2 WHAT IS APHASIA? A COMMUNICATION DISORDER Aphasia (pronounced uh-fey-zhuh) is a condition caused by injury to the brain and affects a person s ability to communicate; it is most often the result of stroke or head injury. People with aphasia may have difficulty expressing themselves when speaking, difficulty understanding the speech of others, and difficulty reading and writing. Although intelligence is not affected, aphasia often results in frustration and isolation for competent adults capable of making decisions but overshadowed by their inability to communicate feelings, thoughts and emotions. Aphasia Recovery Connection offers social and educational programs to overcome these obstacles and allow people with aphasia and their families to re-engage in life. 6 APHASIA RECOVERY CONNECTION OUR STORY In 2012, two young stroke survivors who lost their ability to speak, read, or write, were introduced and started a Facebook group to encourage others with the same disability to connect online. Today, that group has expanded to more than 3,000 members and evolved into an impactful nonprofit organization called Aphasia Recovery Connection (ARC). ARC s founders understand firsthand how aphasia can impact quality of life for survivors, even many years after stroke and are dedicated to helping others overcome the isolation that aphasia brings. David Dow & Christine Huggins stroke survivors and founders of ARC OUR WORK Since its inception, ARC has convened people recovering from stroke or brain injury, their families and friends, together with rehabilitation providers to offer support and socialization thus decreasing the isolation that often results from aphasia. During the past two years, ARC has facilitated a week-long stroke and aphasia conference in Las Vegas, a two-day educational retreat in Ohio, an aphasia training for medical providers, five cruises with destinations ranging from Alaska to the Bahamas, over 100 group video interface opportunities for structured conversation, and daily online interaction through multiple social media channels. Although there are other organizations addressing aphasia, ARC is the only one approaching it by putting the patient and caregiver perspective first. All ARC activities are designed by people with aphasia and leverage the evidence-based Life Participation Approach to Aphasia (LPAA) with interventions that encourage re-engagement in life by strengthening participation in activities of personal choice. OUR MISSION The mission of Aphasia Recovery Connection is to deliver compassionate support services to improve the quality of life for people recovering from aphasia and their families and friends. We are committed to helping end the isolation that aphasia brings. We embody the values of collaboration, compassion, dignity and acceptance. 6

8 Ohio Retreat 2015 Sponsorship Agreement This agreement ( Agreement ) is entered into this day of, 2015 ( Effective Date ) between Aphasia Recovery Connection ( ARC ) and ( Sponsor ) to set forth the terms and conditions upon which Sponsor agrees to support the 2015 Ohio Retreat ( Retreat ). 1. General. The Ohio Retreat is part of the Aphasia Recovery Connection effort. ARC is a non- profit organization exempt from federal tax pursuant to Internal Revenue Code section 501(c)(3), federal tax identification number Term. This Agreement will begin on the Effective Date set forth above and end on July 30, 2015, the scheduled date for completion of Retreat activities ( Term ). 3. Event Date. ARC will use its best efforts to conduct the Retreat event from July 28, 2015 to July 30, Logo Use. ARC hereby grants Sponsor a limited, non- exclusive sublicense to use the Aphasia Recovery Connection name and accompanying logo solely to promote the Retreat during the Term of this Agreement. Sponsor shall not sublicense or transfer the use of the ARC name or logo to any person or entity without the prior written consent of ARC. Sponsor will present to ARC, for its prior approval, any item or material that uses or refers to the name or logo. All advertising and promotional materials should refer to Sponsor s relationship to the Retreat in the following form: Sponsor of Aphasia Recovery Connection s Ohio Retreat. Sponsor shall not use the ARC name or logo in advertisements or promotions that contain a reference to any entity which is not a Retreat sponsor. 5. Sponsor Licensed Marks. Sponsor grants ARC a limited, non- exclusive license to use Sponsor s name, logo, service marks and trademarks ( Sponsor Licensed Marks ) solely for including Sponsor in listings and descriptions of Retreat sponsors during the Term of this Agreement. ARC shall not sublicense or transfer the use of the Sponsor Licensed Marks to any person or entity without the prior written consent of Sponsor. 6. Sponsorship Benefits/Payment. Sponsor shall receive the sponsorship benefits set forth within this Agreement. Sponsor shall pay its sponsorship fee as it appears within this contract on or before June 15, 2015, and failure to do so shall result in a forfeiture of Sponsor s sponsorship rights. ARC may require Sponsor to provide additional documentation to support the value of in- kind donations prior to accepting such donations as credit toward Sponsor s sponsorship fee. To comply with IRS requirements, we wish to advise you that the tax- deductible amount of your contribution for Federal Income tax purposes is limited to the excess of the amount of money plus the value of any non- cash property contributed by you over the value of the goods and services provided by ARC. 7. Event Cancellation. Neither Sponsor nor ARC shall be responsible for damages that result from delays or postponements of the Event due to circumstances beyond their reasonable control. In the event that the Event does not take place, Sponsor s sponsorship fee as set forth above shall be treated as a donation to ARC and shall not be refunded. 8. Relationship/Entire Agreement. The parties to this Agreement have no legal relationship other than as contracting parties to this Agreement. This Agreement represents the entire agreement between the parties and supersedes any prior understandings or agreements. 9. Indemnity. Each party agrees to indemnify and hold the other harmless from and against any and all expenses, including reasonable attorneys fees, that the other party may incur by reason of any claim arising out of the indemnifying party s gross negligence, intentional misconduct performance or failure to perform pursuant to this Agreement, or any service or product sold or provided by the indemnifying party in connection with the Retreat. All individuals provided by or associated with Sponsor who perform services at the Retreat shall perform such services at the direction of, under the supervision and control of, and for the benefit of Sponsor. Such individuals shall not perform such services on behalf of ARC and shall not be agents or representatives of ARC. Sponsor shall be responsible, as between Sponsor and ARC, for any injuries or damages caused by or to said individuals. 10. Governing Law. This Agreement shall be governed by the laws of the State of Ohio. 8
