2 Disclosure statement In this seminar I will refer to a chapter I have written in a book I have co-edited called Stammering Therapy from the Inside: new perspectives on working with young people and adults J & R Press I will receive royalties from the sale of this book.

3 Definition Mindfulness means paying attention in a particular way: Kabat-Zinn, 1994 on purpose in the present moment and nonjudgementally

4 Aims of mindfulness based approaches MBA s To increase the quality of life To help with specific difficulties e.g. depression, stammering, voice

10 Not trying to fix not goal oriented people want to reduce suffering letting go of the wanting fixing versus not fixing counterintuitive Summing up one key aspect of mindfulness is about cultivating a different relationship to difficulty

12 Kindness and compassion Mindfulness is an act of hospitality. A way of learning to treat ourselves with kindness and care that slowly begins to percolate into the deepest recesses of our being while gradually offering us the possibility of relating to others in the same manner. The process simply asks us to entertain the possibility of offering hospitality to ourselves no matter what we are feeling or thinking Santorelli, 1999

13 Jon Kabat-Zinn making ancient practices accessible to contemporary 21st century living the 8 week course mindfulness based stress reduction MBSR practices include bodyscan, mindful movement, awareness of breath and sitting meditation guided by CDs developed by Segal et al (2002) into mindfulness based cognitive therapy for depression MBCT (many other MBCT courses now including Cheasman (2013) mindfulness-based cognitive therapy for people who stammer)

14 Context and range of application offered in a secular context though derived from Buddhist practices demonstrated to be helpful to wide range of groups including depression, stress, chronic pain, chronic fatigue syndrome, oncology patients, eating disorders (Baer, 2005)

15 MBSR/MBCT vs CBT both emphasise that thoughts are just thoughts in CBT focus on working with content of thoughts in MBSR/MBCT emphasis on the relationship to thinking mindfulness approaches encourage a different relationship through developing an attitude of acknowledgement, acceptance and gentleness decentring

18 The mindful therapist - cont d Non-striving not trying to fix, analyse, sort out Acceptance willingness to see things as they are in present moment not struggling to change them Letting be noticing things come and go and coming to know our tendency to cling and resist

19 Some research studies Shapiro et al (1998) health professionals showed greater empathy to patients after mindfulness training Schure et al (2008) showed that health professionals had greater attunement to themselves and their patients as result of practising mindfulness Shapiro et al (2007) showed stress reduction benefits in health professionals Galantino et al (2005) showed reduced burnout

20 Relevance of mindfulness for SLT clients/carers increase in awareness cultivation of non judgemental awareness of thoughts, feelings and behaviours increased awareness of the body increasing ability to respond rather than react can facilitate use of speech therapy techniques cultivation of acceptance (not resignation) means whereby people can develop greater opening up to difficulty stress management/relaxation

21 I practice mindfulness on a daily basis and experience myself as a more balanced and creative therapist when I key into a kinder, more allowing relationship to myself. Matthew Mills SLT Mindfulness offers a chance to be with ourselves without goals, and in the absence of clinical notes or assessments. As such it allows patients to simply be, to open up to accepting themselves and their experience as it is Kate Gooch SLT

22 even a few minutes of mindfulness gives people an opportunity to find deeper awareness and acceptance of their immediate emotions and bodily state Christine Shewell SLT

23 Using my breath as an anchor, I can sit more intimately with a client's profound sense of loss, confusion or anguish, thereby enabling the unbearable to become a little more bearable. Through being more fully alive in the present, mindfulness has enabled me to acknowledge and develop my own humanity as well as connect more deeply with that of the people I work with Sam Simpson SLT

24 Feedback from people who stammer It helped me with the moment of stammering and seemed to enable me to slow my rate of speech down I have a greater acceptance of myself. Some existing conclusions, thoughts are now seen in a different way. New perceptions are forming that have a truth and integrity about them. I have a greater awareness and greater clarity.

25 Feedback from people who stammer am more able to catch stammering moments and choose how to manage them rather than trying to push through I find it easier to accept my stammer as part of me I feel whole and don t feel I am dragging my stammer around like a troublesome appendage I am kinder to myself
