2 What does the service provide? The service encourages and empowers homeless and vulnerable people to gain control of their own health and determine their own lives and future. We offer health advice, information and support to homeless and vulnerable people to empower and enable them to make informed decisions about their health. We also offer advocacy with regard to health related issues. The team monitors the health and well being of homeless and vulnerable families and works in partnership with appropriate agencies. The team acknowledges that the best way to meet the complex needs of service users is to work in partnership, believing that every person regardless of their age, race, religion, sexual identity, disability or lifestyle should have a safe place to live, and be empowered to lead a long, healthy life, free from disease and illness. 3 Team Mission Statement What do we offer? The Homeless and Vulnerable Persons Service believe that the key experts in the development of quality services for homeless and vulnerable people are clients themselves. It is the aim of the team to give users a voice in as many ways as possible and to ensure that their comments, views and suggestions are considered in all stages of service development. The team also acknowledges that the best way to meet the complex needs of the clients is to work in partnership with all relevant agencies and to welcome, respect and value the expertise and knowledge that other agencies can provide. Our belief is that every person, regardless of their age, race, religion, sexual identity, disability or life style should have a safe place to live. That they should be fully informed and empowered to lead long, healthy lives free from disease and illness. Children should be enabled to reach their full potential in a safe, happy family environment. Free access to all health services at the point of need. A quality service that meets all the needs of all homeless and vulnerable people and children allowing them to achieve their full potential and lead happy, healthy lives. An established after care programme. 4 Places accessed by the Homeless service Referral Pathway for Homeless Service user BOND STREET Temporary accommodation for offenders or people at risk of re-offending Homeless Service User BRECON CLOSE Provides safe and secure supported accommodation for year olds, on site training, assistance accessing employment and personal development opportunities, where young people receive the support they need to make the transition to independence. Attend Housing Options Advice Centre for Homeless Interview Allocation to Temporary residence, i.e. Tiernan lodge etc Referral to Homeless and Vulnerable Persons Service for health assessment and engagement with: COOPS FOYER Offers a safe, secure environment that provides young people aged years with accommodation and gives them the chance to access training and employment opportunities all under one roof. Mental Health Services Education Health & Social Care Services Floating Support Substance Misuse services HOLLY HOUSE Offers emergency and temporary accommodation, advocacy and support to women escaping abuse. It is staffed 24/7, 365 days a year with a telephone advice service also available outside of normal office working hours Discharge to permanent accommodation Follow on support visits by Homeless & Vulnerable Persons Service for up to 6 weeks, to assist with orientation to area and access to mainstream health care services. On discharge Homeless and Vulnerable Persons Service to notify all agencies involved with service users. LASARS (Leigh Asylum Seeker and Refugee Support) Is a drop in service, they give asylum seekers and refugees the opportunity to meet other people. They provide support and the chance to socialise and share experiences in a welcoming, safe and friendly atmosphere. Discharge from Homeless & Vulnerable Persons service.

5 THE BRICK DROP IN Wigan and Leigh areas Personalised support service & outreach for people sleeping rough or homeless. PRESTWICH HOUSE Temporary accommodation for offenders or people at risk of re-offending. RAILWAY ROAD Provides accommodation for 22 single homeless men/women with support needs. TRAVELLERS SITES Offering health provision to travellers: i.e. stop smoking etc, access to and arrangement of health appointments. Access to citizen advice services. TIERNAN LODGE Provides safe and secure temporary accommodation and support to those claiming homelessness LEIGH HOUSE Offers emergency and temporary accommodation, advocacy and support to women escaping abuse. SWAP (Support for Wigan Arrivals Project) Offers support to the members of our newest communities, and has a particular focus on helping refugees, asylum seekers and migrant workers from the new European states. As well as giving practical support and help with social integration to new arrivals, SWAP also offers training, support and signposting to service providers working with migrants in the borough who need to increase their understanding of the regulatory framework and social / cohesion issues affecting new communities in Wigan. Services are jointly delivered by staff and trained volunteers.
