3 Lessons Learned Planning for a Patient Navigation Program that Works Need to distinguish from other healthcare professionals, including Case Managers Navigators are focused on addressing perceived barriers to care, rather than providing a particular service. Strength based approach. Rather than looking at what is wrong in an community and trying to fix it, LAF is taking a approach that builds on the community s unique resources, opportunities and strengths. Apply SOAR (Strengths, Opportunities, Aspirations, Results) analyses in place of more traditional SWOT analyses. Importance of culturally competent care (not just language translation).

6 LIVESTRONG PNSP Patient Navigation is a strategic priority for LAF. Goal of the PNSP is to leverage synergies between existing national initiatives and encompass new initiatives to provide comprehensive navigation services to people affected by cancer. LIVESTRONG SurvivorCare, the LIVESTRONG Care Plan, the LIVESTRONG Survivorship Center of Excellence Network, and the LIVESTRONG Patient Navigation Center are all considered integral initiatives within the PNSP.

7 LIVESTRONG Patient Navigation Center In 2009, the LAF actively began to plan for the LIVESTRONG Patient Navigation Center (PNC), which will be housed in LAF headquarters. It is scheduled to open in Extensive research and analysis of existing Patient Navigator Programs being done. Do not want to compete with or duplicate other programs. Carve out a distinguishable niche that is meaningful to stakeholders. Building a systems based model, irrespective of cancer type. Planning a hybrid staffing model. If LAF is able to demonstrate strategic success, the PNC can potentially be replicated and branded outside of Austin, due to it being a systems based model.

8 LIVESTRONG SurvivorCare Within the PNSP, SurvivorCare is one service delivery program that the LAF offers. It is a collaboration of direct services to provide free, one on one support in English and Spanish to anyone affected by cancer from the moment of diagnosis and throughout their cancer experience, including: Persons with cancer Caregivers Family Friends Health care professionals

10 LIVESTRONG SurvivorCare Collaborative partners include: CancerCare Emotional support and counseling Support groups Limited financial assistance Patient Advocate Foundation Employment and financial concerns Co pay assistance and insurance issues Emerging Med Clinical trials matching service

11 LIVESTRONG Care Plan LAF and Penn Medicine s OncoLife have created a co branded survivorship care plan that can be accessed on the Internet by cancer survivors worldwide at Ongoing iterations of the product, including ways to enhance multimedia components. Multiple technical upgrades have been done. Overall high satisfaction with product and usability. Lists LIVESTRONG cancer survivorship support services on homepage. Engaging ACS leadership regarding collaborative opportunities. Engaging in outreach through professional associations (i.e. AARP, Senior Centers, Public Health Corps, Hispanic Nursing Society, Community Centers, Library Systems, US Oncology). Need to better inform PCP s; will be spotlighted in fall issue of JGIM.
