1 17IS PLENARY PRESENTATION CREATING COMMUNITY: CHANGING THE WORLD OF MENTAL HEALTH Joel Corcoran, Executive Director, Clubhouse International Good Afternoon! Well that was certainly a powerful morning. I hope that you are all becoming as inspired as I am about being part of the life- changing work of our Clubhouse International network. I am certainly very pleased to be here and to be part of this great biennial seminar. This gathering has been an energizing force for the Clubhouse movement for many years. Like a strong wind in the sails of a tall ship, our seminars lift us up and generate a renewed momentum that propels us forward. Each time I attend and hear the stories, the successes and the challenges faced and overcome by Clubhouse members I am renewed in my steadfast conviction that we have found a better way. I am re- charged and ready to stand side- by- side with all of you in chasing our vision of a world full of purpose and opportunity for people living with mental illness. We were last together in 2011 for an extraordinary five days filled with new learning, new friendships, new partnerships and new energy to change the way the world sees and interacts with people living with mental illness. Thanks to many of you who were there and to our hosts at Fountain House Stockholm we created that new momentum. After hearing this morning s presentations and knowing what s in our program for the next five days, I am certain we will do the same here at 17 IS. *** The last two years have not been easy for the world. The prolonged effects of struggling economies have challenged almost every one in every country. Nowhere have the effects been felt more than in social service programs. Mental health services and supports have not been spared. Clubhouse funding is as hard to find and sustain as it ever has been. This is not news to 1
2 any of you, and certainly Clubhouse International has not been without our own challenges. But like all of you, we press on and we create success with what we have. This morning we heard Jack Yatsko talk about the inspirational words and ideas of Martin Luther King Jr. When Jack told me he was going to anchor his remarks in the I have a dream speech, it made me think about another quote by Dr. King that I keep framed on my window sill. Here is what it says.. In any social revolution there are times when the tailwinds of triumph and fulfillment favor us, and other times when strong headwinds of disappointment and setbacks beat against us relentlessly. We must not permit adverse winds to overwhelm us as we journey across life s mighty Atlantic; we must be sustained by our engines of courage in spite of the winds. This refusal to be stopped; this courage to be; this determination to go on - in spite of - is the hallmark of any great movement. Yes, these are challenging times. Yes, we are facing some mighty headwinds. But we press on. Our work is too important to do otherwise. In our Clubhouse International community we are pushing ahead on the mission to change the world. We have continued to see more than 20 new Clubhouses start and join Clubhouse International in each of the last two years. We are making progress in published research, public awareness, and our training and accreditation programs. Our international network of Clubhouses is actively creating new projects and program innovations throughout the Clubhouse world. There are more wellness initiatives, young adult programs, social media projects, Clubhouse coalition advocacy efforts, and exciting Clubhouse publications. We are seeing our Clubhouse boards and advisory boards become stronger and attract more local community activists and resources. New Clubhouses are opening all over the world. We press on because we are part of a social revolution. We are insisting on a better world for people living with mental illness and those who love them. Our struggle is great because the problem is great, but our solution, our Clubhouse solution is even greater. Our theme for this seminar is A Place to Belong. Those four words - - A Place to Belong - - have been part of the Clubhouse tradition since the early days of training and the Clubhouse Expansion Program at Fountain House. 2
3 Belonging somewhere is something we as human beings take for granted. After all, from the moment we are born (even before) we belong somewhere. We are immediately connected to another person, a family, an extended family, a town and even a country. From the early days of our lives we join things, a school, a team, a church, a group of other children in the neighborhood. Joining and belonging continues all through our lives. We choose friends, schools, jobs, political parties, social and professional organizations. Belonging somewhere addresses basic human needs shared by all of us. Being included as a member of a group, being part of a community provides us with opportunities to manage important aspects of our self- image and sense of being. Belonging to a group (a community) gives us repeated opportunities to have friendships and love and a sense of self- worth. It connects us with others and that in turn provides us with the context for how we see ourselves and how others see us in the world. It also gives us a kind of power, because belonging provides us with repeated opportunities for meaningful activities and accomplishments. Belonging to a group gives us opportunities to contribute and to be acknowledged for those contributions. These are incredibly important and powerful aspects of the human experience. Unfortunately, as we all know, mental illness can and does cause great damage to a person s relationships and sense of belonging. A lot of the damage is because the world systematically excludes and discourages people with mental disorders from participation in valuable groups and many of life s rewarding experiences and opportunities. Additionally, mental health care and psychosocial services are not prioritized in most places around the world. Even when there is good access to mental healthcare there is usually not enough of the necessary support to help many people overcome the social isolation issues. People living with mental illness far too often become invisible to mainstream society and are therefore left out of opportunities for socially and economically satisfying lives. In my opinion, it is this sense of social isolation that can lead people to be desperate, or to give up on all that life has to offer. In the Clubhouse world we have found a way to help people re- engage and to re- claim the future in spite of mental illness. We are proving every day that when you add the element of belonging to a meaningful community into the total mix of good mental health care, that recovery is possible and that continued achievement is likely. 3
