1 2017 Social Service Funding Application - Special Alcohol Funds Applications for 2017 funding must be complete and submitted electronically to the City Manager s Office at ctoomay@lawrenceks.org by 5:00 pm on Tuesday May 31, Applications received after the deadline or not following the attached format will not be reviewed by the Social Service Funding Advisory Board. General Information: Each year, the City Commission considers requests for the allocation of dollars to a number of agencies that provide services benefiting the Lawrence community. These funds are to be used pursuant City Charter Ordinance 33, which states, Moneys in the special alcohol and drugs programs fund shall be expended on such programs, services, equipment, personnel, and capital expenditures as the governing body may from time to time determine is in the best interest of the public to address one or more of the following: (a) Prevention of alcoholism and drug abuse, including but not limited to education, counseling, public informational efforts and related activities; or (b) Alcohol and drug detoxification efforts and related activities; or (c) Intervention in alcohol and drug abuse or treatment of persons who are alcoholics or drug abusers or are in danger of becoming alcoholics or drug abusers; or (d) Law enforcement, prosecution, court activities and programs, or portions thereof, related to apprehending, prosecuting, adjudicating or monitoring individuals who are alcoholics or drug abusers or are in danger of becoming alcoholics or drug abusers, including individuals who are or may be charged with violating laws related to alcohol or drug use; or (e) Education, counseling, public information efforts, and related and associated activities related to preventing drug abuse and alcohol abuse, including but not limited to efforts to encourage healthy youth and family development and related efforts which include as a partial element drug abuse and alcohol abuse education, counseling, or public information efforts; or (f) Programs, activities, or efforts related to preventing or intervening in drug abuse and alcohol abuse, including programs, activities, or efforts for which drug abuse and alcohol abuse prevention or intervention comprises a partial element of the complete program, activity or effort; or (g) Any program, activity, or effort, or a portion thereof, that the governing body determines seeks to discourage, prevent, intervene, or address issues related to alcohol or drug abuse. The appropriation of funds by the governing body for such a program, activity, or effort shall be conclusive of compliance with provisions of this ordinance, and separate findings shall not be required. Applications will be reviewed by the Social Service Funding Advisory Board at meetings held June 7 from 8:30 to 12:30. Applicants are asked to make a contact person available by phone during this time in case questions arise. Following their review, the Advisory Board will forward recommendations for funding to the City Commission. Recommendations will be based upon the following criteria: availability of city funds the need demonstrated through the agency s application the stated objectives of the applicant s program alignment of the program with City Charter Ordinance 33 the efforts to collaborate and avoid duplication of service demonstrated through the application outcomes that move program participants from total dependency toward measurable levels of independence ability to measure progress toward the program objectives and the City Commission Goals past performance by the agency in adhering to funding guidelines (as appropriate) The final decision regarding funding will be made by the City Commission when they adopt the Annual Operating and Capital Improvement Budget in August. Other Information. Collaboration and/or coordination between organizations is highly recommended and multi-agency proposals to address an identified community problem are encouraged. Programs should have research based effective strategies or promising approaches. All programs must have goals with measurable outcomes. Please note that funds will be disbursed according to the following schedule unless otherwise agreed to in writing: o First half of funds will not be disbursed before April 1 o Second half of funds will not be disbursed before October 1 The budget picture from the Kansas Legislature continues to create uncertainty as to the status of the local portion of the alcohol liquor tax. If the Legislature decides to eliminate or reduce the local portion of this tax, it will impact the City s ability to fund programs. The City of Lawrence is proceeding with the Request for Proposal process and accepting applications for the alcohol tax revenues with the understanding that funding levels are potentially subject to reductions by the State Legislature. REQUEST INFORMATION The criteria for each application questions are explained below. A. Amount of funds requested from the City for calendar year 2017 B. Provide a brief description of the program. Summary of program should be clear and concise. How is program consistent with City Charter Ordinance 33? Is program based on proven effective strategies or promising approaches? Description should address how program is designed to have long-term effects on a person s decisions about alcohol and other drug use. When appropriate, program design takes into account the person s family and/or community. C. Provide a brief description of how the need for this program was determined. The need for the program should be clearly established and outlined. When appropriate, the context of other services available, or the lack thereof, should be provided. Statistical data that supports community need should be provided. D. Describe the desired outcomes of this program (see Logic Model). At least one Process, one Behavioral and one Impact Outcome must be included and clearly outlined on Logic Model. Outcomes must be measured by an identified evaluation tool. Does agency show adequate progress towards achieving their targeted outcomes if previously funded. E. Describe any coordination efforts your agency has made. Program strategy should involve a coordinated approach. Description should address how this particular program or service fits with other existing efforts to reduce fragmentation and duplication. F. Describe how your agency is capable of implementing the proposed program. Based on agency history, management structure, and staffing pattern, does organization have capacity to implement the proposed program and achieve the desired outcomes? G. Provide a detailed budget for the proposed program using the categories provided. The budget request for the program described should be reasonable and spelled out according to the categories below. Does the organization use funding creatively to get the most for the money, i.e. use of students, volunteers, in kind donations, leveraging of this funding to get other funding, etc.? Are long-term plans for program funding discussed will this be an annual request for alcohol tax, is it expected to increase, decrease, etc.? Are other sources of funds used for the program? 