1 YAKIMA SCHOOL DISTRICT CONCUSSION MANAGEMENT POLICY A student athlete that has sustained a concussion will be managed by the following policy described below: 1. Concussion management policy shall be in accordance with Washington State s Zachery Lystedt Law, House Bill A youth athlete who is suspected of sustaining a concussion or head injury in a practice or game shall be removed from competition at that time. A youth athlete who has been removed from play may not return to play until the athlete is evaluated by a licensed health care provider trained in the evaluation and management of concussion and receives written clearance to return to play from that health care provider. 2. Concussion management policy shall follow the recommendations of the Zurich Consensus Statement of Concussion in Sport. The cornerstone of concussion management is physical and cognitive rest until symptoms resolve and then a graded program of exertion to medical clearance and return to play.

On-Field or Sideline Evaluation of Acute Concussion:* When a student athlete shows ANY features of a concussion: (a) The student athlete should be medically evaluated onsite using standard emergency management principles, and particular attention should be given to excluding a cervical spine injury. (b) The appropriate disposition of the student athlete must be determined by the treating healthcare provider in a timely manner. If no healthcare provider is available, the student athlete should be safely removed from practice or play and urgent referral to a physician arranged. (c) Once the first aid issues are addressed, then an assessment of the concussive injury should be made using the SCAT2 or other similar tool if appropriately trained personnel are available. (d) The student athlete should not be left alone following the injury, and serial monitoring for deterioration is essential over the initial few hours following injury. (e) A student athlete with diagnosed concussion should not be allowed to return to play on the day of injury. 2. Subsequent Evaluation and Treatment of Concussion: (a) The cornerstone of concussion management is physical and cognitive rest until symptoms resolve. Physical rest - requires elimination of all types of exercise. The most the athlete can do physically while experiencing symptoms of concussion is walk. Cognitive rest - limiting activities that require concentration and attention (e.g., scholastic work, video games, text messaging, etc.) may lessen symptoms and promote a more efficient recovery. (b) A youth athlete who has been removed from play may not return to play until the athlete is evaluated by a licensed health care provider trained in the evaluation and management of concussion and receives written clearance to return to play from that health care provider. ** 3. 3 SCHOOL DISTRICT RETURN TO PLAY PROTOCOL ALGORITHM FOLLOWING CONCUSSION STUDENT ATHLETE WITH CONCUSSION ATHLETIC STATUS FLAGGED ATHLETIC STATUS UNRESTRICTED EVALUATION BY APPROPRIATE HEALTHCARE PROVIDER NO WRITTEN RELEASE TO BEGIN RTP PROTOCOL YES RECOVERY REVIEWED BY HCP SUCCESSFUL COMPLETION OF STAGES 1-5 SCHOOL OFFICIAL TO SUPERVISE RTP PROTOCOL CONSENT & WRITTEN RELEASE TO STAGE 6 SUCCESSFUL COMPLETION OF STAGE 6 RECORDS TO HCP / SCHOOL & FILED After a student athlete has sustained a concussion a school official will have his/her athletic status "flagged" until full recovery has been achieved and documentation of that recovery is filed. After a student athlete has sustained a concussion he/she must be evaluated by an appropriate healthcare provider (HCP), as defined by state law (ATC, MD, DO, ARNP, PAC). A written release must be obtained by the appropriate HCP who evaluated the athlete to initiate the defined graduated exercise return to play (RTP) protocol. If release is not provided, the student athlete will followup with an appropriate HCP. A school official will review and document completion of Stages 1-5 of the RTP protocol on provided forms. Following the successful completion of stage 5, recovery/forms must be reviewed by the HCP who provided the written release. Consent and written release by the HCP must be provided to advance to stage 6 (full athletic participation). A school official will review and document successful completion of stage 6. All forms will be presented to the HCP and filed at the student athlete s school before the student's athletic status is unflagged/unrestricted. The student athlete will not be allowed to continue participation until this is fulfilled.

Following completion of STAGE 2 I read and School Official School Official If there are any concussion signs or symptoms (if any response on the Symptom Evaluation is not 0 ) STAGE 3 of the Return to Play Protocol Following a Concussion.

6 FOLLOWING COMPLETION OF STAGE 3 I have completed STAGE 3 of the Return to Play following exercises: Jog for 15 minutes or Stationary Cycle or Swim for 30 minutes 3 Sets of 30 [ ]* Jumping Jacks 3 Sets of 30 [ ]* Sit-ups Sports Specific Drills for 20 minutes * If physically challenging but less than 30 reps. record in [ ]. Following completion of STAGE 3 I read and School Official School Official If there are any concussion signs or symptoms (if any response on the Symptom Evaluation is not 0 ) STAGE 4 of the Return to Play Protocol Following a Concussion.

9 FOLLOWING COMPLETION OF STAGE 6 GRADUATED PROVOCATIVE EXERCISES I have completed STAGE 6 of the Return to Play following: FULL PARTICIPATION / COMPETITON Following completion of STAGE 6 I read and School Official School Official If there are any concussion signs or symptoms (if any response on the Symptom Evaluation is not 0 ) If no post-concussive signs or symptoms occurred (all responses are 0 ) the athlete may continue full participation after all forms are returned to the school office. If any signs or symptoms of concussion return in the future the athlete must stop all exercise/athletic activity and report signs/symptoms to health care provider.
