Your health is a crucial aspect of your life. That s why the Yakima Heart Center offers this booklet; to help you identify the numbers that affect your health and well-being. Knowing your medical history and getting annual checkups are vital steps to healthy living. Attaining optimum health is a life-long process, and YOU are responsible for it. The Yakima Heart Center invites you to use this booklet as a tool to help you identify the numbers that affect your health. Record your numbers regularly using the charts in this booklet. Then bring in this booklet to your next appointment with your doctor.
Your name: DOB: Male / Female Weight: Date: Weight: Date: / / / / / / / / / / / / Do you use tobacco: Yes / No If no, the date you quit using tobacco: / / List of current medications:
BLOOD PRESSURE Blood pressure is the force of blood pushing against blood vessel walls during a heartbeat. A blood pressure reading consists of two numbers: the top number (systolic) represents the pressure in your blood vessels during a heartbeat, and the bottom number (diastolic) is the pressure in your blood vessels as your heart rests between beats. High blood pressure is a major risk factor for heart attack, stroke, kidney failure and heart failure.
LEVEL Less than 120/80 CATEGORY Normal /80-89 Prehypertension /90-99 Stage 1 Hypertension 160+/100+ Stage 2 Hypertension HEART RATE Blood pressure and heart rate are not the same! Measuring your heart rate does not indicate high or low blood pressure. The greater your fitness level, the lower your resting heart rate will be. Date Measure Pulse/Heart Rate / / / / /

WAIST CIRCUMFERENCE Your waist circumference is a good indicator of your abdominal fat, which is another predictor of your risk for developing heart disease and other diseases. Determine your measurement by placing a measuring tape snugly around your waist, approximately one inch above your belly button. The risk increases with a waist measurement over 40 inches in men and over 35 inches in women. Date Inches
CHOLESTEROL Cholesterol is a waxy, fat-like substance that your body needs. But, when you have too much in your blood, it can build up on the walls of your arteries, leading to heart disease and stroke. There are no symptoms for high cholesterol. Many people have never had their cholesterol checked, so they don t know they re at risk. A simple blood test can tell your cholesterol levels. Date LDL HDL Cholesterol Triglycerides
LEVEL Total Cholesterol CATEGORY LEVEL Triglycerides CATEGORY Less than 200 Desirable 150 and below Desirable Borderline High Borderline High 240+ High 200+ High LDL Bad Cholesterol LEVEL CATEGORY HDL Good Cholesterol LEVEL CATEGORY Less than 100 * Optimal 60 and above Best Near Optimal Better Borderline High Below 50 (Women) High Below Very High (Men) Poor Poor * Less than 70 if you have vascular disease
BLOOD SUGAR The food we eat is turned into glucose, or sugar, for our bodies to use as energy. The pancreas, an organ that lies near the stomach, makes a hormone called insulin to help glucose get into the cells of our bodies. You can contract diabetes when your body either doesn t make enough insulin, or can t use its own insulin as well as it should. This causes sugar to build up in your blood. Date Measure
STAGE GLUCOSE Optimal Prediabetes Diabetes FASTING BLOOD SUGAR Less than 100 Between 100 and or higher, measured on two different days HEALTH IMPACT Healthy level Increased risk of cardiovascular disease and future diabetes Risk of damage to the eyes, kidneys, blood vessels, heart & nerves as well as cognitive decline and dementia
The Yakima Heart Center promotes your self-care and wellness. We can help you identify risk factors you may have for certain health conditions and give you the information you need to make healthier choices. Knowing your numbers is a good place to start!
