20 Enrolment Process Patient is invited to sign up for Patient Access, is provided with information and completes a signup form Clinician verifies the patient s identity and submits their request for data access to SCI-DC. SCI-DC passes details of patients requesting data access to Citizen s Account

21 Enrolment Process Citizen s Account create user accounts and authentication details as appropriate and send directly to the patient SCI-DC data is validated and transferred securely to My Diabetes My Way Patient uses authentication details received to access personal information via My Diabetes My Way
