2 Key Features Enhanced security features protect your health information and reduce waste, eliminating the need for printed copies. Complete your annual screening questionnaire and access direct links to schedule a screening appointment in one convenient location. View and track your screening results minutes after you screen. Review your monthly rewards statements within My Rewards Track your daily calorie intake, physical activity and weight loss goals. Access webinars and exercise videos on demand and at a time that works best for you.

3 Getting Started Go to and click on the Member Login button located at the top right hand corner. Click the text under Login which reads Not a member? to register. Accept the Healthy Lives Terms of Use and fill in your Company ID information and click Continue. Select your work location from the drop-down list. If you are a spouse of an employee, the location is determined by where your spouse works.

4 Getting Started Participant ID: Your unique identification provided by your employer. Username: Create your own username. Once you are registered, you will use this to log into the website. It must be between 6 to 16 characters long. Password: Your password must be at least eight (8) characters long. It must contain at least one uppercase letter. It must contain at least one number. It must contain one special character #, %, ^, &, *, -). Security Question/Answer: Select a question from the drop-down list and provide an answer to the question selected in the Security Answer box. This information is used to verify you if you need to reset your password.

5 Let s Take a Tour Homepage You can click on the Healthy Lives logo in the top left corner of the screen to get back to the homepage at any time. Here you can complete your Screening Questionnaire, view your screening results, and reward statements, review program resources, and more.

6 Fruit, Veggie and Water Trackers Use the trackers to keep log of your daily intake of fruits, vegetables and water. Trackers reset at 12:00 am CST. Move the mouse over fruit/vegetable circles to view similar calorie choices. Click on the fruit/vegetable circle to add 1 cup serving to your Nutrition Log. The number of servings will appear under Servings in the middle. Click on Servings to view your Nutrition Log. Click on + symbol to add 10 ounces of water consumed. Select symbol to remove. Click on the water glass to view your entry in the Nutrition Log.

7 Need Help? Contact Healthy Lives The Contact Us button at the bottom of the homepage allows you to send an message to the Healthy Lives office requesting more information and member support.

8 Complete HRA Questionnaire The first step to get a better understanding of your overall health and wellness is to complete the Health Risk Assessment (HRA) before attending your Healthy Lives screening. All fields marked with an asterisk (*) are required to be answered.

9 Complete HRA Questionnaire Click on the Enrollment Consent Form underlined below to review the form before checking the box on the left and entering your initials into the box on the right. After you click on Submit, you receive the confirmation below that your questionnaire was submitted successfully.

10 My Health Risk Assessment View and track your screening results minutes after screening within My Health Risk Assessments.

11 My Rewards If your organization offers incentives or rewards for participation, you can view your progress here. Statements are updated around the 15 th of each month.

12 Nutrition/Fitness/Weight Tracking Research shows that people who plan ahead and keep a useful record significantly increase their chance of success.

13 Nutrition Tracker Log your food to keep track of what you are eating each day to help you reach your goals.

14 Fitness Tracker Use the Fitness Tracker to help keep track of your daily activity. If your program awards points for activity, you must log 30 or more minutes of activity each time in order to receive accurate fitness points. If you exercise in short bursts, log all time as one session of fitness for the entire day.
