1 Cardiff and Vale University Health Board (UHB) is working with citizens, patients and partners to make it a reality that a person s chance of leading a healthy life is the same wherever they live and whoever they are. A proposal is being developed to provide a Wellbeing Hub at the Park View site to accommodate a range of health and wellbeing resources, with close links to the Ely hub and other community settings. The new development will be a focus for integrated working, and will link closely with other health and social care providers within the Cardiff SW Cluster area with an aim to increase resilience among the local population. The integrated working will be between all partners, including members of the local community. The new centre will aim to set the tone for community engagement in addressing wellbeing and will actively encourage people to take a full and active role in their future health care. Karen Pardy- GP/ Community Director Cardiff SW Cluster Within Cardiff and Vale of Glamorgan we have three Localities: Cardiff North and West, Cardiff South and East, and the Vale of Glamorgan. Each Locality has three Primary Care Clusters. These are collaborative Primary Care groupings who support the UHB in planning and delivering services for local communities; including GPs, Dentists, Optometrist and Pharmacists. Local authorities work within Neighbourhoods which are geographically the same as Clusters. Within each of the three Localities we would like to create a Health and Wellbeing Centre Within each of the clusters we would like to create Wellbeing Hubs
2 Work in a way that promotes co-production, codesign and co-ownership Propose: to provide the major physical infrastructure required to support improved access to community services and assets, to improve health outcomes, to set the tone for co-production and ultimately reducing health inequalities. How do we Achieve This? Scope: Wellbeing services, lifestyle information and education, signposting, etc. Existing range of community based services. Shift of clinics from hospital, both routine services/interventions and transformational innovative services and supporting diagnostics, therapeutic services, IT/health technology. By locally reflecting the four Shaping Our Future Wellbeing principles, and applying them to the Wellbeing Empower the Person Maintain a focus of prevention and wellness rather than illness Home First Support the delivery of care to individuals close to their home, providing individuals and families with better access to health services that do not need to be delivered in a hospital setting Provide an environment for individuals, families, patients, staff and visitors that embeds the values of collaboration, caring and respect. Outcomes that matter to People Facilitate the delivery of services that are relevant to the priority health, social and wellbeing needs of all individuals and families who live within the cluster Avoid harm, waste and variation Maximise all opportunities to ensure that individuals and families receive the right support at the right time, from the right person- acknowledging the invaluable contribution made from community groups, voluntary sector as well as statutory services
3 Wellbeing is at the heart of the Shaping Our Future Wellbeing: In Our Community Programme it's in the title! Whilst consideration of the 5 ways to wellbeing is being taken throughout the programme, they are most evident as we develop the vision for our Wellbeing Hubs. These are facilities located within communities focusing on providing services which offer a more than medical approach to addressing an individual s health and wellbeing needs. In practice this could mean that in one visit to your local Wellbeing Hub, you could chat to the physiotherapist about managing your back pain, followed by a mindfulness class with a friend helping you to focus on the things that are important to you, and finally take an Into Work computer class 4 of the 5 ways to wellbeing... The Five Ways to Mental Wellbeing The first is to Connect with People around you. This could be with family, friends, neighbours or people at work. These people can be the cornerstones of your life, so invest time in building connections with them. The second is to Be Active. Swapping inactive pursuits with active ones will make you feel good and improve your health. Find something you enjoy and which suits your level of mobility and fitness. The third is to Take Notice of your surroundings. Be aware of the world around you and what you are feeling, and reflect upon your experiences to help you understand what matters to you. The fourth is to Keep Learning. It could be trying something new, or rediscovering an old interest. Learning new things can be fun and will make you feel more confident. Finally, Give. Do something nice for a friend, or a stranger. Volunteer your time, join a community group. Seeing yourself linked to the wider community can be very rewarding, and creates connections with the people around you. New Economics Foundation (NEF)
4 Wellbeing The UHB has held a number of workshops with a wide group of people who use our services, as well as clinicians and partners who provide them. This information, along with an assessment of need and opportunity in our communities, is being used to inform the likely services which will be housed in Heath and Wellbeing Centres and Wellbeing Hubs. Alongside this, the UHB is undertaking more focussed work to establish exactly what services should be provided in specific locations. Co-production Whole system change can provide a way forward with social prescribing, time banking, volunteering, and an asset based approach to produce meaningful outcomes and build individual and community resilience. To facilitate this approach professionals / organisations need to work in partnership, and doing with as opposed to doing for or doing to individuals and communities. Assets based approach is an integral part of community development and involves facilitating people and communities to come together to achieve positive change using their own knowledge, skills and lived experience of the issues they encounter in their own lives.