4 In another plenary session at this seminar we are going to hear from several speakers about how the experience of belonging to a Clubhouse community has pulled them out of their isolation and despair, and helped to give them their lives back. They will do a much better job than I ever could at discussing this topic. What I would like us all to consider is what we do - now that we know and are capable of demonstrating through personal stories, research and repeated successful outcome measures - the effectiveness of the Clubhouse approach. This morning in his welcoming remarks Tom Manning mentioned some of our accomplishments over the last 20 plus years as an international social movement. He mentioned standards, consistent and equivalent training, a very successful accreditation program, Clubhouse development across the world in many different cultures and communities, and an international supportive network. These are big accomplishments. We need now to build on our accomplishments and take the next steps to change the world. The time has never been better for us to influence the way the world sees and acts towards mental illness and the people it affects. For a variety of reasons there is a current public discussion going on about mental healthcare. It seems to be growing and intensifying across the world in a way I have not seen in 30 years. Struggling economies everywhere are forcing governments to re- examine how public money is spent on mental health. Recent high profile and horrific tragedies over the past few years in places around the world are driving a public debate about mental illness. The World Health Organization has identified depression as the fastest growing and leading cause of disability in the world. In addition, three more of the top ten causes of disability are mental disorders. The progress being made in medical science about the brain and better understanding mental illness is moving fast and gaining momentum. Social service systems, jails and judicial systems and educational institutions are beginning to raise a collective voice about the burdens of a failed social response to mental illness. I could go on, but my point is that the need for what we have is greater than it s ever been. On a consistent basis, accredited Clubhouses have more and better outcomes than virtually 4
5 anything else that is available. Accredited Clubhouses are extremely cost effective by comparison to most other programs and in relation the economic burden of mental illness. Accredited Clubhouses also always score well on any user preference scale. We have demonstrated to ourselves, and to a relatively small portion of healthcare and social service systems, the power of belonging to and being supported by a community. We need to expand now and reach the rest of the world. The time has never been better for us to increase our visibility and our reach. But, here s what I really want to say today. To do this, to remain effective and to really make a difference in the future and to scale up our impact we have to change. We have to take what we know about community, and belonging, and what it takes to overcome the issues, barriers and stigma associated with mental illness - and we have to help integrate this knowledge and value system into the larger response to mental illness. We are going to talk a lot more about community and belonging at this seminar. However I would like you to consider an additional meaning of community. Clubhouses are in the community in your city, your town, your county and even your country. We are right there with all of the other important elements of society, but we are mostly invisible to everyone else. We are far too isolated from day- to- day life in our communities. The world is changing. Mental illness is beginning to get mainstream attention for good and bad reasons. There is an opportunity for us to gain more support for accredited Clubhouses, to increase the number and diversity of Clubhouses and more importantly to help the world make the right choices about how to address mental illness. But to do this we have to come out from behind our Clubhouse walls and educate ourselves and participate in the dialogue with the rest of the world. We have to be in the healthcare debate, we have to be in the media discussions about mental illness, we have to be at the decision- making tables about public spending on mental healthcare. We have to be part of the growing coalition fighting for increased attention and resources for mental health. We have to become much more visible in our communities and we have to get better at advocacy and educating the public. Additionally, we have to recognize that where the world is evolving and advancing in mental healthcare is where we have to go in our Clubhouse programs, activities and services. The world is learning very fast that we have to begin to address mental illness much earlier in life 5
6 and that we have to help families and young people understand and identify mental illness sooner rather than later. We are starting to understand that we can help minimize the level of disability and segregation for people with mental illness if we can intervene earlier. Integration for mental health into the larger public healthcare system is happening. There is a growing understanding of when and how mental illness begins. Because of these things and the new public dialogue about mental illness, I believe we have an extraordinary opportunity to help eradicate mental illness stigma and to teach the world what we know about belonging and community and recovery. But we have to learn how to be in the dialogue and be part of our communities. We have to be much more than just a local program tucked away in the corner of our towns. This work I am talking about is not instead of our daily Clubhouse routines and work ordered days and employment and education programs. It has got to be part of how we work in our Clubhouse communities. The work itself, as we already know, is restorative. Within the context of our Clubhouse community we have to look at how we can use the powerful aspect of A Place to Belong and bring it with us as we participate in a new day for mental health. We can change the world. But to do that, we have to be in the world. During the rest of this plenary, and throughout the seminar, you are going to hear about what some Clubhouses are doing to be out in the community and to advocate raise public awareness and build strong alliances with other organizations. You will also hear about how some Clubhouses are reaching out to young adults, others are integrating wellness and general healthcare into their Clubhouse routines, and still others are using technology and media to advance their Clubhouse work. My challenge to all of us is: do not be afraid to get involved outside your Clubhouse walls and to take the best of our Clubhouse approach with you. The world needs us. 6
