2 OUR MISSION OUR MISSION Our mission is to build healthier lives, free of cardiovascular diseases and stroke. That single purpose drives all we do. The need for our work is beyond question. WE'RE WORKING AS A CATALYST FOR COMMUNITY IMPACT & CHANGE At the American Heart Association, our mission is to create a world free of heart disease and stroke the No. 1 and No. 5 killers of all Americans. In Colorado, we are improving and extendi ng the lives of more than 5.5 MILLION people by building a culture of health in our community. Here's how we're working to keep Colorado at Heart. 111 EMS and hospital programs in the area received quality healthcare awards for participating in our Get With the Guidelines programs. 22, people and 125+ companies participated in AHA walking events last year in Colorado. area employers participated in our workplace wellness initiatives. We also provided $14.7 million in funding for 46 projects in Colorado. 278,000 adults and teenagers were trained last year in CPR, AED usage and basic life-saving skills using AHA guidelines, methods and materials. 10, Coloradoans have joined our advocate networks and coalitions.

4 COACH CHECKLIST Step #1 Plan for Success Register as a coach at Set your team's #DenverHeartWalk goals. Recruit at least 15 walkers for your team. Step #2 Know Your Team Find a Heart Hero - a walker that can raise $1,000 or more. Rally your team for a heart walking activity! Have questions? 5 STEP 1 Register If you have not registered, click the REGISTER button and complete the registration process. Click on Start a Team Click on Choose Your Company and enter your desired team name. If you are registered, click the LOGIN button and enter your Username and Password. STEP 2 It's Your Space. Customize it. Click on EDIT PAGES. Walkers can edit their page and coaches can edit team pages. Nobody likes stock photos! Upload your own. Make it personal. Tell the world your story. Page personalization can increase donations by up to 7 times! Personalize the stock video, or upload your own from YouTube. Tell us your "why" in your own words. 5

6 STEP Shout it from the rooftops - digitally! Send s to recruit walkers & fundraisers to join you. Click SEND . Enter addresses. Time Saving Tip: import addresses from Gmail, Yahoo or Outlook Got the addresses? Good. Click CONTACTS, check the box, click SELECTED. Click the dropdown to CHOOSE A SAMPLE MESSAGE. Customize the message. Your own words are powerful. DON'T FORGET! Add a subject line before you click out or the draft won't save. Now, let's take a look. Click PREVIEW & SEND. Like it? So do we. Click SEND . STEP 4 Let's Get Social. Fundraising with social media. It's a social media world. The quickest way to fill up your team and donation bank is by asking your social networks to support you. Log in, click My Dashboard and scroll down. On the left, you ll see these icons. Click any of them to share your page! In the Dashboard, you will also find a Go Social section featuring tips & tricks on how to maximize your efforts! 5 6

8 LIFESMILE CHANGE AWARD Delta Dental of Colorado and the American Heart Association have partnered together to bring you the LifeSmile Change Award. The award will be presented to individuals who have made the commitment to make positive changes to improve their quality of life and overall health.
