010 just over 1,900 people with diabetes have attended X-PERT. We currently offer the sessions at 8 different venues in the borough, 11 courses are running in June and July with 13 courses set to begin in September covering mornings, afternoons and evenings in the week and on Saturdays. If you would like to learn more about what X-PERT has to offer or to observe the programme please contact the diabetes team, on /12 Bexley diabetes patient satisfaction survey Thank you to those of you who were involved in disseminating the survey at the beginning of this year. Full results can be seen on the Bexley diabetes website. 642 surveys were returned and specific issues addressed were medication, education, diabetes care and physical activity. Highlights of the survey showed that respondents understood that medications were important to their health and generally adhered to their medication schedule and that many of them would like further information and advice about their medication. The survey also demonstrated that those who attended structured education felt more involved in the decision making regarding their care. Many respondents had issues regarding foot care information and the services available. Finally the main barriers for people not increasing their physical activity were concerns over their mobility and general health and the cost of organised activities. Another survey will be carried out in the autumn of this year so that the results can inform the LES and future planning for the diabetes service. 5

There are plenty of other useful pages for patients to browse Local patient support group The Diabetes UK local support group welcomed a new chairman earlier in the year, Graham Saunders. Our thanks go to Helene Brenchley-King for all the years she dedicated as Chair. Helene continues her role as an X-PERT trainer and as the patient representative on the Practice Development Team For more information about the local group and for details about their educational meetings and speakers and the various activities they organise, please visit their website on They are always happy to have anyone attend their evening meetings that take place on the third Friday of every month.
