2 WHO WE ARE The Habitude Program offers an authentic Bio-Psycho-Social approach with holistic principles and modalities. Since 2005, our in-patient program has helped individuals and their families with substance use disorders, mental health issues, and compulsive behavioral disorders. In addition to helping our clients, we also support the family and/or employer with extensive education to create stronger supports for the individual. 2
3 MEET THE TEAM HABITUDE S MULTIDISCIPLINARY TEAM Medical and Naturopathic Doctors Neuro Psychologists Nutritionists Psychotherapists Case Managers Addiction, Grief, Anger, Anxiety & Depression Specialists Withdrawal Management Specialists Family Therapists Life and Vocational Coaches Medical Exercise Specialists Yoga, Tai Chi and Auricular Acupuncture Instructors Therapeutic Art Classes We are an integrative multidisciplinary inpatient team, specialized in substance abuse and concurrent mental disorders (anxiety, depression, anger, post-traumatic stress and severe mental illness). In our program, we offer a treatment approach including the biological, psychological, social and spiritual aspects of substance abuse and compulsive behaviors. We provide, based on individualized assessments, quality of care and appropriate treatment according to the patient s needs in the different stages of recovery. During the first stage, the focus is achieving abstinence, preventing relapse, and managing cravings. While, In the second stage, we will guide the clients in to recognize, understand and manage their emotions (Anger, Guilt, Shame, etc.). It is during this time, that we are able to identify any concurrent disorders. Finally, during the last stage, our clients will adopt a more introspective, relational view of themselves, incorporating new skills, which will help to improve the communication with their family, friends, and co-workers. 3
4 GETTING STARTED It is mandatory that every client take part in the screening process before being accepted into The Habitude Program. This assessment will help to determine if the individual is a good candidate for the program. The Preliminary Interview can be done over the phone or in person. Should the individual meet the criteria to enter the residential facility, an initial assessment form will then be sent. Once completed and returned, our medical and clinical team will review. Acceptance into The HabitudeTM Program is determined by many factors. Following acceptance, a date and time for intake will be arranged. There is regular contact and support with the individual and their support prior to intake to help create a smooth transition into the program. HOW IT WORKS - STAGES BIOLOGICAL & PHYSICAL 01 According to the Substance Abuse and Mental Health Services Administration more than 20 million persons aged 12 or older needed treatment for illicit or legal drugs. Approximately, 10 percent of those who needed treatment received it at a specialized facility1. Our medical team works with our multidisciplinary team to help expedite the bio-physical detoxification process and provide our clients with a strategic plan for long term recovery. PSYCHOLOGICAL & EMOTIONAL The Center for Addiction and Mental Health (CAMH) published statistics mentioned that individuals with substance use disorders, excluding alcohol, are 4.5 times more likely to have a concurrent disorder compared to general the population2. Based on a client s assessment, our multidisciplinary team will be SOCIOLOGICAL Improving relationships with family, friends and co-workers is imperative to maintain a healthy recovery. Family therapy and education help able to identify possible co-morbidities and give a better understanding of the reason for the substance use disorder. During this stage, a qeeg is completed* this is to identify the areas of the brain that will need additional support via Neuro Feedback and Bio Feedback Sessions. individuals and their families to achieve an improved homeostasis and maintain a balanced lifestyle.
5 04 TRANSITION In order for individuals to transition back home, clients create a Healthy Living Plan which combines all aspects of the individual s life and how they will live without escaping to substances. Clients will learn how to reconnect to all the things that used to make them happy and leave with a feeling of worth and purpose. 6 PROGRAMMING CLIENT PROGRAMMING Clients will be assigned a personalized treatment team consisting of a case manager, addiction counselor, psychotherapist and family therapist. NEURO FEEDBACK PROGRAMMING* Dr. Chris Friesen uses the QEEG known as mapping the brain, to identify the areas of the brain that will need additional support. Neuro-feedback sessions along with the Bio-feedback sessions, are both cutting edge technologies that help Habitude s medical & clinical professionals identify the specific treatment direction needed to advance each individual safely, efficiently & effectively. HOLISTIC PROGRAMMING Our holistic Bio-Psycho-Social-Spiritual approach incorporates the engagement and development of the whole person, physically and psychologically by bringing any imbalance into alignment with its natural state of functioning. Clients will be given the opportunity to benefit from a multitude of holistic modalities and make an informed decision on whether they would like to include them in their recovery treatment plan. FAMILY EDUCATION WORKSHOP (PSYCH-EDUCATIONAL) In the first week, a family counselor will communicate with the primary contact to set up the Habitude Family Education Workshop. The workshop is empowering and helps individuals deal with the impact of addiction. This is scheduled within the first part of the program for the individuals who will be actively involved with the client s recovery. The comprehensive workshop is 4 hours in length initial session is 3 hours and the follow up session will be 1 hour. Visitation will not be permitted until the first segment of the Family Education Workshop is completed if this applies. FAMILY THERAPY The client s family will participate in these sessions, with the aim of improving communication and building or re-establishing healthy relationships. Family therapy sessions will take place at the residence or Video Conference Call. Family Education Workshop (#1) must be complete prior to this session happening. (This is available during inpatient treatment for programs starting at 30 days.) FAMILY COUNSELLING OUTPATIENT * In addition to family therapy - A family treatment plan may be recommended and will be comprised of one-on-one sessions, couple counselling and/or additional family therapy. Specific intensive workshops will be recommended for families wanting to reconnect and establish a healthy foundation. Various packages 6
7 and workshops are available and will be recommended during the intake process and throughout the program. *Please see Habitude s Family Wellness Packages CONTINUUM OF CARE/AFTERCARE* Once program is complete, we will communicate at 30, 60, 90 days as well as 6 and 12 months. Aftercare Packages are available via outpatient treatment and can be facilitated in-person, by conference call or video conference. See additional literature on our aftercare packages.
