3 5K WALK/RUN FOR SICKLE CELL We cordially invite you to participate as a Sponsor of the 5K Walk/Run for Sickle Cell which will take place on Sunday, September 9, This event will benefit the Sickle Cell Alliance Foundation ( whose mission is to bring awareness, inspire hope and advocate for families coping with the many facets of Sickle Cell Disease. Your sponsorship helps to defray the cost of organizing the event and in turn will give you exposure to the Forest Park community and sickle cell patients, caregivers and loved ones. Funds received from the sponsorships are used to purchase participant event day items. Any additional money from sponsorships will help towards creating transitional, life skills and educational programming to change the life expectancy for the sickle cell community. There are various levels of sponsorship for the 5K Walk/Run for Sickle Cell. Size and placement of logos on t-shirts, website and print materials are based on sponsorship level. ABOUT SICKLE CELL ALLIANCE FOUNDATION Launched in May 2014, Sickle Cell Alliance Foundation is a passionate 501(c)3 nonprofit, community-based organization. It is committed to being a valuable resource to guide and help families and patients children, teen and adults living with the lifethreatening sickle cell disease. Our goal is to ensure the appropriate programs and needed resources are available and offered to the patient and caregiver. Page 3 of 5

4 Event Overview Date: Sunday, September 9, 2018 Location: Central Park, 1201 West Kemper Road Start Time: 8:00am - Packet pick up and check-in will begin End Time: 12:00pm Enjoy after-walk festivities Run Donations: Registration opens 7/1/18! Online registrations: $25 for adults online / $30 day of event $20 per person for groups of 10+ (must pre-register online) $8 per person for youth groups of 10+ (must pre-register online)* $10 for youth (ages 6 to 17) / $15 day of event $5 for toddler (ages 5 and under) / $10 day of event *No t-shirt provided; youth will be encouraged to wear the shirt of their school or organization. All pre-registered walkers will receive a t-shirt and packet, unless otherwise specified. Please specify adult or youth t-shirt size when registering. No exchanges will be available on event day. NOTE: T-shirt is not included for walkers who pay on event day. Accepted event day payment methods: cash, credit card or checks. Course/Timing: Parking: Prizes: Publicity: MEMORY MOMENT: This is an official, measured 5K course. The course will begin at Central Park in Forest Park, run through the Forest Park neighborhood and end at Central Park. Water will be provided on the course. Course map will be posted at Parking is available at the Winton Woods High School (near the Athletic building), 1231 West Kemper Road; Municipal Building, 1201 West Kemper Road, and the Forest Park Senior Center, Winton Road. Medals will be given to the first 3 male and female overall finishers. Press releases will be submitted to local TV stations, radio stations and newspapers. Register online at scafcincy.com/5k-walk. Sickle Cell Alliance Foundation will share event information, advertisements, forms, post-event photos online at scafcincy.com/5k-walk, as well as on our social media pages. For just $5 on the day of the walk, customize a ribbon and place it on our WARRIOR TREE to celebrate your loved one(s) who live(s) or passed away from sickle cell. Page 4 of 5
