This year will be a slightly earlier start which gives you a VIP experience of the zoo before the public arrives! The Walk registration will begin at 7:30am and the Walk will start at 9:00am.

2 Hello Wonderful Wish Family! You are personally invited to join us for our Walk for Wishes 5 th year celebration! This year, for our Philadelphia Walk on Sunday October 11 th, we will once again be returning to the Philadelphia Zoo! This year will be a slightly earlier start which gives you a VIP experience of the zoo before the public arrives! The Walk registration will begin at 7:30am and the Walk will start at 9:00am. This packet will provide you with all the necessary information about the Walk and how to start registration! This year, all of our wish families are required to complete the registration form in this packet and return to: Make-A-Wish Attn: Walk for Wishes 5 Valley Square Suite 210 Blue Bell PA Once we receive your Registration form, we will enter you and your family into our Walk site and will send you the log-in information via . By participating in the Walk you will be able to inspire the Wish children with your dedication and integrity for an incredible experience. Your support will raise funds to grant the Wishes of children with life-threatening medical conditions to enrich the human spirit with hope, strength and joy. As a Walk for Wishes participant, you will be responsible for: Setting a personal fundraising goal Supporting your Team in a variety of methods Spreading the word about the 5 th Annual Walk For Wishes and encouraging friends, neighbors and co-workers to join your team or to get involved Getting creative with your Team on Walk Day by wearing Team shirts, buttons and carrying Team banners The Philadelphia, Northern Delaware & Susquehanna Valley Walk for Wishes 2015 website is philasv.wish.org/walk. 3 WHAT: WHERE: Make-A-Wish Philadelphia, Northern Delaware and Susquehanna Valley 5 th Annual Walk For Wishes Philadelphia Zoo 3400 W Girard Ave Philadelphia, PA WHEN: Sunday October 11, :30am Registration begins (registration will close at 8:45 sharp!) 9:00am Walk Start WHY: WHO: FEES: The reason we walk is to raise funds to continue to grant wishes for children in the Philadelphia, Northern Delaware and Susquehanna Valley area with life-threatening illnesses. The Walk for Wishes is also an opportunity to interact with Wish families, learn about Make-A- Wish and most importantly Have Fun! This event is open to our Wish Families, Friends, Community Members, Corporate Teams and School Teams. All are invited to attend! Entry fee includes all-day admission to the Philadelphia Zoo, a Walk for Wishes t-shirt, Wish experience necklace, a light breakfast, raffles, crafts, entertainment and a Walk through the Philadelphia Zoo! Registration: Wish child and children under 11 years old are both free. All participants (except the Wish Child) 12 years and over: $ Parking: Wish Families receive a free parking voucher (1 per car). Regular parking charges are $15 per car in any Zoo lot.

5 I wish to participate in the Walk for Wishes. I understand the acceptance of the waiver is required to participate in the event. I further understand that I am solely responsible for my health and safety and I acknowledge that I am physically capable of participating in and completing this event. I agree to abide by any decision of an event official relative to my ability to complete this event safely and I further agree that event officials or volunteers may authorize necessary emergency treatment for me. If I am injured as a participant in the event, I agree to assume all risks and to release and hold harmless Make-A- Wish Foundation of Philadelphia, Northern Delaware & Susquehanna Valley and its officers and representatives. I agree to allow Make-A-Wish Foundation and their contractors, agencies and sponsors, the use of my name and likeness in connection with this event, for any purpose related to advertising or promotion of the event worldwide in perpetuity in all forms of media. I have carefully read this Waiver and release and fully understand its contents. I am aware that this is a RELEASE OF LIABILITY and a contract between me and the persons and entities mentioned above and that I accept of my own free will. If the participant is under 18 years of age at the time of registration, the participant s parent or legal guardian must completely review this Waiver and Release. The parent or legal guardian understands and consents to its terms, and authorizes the participation of the registrant by his/her acceptance below.
