Submit Annual Physical Form Dental Check-Up (2) Visit your dentist for a Routine Bi-annual Oral Exam (2 Visits /Year) Vision Exam Visit your eye doctor for an Annual Vision Exam Primary Care Physician Provide proof of being under the care of a physician Teladoc Register for Teladoc a telehealth company that uses telephone and videoconferencing technology to provide on-demand remote medical care via mobile devices, the internet, video and phone. Annual Gynecological Exam as Annual routine gynecological exam (Pap Smear and/or general health) Mammography as Age-appropriate annual exam (starting at age 40; baseline at years of age Colonoscopy as Age-appropriate colonoscopy (age 50, then every 10 years; more if high risk) Prostrate Screen as Age-appropriate PSA test and/or digital exam starting age 40
3 EDUCATION Benefit Basics 10: 6 Times up to 6 of the NEX sponsored benefits classes. Weight Management Program 25: 2 Times Completion of a Physician controlled weight management program and/or program conducted by accredited organization. Note: Program duration is a 12-week minimum. (e.g. Weight Watchers) Tobacco Cessation Program 25: 1 Time a program using one of the following approved programs: American Lung Association, American Cancer Society, Ohio Quit Line or Physician directed cessation program. Diabetes Management Class 15: 1 Time a diabetes management program taught by a healthcare professional (Physician, Nurse, Dietitian, Diabetes Educator, etc. Financial Fitness Class(s) (Financial Health) 10: 5 Times one or more of the NEX sponsored Financial Health classes offered by 5/3 rd Bank. (Budgeting, Credit Score, Debt & Identity theft) Financial Fitness Series (Retirement Planning) 10: 1 Time the NEX sponsored Retirement Planning class. EDUCATION IN ACTION Weight Management in Action 1 Point per pound lost: 1 Times Loose weight and maintain the weight loss for a minimum of 3 months. Tobacco Cessation in Action 50: 1 Times Quit smoking for a minimum of 6 months Flu Shot 40: 1 Times Be vaccinated against the Flu virus
4 PHYSICAL ACTIVITY 30 Workouts 15: 4 Times thirty (30) 20-minute or more workouts. Workouts may consist of weight training, cardio, stretching, swimming, biking, etc.. Classes 15: 4 Times a 4-6 weeks fitness/exercise class. Classes include Yoga, Spinning/Cycling, Aerobics, Martial Arts, etc. Distance Event 15: 10 Times a distance event equal to or greater than 3.2 miles/5k. *Qualified registration required for events Digital Step Tracking (APP or Device) 1: 100 Times Meet step tracker goal of 8,000 steps with your digital tracking device. Note: Sync your Fitbit to the protal for automatic reporting. Can t sync or have no device complete and turn in 8,000 Step Form monthly. NEX Sponsored Health Challenges 10: varies one of the NEX Sponsored Health Challenge s offered throughout the year. KYN Health Coaching session 10:1 Time Review your data with a Christ Hospital Health Coach to learn more about what your biometric data means for you. TCH enter after completing coaching Low Risk for Metabolic Syndrome Up to 15: 1 Time Based off your biometric data, if you are at a low risk for metabolic syndrome, you will earn these points. No action required: Incentive evaluation Coaching for Metabolic Syndrome Risk 40:1 Time Based off your biometric data, if you have 3 of 5 parameters for metabolic syndrome, you can participate in 4 coaching sessions with a TCH health coach to help you lower your risk.
