1 The Neuro-rehabilitation enhanced transition team (NETT) A community neurological rehabilitation service for Lambeth and Southwark
2 Who we are We are a community team that aims to support you and your family from hospital to home. You will have a keyworker who is part of our highly specialist team. We will work with you on the areas that are important to you as you start to recover and rebuild your life. We will find out: what you can do at the moment what you want to be able to do what you are likely to achieve during your rehabilitation Please be aware that: your therapy programme is time-limited, and therapy can be provided for up to 12 weeks depending on your need if you fail to attend your appointments, discharge will be considered We need you to: be actively involved in your therapy programme follow the guidance of our specialist staff contact us if you are unable to attend an appointment.
3 Our specialist staff There are different specialists who may be involved during your time with NETT. We have outlined some of the people you may meet and how they can help you. Speech and language therapists: Helping you communicate with friends, family and other people Helping you eat and drink safely Occupational therapists: Enabling you to improve or manage how you carry out activities such as personal care, meal preparation, shopping and leisure activities. Enabling you to explore particular roles like parent/carer, or roles in employment Neuropsychologists: Supporting you and your family to understand more about your condition Exploring your memory, attention, planning and organisation Helping you cope with anxiety and depression
4 Physiotherapists: Increasing your confidence and ability to move around your home and in the community Increasing your general fitness Rehabilitation assistants Supporting the programme set by your therapists Our ethos We follow the Bridges self-management approach to therapy. This will enable you to feel confident in managing your own care now and in the future.
