Wurli prides itself n delivering a range f health services t the cmmunity in a style and manner that makes clients fell welcme and cmfrtable. Wurli perates a range f clinics and prvides a range f primary health care and ther services which include main clinic (acute and general care), Gudbinji Chrnic Disease Clinic, Binjari Health Centre, Wmen and Children s Health and Men s Health. The Diabetes Educatr is respnsible fr cntributing t imprving health utcmes fr Abriginal and Trres Strait Islander peple with chrnic health cnditins; in particular diabetes, thrugh better access t crdinated and multidisciplinary care. The Diabetes Educatr will prvide fr a range f services within a multi-disciplinary team including prvisin f diabetes self-management, educatin, and care and being a resurce persn fr healthcare prfessinals. 2 Prerequisites Fully Registered as an Abriginal Health Practitiner/ Nurse with Australian Health Practitiners Regulatin Agency (AHPRA) Natinal Plice Recrds Check and Wrking with Children s Clearance Current Nrthern Territry Driver s License Key Result Area Respnsibilities / Activities Standard Measure SERVICE DELIVERY Prvide expert delivery f safe effective evidence based clinical care and effective educatin t peple with Diabetes Type 1, Type 2, GDM, IGT, which will include but is nt limited t:- Cmmunicate with all clinical staff t ensure clients are referred t diabetes services apprpriately and are prvided service in a timely manner. Initiate treatment using CARPA standards as the clinical guidelines Supprting clients with clinical care including insulin stabilisatin Assisting with cntinuus glucse mnitring systems Prviding infrmatin and supprt when cmmencing insulin pump therapy Mnitr waiting list and make appintments accrding t needs f the clients referred Assess and prvide diabetes care, determine eligibility fr service and make referrals where apprpriate. In cnsultatin with the client, develp an apprpriate treatment plan and gals t achieve success. Prvide diabetes educatin and care interventins fr peple with diabetes in an individual and grup setting. Prvide feed-back and cmmunicatin where clients are invlved with ther services.
