3 Myelogram Why do you need a myelogram? Most people who have a suspected problem with the nerves in their neck or back can be investigated with an MRI scan. However, for some people an MRI scan is not safe because the patient has a pacemaker or other device inside their body. Alternatively, your doctor may wish to find out further information about your spine in addition to an MRI scan. In either of these situations you may be referred for a myelogram. page 3 of 8
4 What is a myelogram? A myelogram is an x-ray examination of your spinal cord and the nerves from your spinal cord. There is fluid-filled space around the spinal cord. A fine needle is inserted into this space either in the lumbar spine (low back) or occasionally in the neck, and the doctor, who is a Consultant Radiologist, injects dye or contrast. The needle is then removed and a series of x-ray pictures and then a CT scan are taken of your spine. You may be asked to adopt different positions, such as lying on your back and then on your side. What preparation is needed before the test? Tell the ward staff if you are allergic to anything, including any antibiotics or the dye used with X-rays. In addition, you should mention if you are taking any medication to thin your blood, such as Warfarin. On the morning of the test only eat a light breakfast. You will be given a gown to wear so that your back is accessible. A nurse will stay with you during the procedure. You may ask questions if you wish when you meet the radiologist. Is it painful? You will be given local anaesthetic but may feel some discomfort when the needle is put in and the dye is injected. This discomfort will only last for a few minutes. If you feel that it is too uncomfortable during the procedure please let the doctor know. There is likely to be a little bruising around the injection site which may be painful for several days after the procedure. page 4 of 8
5 How long does it take? A myelogram usually takes about 30 minutes to complete. You may, however, also have a CT scan afterwards, which will take at least another 30 minutes. What should I do if I suspect I may be pregnant or I am still able to have children? This test is not carried out on pregnant women. If you are pregnant, or suspect that you may be, please telephone the department and discuss this with one of the nursing staff. Women who are still having periods should have this procedure during the 10 days following the start of your menstrual period (this includes the time that you are menstruating). A suitable appointment will be negotiated with you. If you think that you may be pregnant or the procedure date is more than 10 days after the start of your period, please telephone the contact number on your letter. Are there any side effects? The injection may occasionally cause a leak of fluid from the spine and this may also cause a severe headache, particularly when sitting up or standing. If you do develop a headache take paracetamol or your regular painkillers and drink plenty of fluids, particularly tea and coffee, for the next 24 hours (avoid alcohol). This will help to flush the dye or contrast from your system and help to replenish the fluid in your spinal canal. Occasionally, the dye used during the procedure can cause an inflammation reaction (i.e. a chemical meningitis ). This will cause headaches, spinal pain and feeling unwell. It will usually settle over one to two weeks. page 5 of 8
6 Very rarely the injection may cause infection around the fluid in the spine and meningitis. If you develop a flu-like illness or back pain a few days after the myelogram, this may be a sign of infection. It is possible to cause damage to the nerves in the spine, including the spinal cord, although the risk of this happening is less than 1:1000. What will happen after the test? You will be taken to a comfortable chair for a cup of tea/coffee and we will monitor your blood pressure and pulse. You will need to stay in the department for 1 hour after the procedure. Do not do anything strenuous for 2 days and we recommend that you do not go to work the following day. Drink plenty of fluid for the first few days after the myelogram. You will need someone to drive you home. Please do not drive for 24 hours after the procedure. Is there anything I should look out for when I go home? It is extremely rare to have any problems once you have gone home. However, if you do feel unwell with symptoms such as an increase in neck or back pain, numbness or weakness in your arms or legs, or you feel feverish or generally unwell, you should consult your GP or your local Accident and Emergency Department.