3 Section 2 Requested Information A. Hearthstone is requesting $7,000 for the year B. Hearthstone is a community-based program to help indigent men recover from alcohol and drug addiction. It is run by a board of directors who themselves are alcoholics and addicts. Board members sobriety runs from 3 years to more than 30 years. We are mostly self sufficient, excepting requests such as this one for support from the Alcohol Tax Fund. Our focus is entirely on sobriety through 12- step programs. Full time employment and personal responsibility are an important part of our approach to re-integrate addicts into the community. C. Need for Program. There is no shortage of alcoholics and drug addicts in Lawrence, Kansas. We have 10 beds available and most commonly have 8-10 residents at any time. We are continuously contacted by rehab centers, parole officers, prisons, jails, de-tox facilities and private individuals looking for placement. We would love to go out of business due to lack of customers, but that will not happen any time soon. D. Desired Outcomes. E. Efforts at Coordination. We are in frequent communication with treatment centers, de-tox centers and parole officers. We also have a strong presence in Alcoholics Anonymous and Narcotics Anonymous. F. Capability of Agency. Hearthstone has been in operation for more than 30 years. During that time we have maintained services without interruption and have paid all our bills on time. We will continue to do so, using our unique model of self-sufficiency along with volunteer management from the Board of Directors. G. Detailed Budget. Hearthstone has no employees of any kind. No money is spent for travel, office space or fringe benefits. All of our expenditures are directly related to running the house. Budget figures from 2016 are below. It is obvious that Hearthstone operates on a thin margin, sometimes dipping into our reserves to keep the house afloat. This is by design. We take the Not For Profit designation seriously. More importantly, our residents are indigent. The come to Hearthstone with no funds, indeed, often owing to the courts and debt collectors. It is vital we keep costs as low as possible, and we do. Hearthstone offers excellent food, prepared by the residents, and a very nice home with prices that cannot be matched in Lawrence. We get the most out of our money through an allvolunteer board that does much of the home maintenance ourselves. 3

4 We have been in operation for over 30 years and will continue. We will ask for Alcohol Tax Funds in the future to help keep costs low and to prepare for contingencies to a home that is some 130 years old. We feel this is an appropriate use of the Alcohol Funds because we are the only agency whose sole purpose is 24-hour care and recovery for alcohol and drug addicts. To accomplish that we have intermediate goals for Hearthstone residents. * Men must maintain continuous sobriety. * Residents must attend 5 meetings in-house and at least 4 meetings per week of a 12-step program. * Shortly after moving in, men must find and keep full time employment. Rent and food bills must be current. * Residents must commit to a minimum stay of 90 days. 4

5 SECTION 3. LOGIC MODEL Please complete the Logic Model below. Assessment Data One hundred percent of Hearthstone residents are late stage alcoholics and addicts. The men are self-selected after every other alternative has been tried. After a group interview nearly every applicant is accepted. Program Goals and Objectives Our primary purpose it to help residents achieve sobriety while living at Hearthstone, and to maintain a sober life once they have left. Target Group Hearthstone works with indigent alcoholic and addicted men who are in early recovery. Strategies Hearthstone s structure of having a board of directors, all of whom are alcoholics or addicts, is unique as far as we know. Every board member is in the house weekly, at a minimum, and is directly involved in running Hearthstone. Board members lead daily meetings, set policy and ensure regulations are followed. The program is based on the 12-Steps of Alcoholics Anonymous and Narcotics Anonymous. Every week residents attend 5 house meetings per week and at least 4 meetings outside of the house. Residents are required to get sponsors and fully participate in addiction recovery. Residents also run and maintain the house. As men grow in recovery they are given more responsibility and freedom of action. Process Outcomes While in residence, every man fulfilled the requirement for attending house meetings and outside meetings. Every man met the requirement of full employment, allowing for a short while to find a job. Every resident maintained a positive balance for room and board, excepting brief periods when they were looking for employment. Behavioral Outcomes This is a snapshot of Hearthstone residents from January 1, 2016 through May 23, * Seventeen individuals were served. * The combined total of weeks in residence for these men was 372, including residents who arrived last year. * Of the 17 residents served this year, nine met their commitment of 90 days. * Five men stayed for less than 2 weeks, though they were believed to be sober when they left. * One man stayed for 6 weeks, falling short of his commitment, but apparently leaving sober. * One man was expelled for drinking while in the house and earned a lifetime ban from Hearthstone. * Residents manage daily chores and are responsible for keeping a neat and clean home, as well as cooking, shopping and outdoor maintenance. 6 Impact Outcomes If every Process and Behavioral targets are met then Hearthstone residents will go on to be sober citizens who work jobs, care for families and make living amends for the chaos caused when they were active addicts. For our residents this means that those who once cost society money and peace, who left disorder in their wakes have turned around and become contributors for a better society. In fact, there are many examples of Hearthstone residents who have done exactly that, men who have succeeded in the trades and professions, who have rebuilt families and help others in recovery. We do know this is not true for everyone who leaves Hearthstone, even those who leave sober. Addiction is a stubborn problem without a perfect solution. However, we remain inspired by the many who started on the path to recovery at Hearthstone and who are walking that path to this day. 6
